Our current plan fferings are as fllws: HealthyBlue r ActiveUnivera - Base Plan PPO r a Buy-Up Plan PPO Per bi-weekly paycheck cst share varies fr single, emplyee & child(ren), emplyee & spuse, r family cverage (rates prvided upn hire). Plan highlights that ffer first dllar cverage include: Preventive services (e.g. well visits, immunizatins, mammgraphy, etc.) including well child physician visits t age 19 are cvered in full Specialist cpays $40.00 in the Base Plan, $25.00 in the Buy-Up Plan Prescriptin drug tiers vary frm $10/$35/$70 in the Base Plan t $5/$25/$50 in the Buy-up Plan, with a $0 cpay n generics fr children t age 19 in bth plans Emergency rm, ambulance, hspital admissin, surgery, x-rays, radiatin & chemtherapy treatments are amng the ther services where the cpayment will vary HealthyBlue r ActiveUnivera High Deductible Health Plan with HSA Per bi-weekly paycheck cst share varies fr single, emplyee & child(ren), emplyee & spuse, r family cverage (rates prvided upn hire). Quarterly cmpany cntributins are made t a Health Savings Accunt: $260 fr single cverage, $520 fr cverages ther than single. Limited FSA (fr eligible dental and visin care expenses) available. 4 HealthyRewards r ActiveRewards Prgram Emplyees and their spuse are eligible t earn gift cards f up t $500 (single)/$1,000 (family) in healthy incentives each year by participating in HealthyRewards / ActiveRewards, a feature f the 3 health plans available. With HealthyRewards / ActiveRewards n-line incentive prgram yu can earn up t $1,000 in gift cards per family each year fr living healthy. Tracking yur wellness jurney activities n-line will earn yu pints. Then, cash in yur pints as yu earn them and get gift cards anytime thrughut the year. Dental Insurance Dental insurance is prvided frm the 1 st f the mnth fllwing yur date f hire. Our current plan ffering is as fllws: Dental Blue Optins r Univera Dental Select Bi-weekly cst share varies fr single, family and part-time cverage (rates prvided upn hire). Plan highlights include: Cvered preventive services include 2 cleanings and exams per calendar year with fluride treatments cvered fr children t age 16 A $50 individual / $100 family annual deductible n restrative services Annual maximum benefit f $1,500 per member A $1,500 per member lifetime maximum fr rthdntia fr children up t age 19 Mst basic restrative services are cvered at 100% with majr restrative (e.g. bridges, crwns, implants, etc.) cvered at 50%, bth subject t the deductible Flexible Spending Accunts (FSA) FSA accunts are available frm yur date f hire. Plan highlights include: $2,600 Medical Flexible Spending and/r $5,000 Dependent Care Flexible Spending Accunts Limited purpse FSA available fr emplyees wh chse the HDHP VISA debit card may be used t pay eligible expenses Life Insurance Life insurance is prvided frm the 1 st f the mnth fllwing yur date f hire. The carrier fr ur grup plicy is Vya. Plan highlights include: Cmpany Paid cverage f 2 times yur annual salary, t a maximum f $1,500,000. Please nte that ur insurer requires Evidence f Insurability (EOI) fr cverage ver ur $1,000,000 Guarantee Issue amunt Emplyee Optin t Purchase Supplemental Life (1x t 5x annual salary, subject t a $1.5M maximum when cmbined with Basic cverage). Mnthly rates vary by banded ages, e.g. a year ld pays 41.1 cents per $1,000 f cverage, a year ld pays 23 cents per $1,000, etc. Spusal Life Insurance in increments up t $200,000, with EOI required fr amunts ver $50,000 & Child Life in $5,000 r $10,000 increments. 5 Time Away Frm Wrk Our current time ff plicy prvides fr the fllwing: 8 Hlidays Paid Sick Time 5 Paid Persnal Time Off Days 1 Vlunteer Time Off Day (effective starting the calendar year after (prrated) yur hire) Paid Vacatin - This time ff is available after six mnths f emplyment and is prrated fr year f hire. Retirement Prgrams The Cmpany ffers a Defined Cntributin benefit t assist yu in saving fr yur retirement needs. Our current plan ffering is as fllws: Excellus 401(k) Plan The plan is an ERISA qualified plan that ffers the availability f pre-tax deductins and cmpany matching cntributins. Plan highlights include: Immediate participatin is available t defer up t 50% f yur pay, subject t the 2017 IRS maximum f $18,000. Catch-up cntributins, IRS maximum f $6,000, are als available t thse 50 and lder. Aut enrllment equal t 4% f pay Cmpany matches 100% n first 4% f yur regular payrll deferrals (n match is prvided n catch-up cntributins). During the first quarter f each year, the Cmpany will als cntribute an additinal 4% f yur prir year s plan pay (up t IRS limit) fr thse that were emplyed n December 31 st f the prir year. The Cmpany cntributin, as well as any Cmpany matching cntributins, are vested after 3 years f service. Current administratin is thrugh Fidelity Investments.
