1 OAR (Online Assigned Risk) User s Guide Page 16 CERTIFICATES OF INSURANCE PAGE Clicking the Certificates f Insurance link in the menu bar at the tp f the OAR Hme Page will bring a prducer t the Certificates f Insurance hme page. Read the infrmatin and click n the Prceed t Create Certificate(s) f Insurance buttn at the bttm f the page. The Certificates f Insurance hme page prvides the fllwing infrmatin: When Certificates f Insurance are Available As f September 2015, the WCRIBMA is prviding t prducers wh have Online Assigned Risk (OAR) accunts a tl thrugh which they can request Certificates f Insurance frm the WCRIBMA that prvide evidence f Massachusetts wrkers cmpensatin and emplyers liability insurance cverage in the fllwing limited situatins: The plicy is a Massachusetts Assigned Risk Pl plicy. The plicy has been issued by the assigned carrier. The carrier has reprted the plicy t the WCRIBMA, and it has been successfully laded int the WCRIBMA s cverage database. And, The prducer reprted n the plicy as the brker f recrd is the same as the prducer lgged int OAR and requesting the certificate f insurance. When Certificates f Insurance are Nt Available In the fllwing circumstances, certificates f insurance cannt be requested and printed thrugh the WCRIBMA s system, and the prducer will need t get certificates frm the assigned insurance carrier: The assigned carrier has nt yet issued the plicy. The assigned carrier has nt successfully reprted the plicy t the WCRIBMA s cverage database. The plicy, as captured by the WCRIBMA s database, des nt shw that the prducer attempting t request a certificate is the prducer f recrd n the plicy. Multiple emplyers r addresses need t be shwn n a single certificate f insurance. The plicy effective date is mre than three (3) years ld. Special cverages are affrded by the plicy that the plicyhlder needs reflected n the certificate. The plicy was reprted t the WCRIBMA as an emplyee leasing plicy. Imprtant Ntes Since the prducer f recrd is nt a cntract agency r agency f the Massachusetts Wrkers Cmpensatin Assigned Risk Pl, the prducer has n authrity t issue certificates f insurance n assigned risk business. Prducers may nly btain certificates f insurance fr assigned risk plicies by either using this tl r requesting certificates frm the assigned carrier. N wrding can be added t r remved frm the certificate f insurance.

If an address is nt prvided fr the Certificate Hlder, it will be the respnsibility f the prducer t send the certificate f insurance t the Certificate Hlder. Certificate f Insurance Page The Certificate f Insurance page cntains three sectins, which can each be cllapsed r expanded by clicking n the sectin s name. The three sectins are Prducer Infrmatin, Plicy Infrmatin, and Distributin Infrmatin. Prducer Infrmatin All f the infrmatin in the Prducer Infrmatin sectin will be displayed n the Certificate f Insurance. The infrmatin displayed is frm the accunt f the individual wh is lgged int OAR. If the infrmatin in the OAR Accunt needs t be changed, please cntact the agency s Grup Administratr. All fields are required. The Insurance Prducer cannt be changed; it must reflect the name f the Agency as shwn in the user s OAR Accunt. The Insurance Prducer Address can be adjusted; the user can select any f the addresses frm the drp-dwn, which lists all addresses frm the Agency s OAR Cmpany accunt. The ther fields, Insurance Prducer Cntact Persn, Insurance Prducer Phne, and Insurance Prducer , can all be edited as needed. Plicy Infrmatin Search Criteria Enter search criteria t lcate the plicyhlder that has requested a certificate f insurance. Criteria can be entered in ne r mre criteria fields. T search by Plicyhlder Name, enter sme r all f the emplyer s name. The search used is a cntains search, s nly plicyhlders names that cntain the exact text entered will be returned. The name entered must cntain at least fur digits. T search by Plicyhlder FEIN, enter the federal emplyers identificatin number (FEIN) f the plicyhlder as it is shwn n the plicy. Fr search results t be prduced, there must be an exact match between the FEIN entered and the FEIN reprted n the plicy. T search by Plicy Number, enter the plicy number n the issued plicy. Fr search results t be prduced, there must be an exact match between the plicy number entered and the plicy number reprted n the plicy. T search by Plicy Effective Date, use the date tl t select the effective date f the desired plicy. All plicies with the entered effective date will be returned. Click the Search Plicy buttn when the criteria has been set.

Search results can be srted (AZ r ZA) by any field by clicking n the clumn header. The number f results returned is listed under the table f results. If t many results were returned, refine the search criteria entered. If the desired plicy was nt returned, verify the fllwing: The plicy is assigned risk. The plicy was issued by the carrier and has been reprted int the WCRIBMA s systems. The status f the plicy can be verified by accessing the Prf f Cverage - Cverage Verificatin Search tl at The plicy reflects the lgged in user s agency as the brker f recrd. The plicy effective date is nt mre than three years ld. The plicy is nt an emplyee leasing cmpany. If the desired plicy still cannt be lcated, cntact the assigned carrier and request that they issue the certificate f insurance accrding t established prcedures. Refer t the Residual Market / Prducer and Emplyer Resurces / Certificates f Insurance page n the WCRIBMA website. Select Plicy & Named Insured When the desired plicy is lcated, click the Select Plicy buttn t the left f the Named Insured clumn. If mre than ne emplyer is named n the plicy, the user must select which name t list n the certificate. Only ne name ptin can be selected. If the certificate hlder requires evidence f wrkers cmpensatin insurance fr mre than ne emplyer, multiple certificates can be requested. All cntinuatin sequences fr ne and nly ne name link will be displayed n the certificate f insurance. Names that were added t r deleted frm the plicy midterm will nt be selectable. Plicies that have n names listed n the plicy fr the full term will nt be selectable. Distributin Infrmatin Certificate Hlder Cntact and Distributin Infrmatin The Certificate Hlder is the persn r cmpany that has requested evidence f wrkers cmpensatin insurance frm the plicyhlder. Only the Certificate Hlder Cmpany Name and Certificate Hlder Cmpany Address are required; the Certificate Hlder Cntact Persn s Name, Phne and Address are ptinal. Hwever, if an address is nt prvided, it is the respnsibility f the prducer t send the certificate f

Certificate Hlder Cmpany Name required This is the certificate hlder name that will be displayed n the certificate f insurance. Certificate Hlder Cmpany Address required - This is the certificate hlder address that will be displayed n the certificate f insurance. Certificate Hlder Cntact Persn nt required and nt displayed n the certificate f insurance. Certificate Hlder Cntact Persn Phne nt required and nt displayed n the certificate f insurance. Certificate Hlder Cntact Persn - nt required and nt displayed n the certificate f insurance. This address is used t distribute the generated certificate f insurance t the certificate hlder. If this field is left blank, it is the respnsibility f the prducer t send the certificate f insurance t the certificate hlder. Plicyhlder Cntact and Distributin Infrmatin The user can pt t have the WCRIBMA a cpy f the certificate f insurance t the plicyhlder. If s desired, enter the Plicyhlder s Cntact Name and Address. Prducer Cntact and Distributin Infrmatin In additin t having the ability t save the pdf f the certificate f insurance at the end f the prcess, the prducer can als have the WCRIBMA a cpy f the certificate f insurance t their address. The default setting is YES fr this field, s t prevent an frm being sent t the prducer, click the NO buttn. Preview r Generate/Send the Certificate f Insurance The prducer has the ptin f viewing a draft versin f the certificate f insurance prir t generating s. When finished viewing the draft, simply clse the tab cntaining the draft and return t the pen Certificate f Insurance tab. If the draft cntains the crrect infrmatin and is ready t be sent, click the Generate/Send buttn t have the WCRIBMA s systems generate and send s t the certificate hlder and pssibly the plicyhlder s and/r prducer s addresses, depending n the infrmatin prvided and the selectins made abve. When the WCRIBMA system issues the certificate f insurance, it als makes a cpy f the certificate f insurance available t the insurance cmpany fr their recrds. Issuing Multiple Certificates T request anther certificate f insurance fr the same plicyhlder and certificate hlder but with a different plicy term, return t the Plicy Infrmatin sectin, select the desired plicy term, and click Generate/Send.

T request anther certificate f insurance fr different plicyhlder and certificate hlder, simply click the Clear Search Criteria and Clear Distributin Infrmatin buttns and start the prcess ver again.
