Last year was such a successful cmmunity event that we are eagerly lking frward t this year. Our plans include an extended Fun Run fr all ages and abilities, musical entertainment, raffle and tns f infrmatin and prducts relating t Health & Wellness as well as an abundance f kid-friendly activities. Our beautiful venue and great Suthern Marin lcatin, as well as ample parking, will cntribute t make this a great event! VENDOR DETAILS T be an eligible vendr: Yu must prvide sme service that is applicable t Health & Wellness, Exercise, Sprts, Cmmunity Health, Sustainable Resurces, Healthy Eating/Fd, Gardening r Kid Friendly. Applicatin Fee f $60 (Refundable if yur bth is nt chsen t participate) gets yu a bth space apprximately 8 wide and 6 deep. 1 tables & 2 chairs are prvided. There are a few electrical utlet spaces available which cst an additinal $20. Fee includes bth space, prmtin f event, Facebk & website marketing. Event is held indrs in the Cascade Rm. Fun Run is held rain r shine utside. Please nte: bth spaces may vary depending n lcatin. All displays must be free standing and n taller than 8. If yu need additinal rm, yu must indicate yur request n yur applicatin. If yu are a massage r bdy wrk practitiner and wuld like t have a table in ur quiet rm t prvide free sampling f yur wrk (aka extra bth space) please indicate n applicatin. Prvide a free service r prduct during the event The purpse f the Health & Wellness Fair is t be active and engaging. All bths must prvide a free service/prduct (such as chair massage, fd samples r an activity, etc.). Bths that are merely prviding infrmatin will nt be cnsidered. Yu are allwed t als sell yur prducts in additin t prviding samples. Dnate a raffle item Each vendr is required t dnate an item representative f their bth with a value f n less than $ All mnies cllected frm the raffle will be used t defray csts f the event with any surplus ging t ur schlarship fund. Please nte each runner receives 5 free raffle tickets as encuragement t g t all f yur wnderful bths. The City f Mill Valley is required t cmply with Sectins 6073 and f the Califrnia Revenue and Taxatin Cdes. Therefre, all vendrs must fill ut the Swap Meets, Flea Markets r Special Events Certificatins fr (#BOE-410-D) that is attached. The frm asks fr a valid seller s permit fr anyne wishing t sell merchandise at this fair. If yu are exempt due t selling exempt prducts, r because yu are an

2 ccasinal seller (n mre than 2 events per year) yu d nt need a seller s permit but please mark the apprpriate bx n the frm. 4 Mill Valley Health & Wellness Fair Vendr s Agreement 1. Bths must be staffed n later than ne half hur befre the pening t the public at 9:00 a.m. and must remain pen and staffed during all Fair hurs. (9 2pm) 2. All unlading fr event must be in place by Saturday mrning at 8:00 a.m. 3. All bth structures must be remved by the clse f the Fair. 4. MVCC is nt respnsible fr yur bth, any items pertaining t yur bth nr will MVCC prvide any dumpsters fr yur materials. 5. Vendr agrees t cnfine all display and selling activity within the assigned bth space, refraining frm placing signs and gds r ther materials within r ver public aisle space, vendr aisle space, r in ther vendrs spaces. 6. Vendr agrees t abide by all applicable laws, rdinances, and regulatins pertaining t health, fire preventin, public safety, business licenses, and sales tax permit. 7. Vendr agrees t maintain the dignity and integrity f the Fair. The Mill Valley Recreatin Department reserves the right t ask any exhibitr/vendr t leave the fair if nt acting in the best interest f the Fair, r wh des nt meet vendr requirements. 8.
