AARP Husehld Discunt: 5% ff f the mnthly premium if tw members are n the same AARP membership husehld accunt and each is insured under an AARP-branded supplement insurance plicy with UHC. (Des nt apply t AARP Medicare Rx r AARP Medicare Cmplete plans.) Enrllment Discunt: The Enrllment Discunt is available t applicants age 65 and ver nly. Eligibility: Applicants are eligible fr the Enrllment Discunt if their age n their plan effective date is: 65 t 67, OR 68 t 74 AND their plan effective date is within 3 years f their Medicare Part B effective date, OR 68 t 74 AND their plan effective date is 3 r mre years but less than 6 years frm their Medicare Part B effective date AND they d nt have any medical cnditin that qualifies fr the Level 2 Rate Applicants age 75 and ver are nt eligible fr the Enrllment Discunt and will pay the standard rate. Discunt Percentage and Duratin: If applicants are eligible fr the Enrllment Discunt, the discunt percentage is applied t the Standard Rate. The first-year discunt percentage and the duratin f the discunt prgram will vary based n applicant s age as f the plan effective date (see table belw). The discunt percentage reduces 3% each year n the anniversary date f the applicant s plan until the discunt runs ut.*

2 After the eligible discunt duratin expires, applicants will pay the Standard Rate. 5% discunt fr 2 members enrlled under the same AARP ID *This chart applies t PA, MD, NJ, OH, DE, NY, and Washingtn DC. If the state in questin is nt listed, please refer t yur state s Prducer Handbk. Aetna Health and Life Insurance C. Husehld Discunt In rder t be eligible fr the Husehld discunt under an Aetna health and Life Insurance Cmpany Medicare supplement plan, yu must apply fr a Medicare supplement plan at the same time as anther Medicare eligible adult r the ther Medicare eligible adult must currently be cvered by an Aetna Health and Life Insurance Cmpany Medicare Supplement Plicy. The Medicare eligible adult must be either (a) yur spuse; (b) be smene with whm yu are in a civil unin partnership; r (c) be a permanent resident in yur hme. The husehld discunt will nly be applicable if a plicy fr each applicant is issued. The discunted rate will be lwer than the individual rates and will apply as lng as bth plicies remain in frce. 7%- Pennsylvania 5%- Alabama, Idah, Michigan, Nevada, Tennessee *Qualificatin criteria can vary by state. Refer t the apprpriate applicatin frm fr details regarding qualificatin based n the applicant s state f residence (plicy issue state).

3 Americ Husehld Discunt: 10% (Nt available in all states) PA des nt require bth eligibles t get the plicy; sme ther states d. If questin 1 in the Husehld Discunt sectin n the applicatin is answer Yes, the individual may be eligible fr the discunt. Individuals are eligible fr a Husehld Premium Discunt if fr the past year they have resided with at least ne, but n mre than three, ther adults wh are age 60 and lder.* If the live with anther adult wh is a legal spuse, we will waive bth the ne-year requirement and the age 60 requirement. Fr the purpse f this discunt, a civil unin partner r dmestic partner will be cnsidered a legal spuse when such partnerships are valid and recgnized in the applicant s state f residence. We may request additinal dcumentatin t determine eligibility. The premium will be reduced by the percentage shwn n the Outline f Cverage. The plicy s Husehld Premium Discunt will be remved if the ther adult n lnger resides with the applicant (ther than in the case f their death).* *The Husehld Premium Discunt prvisin is subject t the state variatin. Avaln Capital Blue Crss CSI & CSI Life Husehld Discunt *Husehld Discunts are ffered in the fllwing states: Gergia, Iwa, Mississippi, Nevada Guidelines: A Husehld Discunt is available t spuses r individual wh have lived the same husehld fr 12 mnths and wh apply fr cverage at the same time. The discunt will apply t bth applicants if bth are apprved fr cverage. In the cases where ne f the applicants is declined, the discunt will be nullified and the remaining applicant will receive the nn-discunted rate.

4 In situatins where an individual is applying fr cverage and they have lived with an active CSI Medicare Supplement plicyhlder fr at least 12 mnths, the Husehld discunt will be applied as fllws: The individual applying fr cverage will be given the discunt if cverage is apprved. The active plicyhlder will receive the discunt n his/her next plicy billing cycle. When the Husehld Discunt is 7% in a given state, the discunt will be a traditinal tw plicy discunt. In the states where it is a 5% discunt, the requirements are that the client lives with smene abve the age f 50 r is married t smene f any age. In the 5% states, it is a ne plicy discunt. Cigna (American Retirement Life Insurance C.) Husehld Discunt: 7% If anther member f yur husehld is applying fr r currently has a Medicare Supplement plan with ARLIC r an affiliated cmpany, yu may qualify fr a Husehld Discunt; see the Outline f Cverage fr details. Make sure the applicant prvides the name and Scial Security number f the individual(s) living at their current address during the applicant prcess. Husehld is defined as cndminium unit, a single family hme, r an apartment unit within an apartment cmplex. Assisted Living Facilities, Grup Hmes, Adult Day Care facilities, and Nursing hmes r any ther health residential facilities are nt included in the definitin f Husehld. If husehld discunt applies, then multiply the rate by Affiliate means an insurance cmpany that is under cmmn wnership r cntrl with American Retirement Life Insurance C. and that is a member f the same insurance hlding cmpany system. Husehld Discunt is a discunt that is available when mre than ne member f yur husehld enrlls r is enrlled in a Medicare Supplement plicy prvided by r thrugh an Affiliate f American Retirement Life Insurance C. Husehld is defined as cndminium unit, a single family hme, r an apartment unity within an apartment cmplex. Assisted Living Facilities, Grup Hmes, Adult Day Care facilities, and Nursing hmes r any ther health residential facilities are nt included in the definitin f Husehld. The Husehld premium discunt will be remved if the ther Medicare Supplement plicyhlder whse plicy status entitles yu t the discunt n lnger resides with yu r n lnger has a Medicare Supplement plicy thrugh an Affiliate f American Retirement Life Insurance C. Hwever, if that persn becmes deceased, yur discunt will still apply. The additin r remval f the discunt will ccur n the billing cycle fllwing the date ARLIC learns that yur eligibility has changed.

5 Cmpanin Life Husehld Discunt: 5% Yu are eligible fr a Husehld discunt if yu currently have a husehld resident (at least ne nt mre than three): With whm yu reside and t whm yu are either married r in a civil unin partnership; r Wh has an existing Medicare Supplement plicy f is applying fr such a plicy with Cmpanin Life. We may request additinal dcumentatin t determine eligibility. The discunted rates will be 5% lwer than the rates illustrated. Yur plicy s husehld premium discunt will be terminated if n ther adult wh is age 50 r ver r yur legal spuse cntinues t reside with yu (ther than in the case f his r her death). Equitable Husehld Discunt: 7% A 7% Husehld discunt is available t applicants living in the same husehld. Marriage is nt a requirement. This discunt is available fr applicatins dated n r after the discunt implementatin date. Sme states differ in requirement and availability. See chart fr details: Excellus Gerber

6 Highmark Humana Dental Husehld Discunt: 5% (where apprved) Plicyhlders with effective dates f 06/01/2010 and later sharing a residence save 5% n their mnthly premium. T enrll in the Husehld Discunt prgram be sure applicants prvide the name and Medicare ID f the ther plicyhlder living at their residential address in the Discunting sectin f the enrllment applicatin. (Husehld is defined as a cndminium unit, single family hme, r apartment within an apartment cmplex.) Please Nte: in Washingtn the discunt is ffered as a Spusal Discunt. Early Enrllment Discunt (Arizna and Massachusetts nly) Arizna plicyhlders save n their mnthly premium if enrlling between the ages f 65 and 73. They will cntinue t receive the discunt, which diminishes by 3% annually at time f renewal, thrugh age 77. Manhattan life- MLIC The HHD is a ne plicy discunt. The client will either need t be living with smene abve the age f 60 r married t smene f any age. This is a 7% discunt.

7 Medic PA & Ohi- Cuples Discunt In rder fr applicants f PA and Ohi t qualify fr the 7% Cuples Discunt with Medic, the must answer yes t all questins in Part G f the applicatin. Yu must be part f a legal relatinship recgnized by the state such a marriage, dmestic partnership, and civil unin. Yu must live at the same address. The ther member must als apply fr cverage with the cmpany r already be enrlled in an existing Medigap plicy with the cmpany. The cverage will cntinue fr as lng as the cverage fr bth plicies remains in frce and the Cuple status des nt change. All ther states- Husehld Discunt The Husehld discunt is applied if the applicant lives with anther persn ver the age f 18. The ther husehld member des nt need t apply. T qualify fr this discunt, review and cmplete the questins n Applicatin. Please be sure t prvide the name f the persn ver 18 that resides with the applicant. The underwriter may call the client and/r the ther persn t verify the infrmatin. Mutual f Omaha Husehld Discunt: If yu reside with a legal spuse, yu will be eligible fr a husehld premium discunt. The discunted premium will be priced lwer than the rates illustrated. Yur plicy s husehld discunt will be remved if ur legal spuse n lnger resides with yur (ther than the case f death).
