1 2018 Healthy Biler Wellness Incentive Prgram FAQs As healthcare csts cntinue t rise acrss the cuntry, the university s best way t cmbat this is by helping t imprve the verall wellness f ur ppulatin. Therefre, as part f ur initiative t prmte wellness within ur ppulatin, Purdue is intrducing a new wellness incentive plan fr the 2018 plan year, effective January 1, By cmpleting all three bjectives listed belw, eligible emplyees and spuses will receive the respective funds int their Health Savings Accunt (HSA) r int a Health Reimbursement Accunt (HRA). 1. Select a primary care prvider - $50 emplyee nly/$100 emplyee & spuse 2. Cmplete a physical & bimetrics - $100 emplyee nly/$200 emplyee & spuse 3. Cmplete tw pre-apprved wellness activities - $100 emplyee nly/$200 emplyee & spuse Eligibility Q. Wh is eligible t participate in the prgram? All benefits-eligible emplyees as well as spuses cvered under ne f the Purdue health plans. Q. Are dependent children eligible t participate? N, nly benefits eligible emplyees and spuses cvered under ne f the Purdue health plans are eligible. Q. If I am nt n Purdue s medical plans, am I eligible t participate in the incentive prgram? Yes, nn-medical participants are eligible t participate. HSA, HRA, and FSA Q. Hw much mney can I earn? Up t $250 fr emplyee-nly participatin and up t $500 fr emplyee and cvered spuse participatin. Q. Hw will I receive the incentive dllars? If yu are currently receiving cntributins t an HSA, the funds will be depsited int yur accunt quarterly when eligible. If yu are nt eligible t receive cntributins t an HSA, an HRA will be pened fr yu and the funds will be depsited quarterly when eligible. An HRA will nly be pened fr thse wh are ineligible t receive cntributins t an HSA. Sme examples f this are as fllws: A persn n the Purdue Health Plan, a persn taking Medicare Part A, a persn pted ut f Purdue s medical plans/is n a health plan utside f Purdue. An individual with a FSA will receive a HRA.

2 Q. What is an HRA? An HRA is a Health Reimbursement Accunt. HRA accunts are similar t flexible spending accunts (FSA). The funds in this accunt can be used fr qualified medical, visin, prescriptin and dental expenses yu pay ut f pcket, like funds in a Flexible Spending Accunt (FSA). Q. Hw des the HRA get set up? Fr thse wh are nt receiving cntributins t an HSA, an HRA will be pened autmatically based n the quarterly incentive pay schedule if yu have cmpleted the first eligible bjective. Ex: If yu identify yur primary care prvider in January, an HRA will be pened n April 15 with $50 (r $100 if yur cvered spuse identifies their primary care prvider as well). Q. Hw will emplyees access HRA funds? Emplyees will receive a debit card frm PayFlex which can be used t pay fr eligible expenses. Emplyees wh are participating in a health care Flexible Spending Accunt (FSA) will use the same PayFlex card they received fr the FSA. Funds will first be spent frm the HRA, then frm the FSA nce the HRA is exhausted. Q. When will I receive the incentive dllars I have earned? The funds will be depsited quarterly: April 15, July 15 and Octber 15. A reprt in aggregate will be prvided t Human Resurces indicating hw much each persn shuld receive based n cmpleted items. Human Resurces will nt receive any persnal health infrmatin related t yur incentive cmpletin. Q. D these funds rll ver int the next year? If yu have an HSA, yes the funds will rll ver. If yu have an HRA, n. Yu will have until the end f the year (unless yu retire r leave Purdue, in which case yu wuld have until the date f separatin) t incur eligible expenses and until March 31, 2019, t submit claims fr reimbursement. Q. Des the HSA cntributins received by the incentive prgram apply t ur annual maximum? Yes, participatin and funds received int an HSA frm this prgram apply t yur annual maximum. Q. What if the additinal HSA cntributin exceeds the maximum? HSA s can be changed at any time during the year. Yu will need t change yur cntributins t avid the max. If yu g ver the max, yu ll have t pay taxes n any exceeding amunt.

3 Prvider, Annual, and Bimetrics Q. What if I dn t have a primary care prvider? Yu can find a prvider by ging t the Anthem website at r the Castlight website at Once yu chse a prvider, make an appintment t establish him r her as yur primary care prvider and t receive yur annual physical, including a bimetrics screening. Q. Can I designate smene at the Center fr Healthy Living as my primary care prvider? Yes. If yu are currently established with a prvider at the Center fr Healthy Living, yu may designate them as yur primary care prvider. If yu are nt currently established with a primary care prvider r yu wuld like t change prviders, yu may designate a Center fr Healthy Living prvider. Q. What cnstitutes an annual physical? The annual physical is a cmprehensive exam that is preventin fcused, nt prblem fcused. Typically the exam is perfrmed by yur primary care prvider and includes the fllwing: Past medical, scial and family histry Cmplete physical exam and review f bdy systems Review f medicatins Immunizatins Cunseling/anticipatry guidance/risk factr reductin interventins Review f age/gender apprpriate screening tests Q. What if I already had my annual physical? T qualify fr the incentive, yur physical will need t have been cmpleted between January 1, 2018, and September 30, Yu are eligible t get a physical even if it hasn t been a full year since yur last physical. Q. Will a health screening at a health fair cunt fr the incentive? N. The bimetrics frm needs t be cmpleted by yur prvider as part f the annual rutine physical. Q. Will my Mammgram, Clnscpy, r Pap smear cunt as my annual physical? N, a mammgram, clnscpy, r a Pap smear are typical testing that can be a part f yur annual physical but des nt cunt as receiving a cmplete physical. Q. Are the annual physical and bimetrics cvered by ur insurance? Yes, bth are cnsidered preventative. Preventatives are n cst t the emplyee.

4 Q. What is included in a bimetric screening? Bimetric tests required include: Bld Draw fr Chlesterl (TC), HDL Chlesterl, LDL Chlesterl, Glucse, A1C and Triglycerides Bld Pressure Height/Weight/Bdy Mass Index (BMI) Yu will need t dwnlad the Center fr Healthy Living Prvider Screening frm and take it with yu t yur annual physical. Yur primary care prvider will need t sign it befre yu uplad it t the Healthy Biler prtal as dcumentatin f cmpleting this step. Q. What if my primary care prvider wants t charge me t cmplete the bimetrics frm? Typically, a prvider will cmplete the frm as part f yur physical but sme may charge a fee. Yu can inquire abut yur prvider s plicy when scheduling yur appintment. The Center fr Healthy Living will nt charge fr the rutine physical and cmpletin f the bimetrics frm. Q. What will the bimetrics be used fr and wh can see them? The infrmatin frm yur physical and the results frm yur bimetric screening will be used t prvide yu with infrmatin t help yu understand yur current health and ptential risks, and may als be used t ffer yu services thrugh the wellness prgram, such as, but nt limited t health caching, medicatin therapy r weight management. Yu als are encuraged t share yur results r cncerns with yur wn primary care prvider. Q. D I have t cmplete the tasks in rder? N, yu can cmplete the tasks in any rder; hwever, task 1 must be cmpleted t be paid fr task 2 and tasks 1 and 2 must be cmpleted prir t receiving funds fr task 3. Fr example, if yu run a 5K in January, it will still cunt as an apprved activity but yu will nt receive funds until tasks 1 and 2 are als cmpleted. The same applies t task 2 and task 3. Fr example, yu can run a 5K and attend a wellness educatin wrkshp in January (task 3) and then get yur annual physical and submit yur bimetrics in February (task 2), but yu wn t receive the funds fr thse until yu have identified yur Primary Care Prvider (task 1). See the chart belw fr reference: Must cmplete task 1 t be paid fr task 2. Must cmplete tasks 1 and 2 t be eligible t receive payment fr task 3. Emplyee r Spuse Only Emplyee + Spuse 1. Identify Primary Care Prvider $50 $ Cmplete annual physical & bimetrics $100 $ Participate in a minimum f tw pre-apprved wellness activities $100 $200

5 Enrllment Q. Hw d we enrll fr the Incentive prgram? After the emplyee has registered n the wellness prtal, cmpletin f the bjectives will enrll the emplyee int the incentive prgram. Q. Hw des my spuse enrll? After the spuse has registered n the wellness prtal, cmpletin f the bjectives will enrll the spuse int the incentive prgram. Q. If I started a wellness prgram in 2017 and cntinue in 2018, will it cunt tward the incentive? If the wellness prgram is started in 2017 and is cntinued int 2018 while meeting the incentive criteria, the prgram will be awarded. Fr example, if yu are enrlled in a weight management series prir t January 1, 2018 and cntinue yur enrllment int 2018, nce yu cmplete at least three sessins, yu wuld have cmpleted the wellness educatin series categry. Q. Where can I g fr mre infrmatin n the Healthy Biler Wellness Incentive prgram? Visit the Healthy Biler Wellness Prgram prtal at T register, Click the REGISTER buttn (in gld) When prmpted, prvide yur first name (as it appears n yur pay stub) Prvide yur last name (as it appears n yur pay stub) Enter yur Emplyee ID number including tw leading zers, fr a ttal f 10 digits Cntinue with the steps as prmpted t finish registratin healthybiler@purdue.edu. Assistance is als available frm Human Resurces.
