Enrlling fr Benefits Cverage Health and Welfare Benefits Within tw weeks f yur hire date, yu will receive an invitatin t make yur benefit electins [excluding 401(k)] frm Yur Benefits Center. Yu have 30 days frm yur date f hire t make yur benefit electins. If yu have questins r yu d nt receive yur enrllment within tw weeks f yur hire date, cntact Yur Benefits Center at Mnday thrugh Friday, 8:00 a.m. ET t 11:00 p.m., ET. 401(k) Plan The 401(k) Plan is managed by Fidelity Investments. Yu will be able t enrll in the 401(k) Plan after yur first paycheck. There is n deadline t enrll yu can enrll at any time. 4 Health and Welfare Benefits Benefits Plan Year The Benefits Plan Year runs frm January 1, 2016 thrugh December 31, Eligibility Eligible Emplyees Yu are eligible fr the health and welfare benefits described in this guide if yu are a regular emplyee (i.e., nt a temp r intern) scheduled t wrk at least 30 hurs per week. Cverage begins the first f the mnth fllwing yur hire date. If yu are hired n the first f the mnth, yur cverage begins n yur hire date. Eligible Dependents When yu becme eligible fr health and welfare benefits, s d yur eligible dependents. In general, eligible dependents include yur lawful spuse, and children up t age 26. If yur child is mentally r physically disabled, cverage may cntinue beynd age 26, nce prf f the nging disability is prvided. Children may include natural, adpted, fster r stepchildren. When yu enrll a dependent, yu certify that he r she meets the definitin f an eligible dependent under the terms f the plan. If yur dependent lses eligibility fr cverage, yu must cntact the Yur Benefits Center within 30 days f the lss f eligible t remve the dependent frm cverage. If it is determined that yu have enrlled, r failed t remve, smene wh des nt meet the definitin f an eligible dependent, cverage fr the ineligible dependent may be terminated prspectively frm the date f determinatin f ineligibility. If it is fund that yu btained cverage thrugh fraud r an intentinal misrepresentatin f material fact as prhibited by the terms f the benefits plan, cverage fr yu and the ineligible dependent may be rescinded. The Cmpany will prvide yu with 30 days written ntice f the rescissin. Yu may als be subject t disciplinary actin, up t and including terminatin. Dmestic Partner Cverage Benefits are available t the dmestic partners f eligible emplyees. Eligible dmestic partners include: Oppsite-sex dmestic partners f emplyees wh are registered as dmestic partners in a lcal jurisdictin that maintains such a registry. A same-sex dmestic partner. Same-sex dmestic partners must meet all f the fllwing requirements t be eligible: Be at least 18 years ld Nt be legally married, under federal law, t anyne r be part f anther dmestic partnership during the previus 12 mnths Currently be in an exclusive, cmmitted relatinship with each ther that has existed fr at least 12 mnths and is intended t be permanent Currently reside tgether, have resided tgether fr at least the previus 12 mnths, and intend t d s permanently 4

Therefre, the prtin f premiums that the Cmpany pays n behalf f yur dmestic partner fr health and dental insurance must be taxed. This prcess is called imputed incme. Als, any payrll cntributins yu pay that are attributable t yur dmestic partner s cverage must be taxed. This means a prtin f yur payrll cntributins will be deducted after taxes are deducted. Changes in Benefit Electins Yur 2016 electins will be in effect thrugh December 31, Each year during Open Enrllment, yu will have the pprtunity t change yur electins fr the fllwing plan year. During Open Enrllment, yu will have the pprtunity t: Add, change r drp benefits cverage Add r remve eligible dependents frm cverage Enrll in a Health Care and/r Dependent Care Flexible Spending Accunt Change yur ptinal life and AD&D insurance amunts Yu can make sme limited changes during the year due t a Qualified Life Event. Hwever, yu must ntify Yur Benefits Center within 30 days f the Qualifying Life Event. Enrllment in Medicare 5

6 Medical Cverage Yu may enrll in ne f ur tw medical plans available thrugh Aetna: Aetna 2000 with HSA Plan Aetna 1300 with HSA Plan Each plan is a high deductible medical plan with a health savings accunt (HSA). Emplyees (1) enrlled in Medicare; r (2) enrlled in Medicaid; r (3) wh have medical cverage thrugh Tricare; r (4) wh have received VA benefits within the past three mnths are eligible fr the Aetna 2000 and the Aetna 1300 medical plans withut the health savings accunt (HSA) cmpnent. These plans have the same prvisins as the Aetna 2000 and Aetna 1300 Plans with an HSA except there is n HSA with these plans (and cnsequently, n HSA emplyer funding). If yu meet at least ne f the abve fur criteria, cntact Yur Benefits Center fr mre infrmatin n the Aetna 2000 and Aetna 1300 withut an HSA Plans. Health Savings Accunt (HSA) Plans The HSA plans cmbine medical benefits with a tax-favred savings accunt yu wn and can use fr qualified medical expenses. Bth HSA plans prvide: Freedm t chse. Yu may chse any licensed prvider when yu need care; hwever, each plan pays higher benefits and yu pay less ut f yur wn pcket when yu use prviders that are in the Aetna netwrk (i.e., in-netwrk). An emplyer cntributin t yur HSA. Yu can benefit frm an emplyer cntributin t yur HSA when yu cmplete a health assessment and/r bimetric screening in Triple-tax-advantaged savings. Yu can have mney deducted frm yur paychecks and depsited int yur HSA t pay fr current and future health care expenses. In ding s, yu get a triple-tax advantage. Yur cntributins are deducted frm yur paycheck befre incme taxes are withheld s yu pay less in federal and state incme tax. Plus, the mney in yur accunt earns interest tax free and any withdrawals yu make t pay qualified expenses are nt taxed. Cnvenient payment ptins. Yu can use yur Aetna debit card, nline bill payment r the PayFlex mbile app t pay yur medical expenses. Investments. Once yur accunt balance reaches $1,000, yu can chse frm several investment ptins. Prtability. Yu wn yur accunt. Yu can take it with yu if yu leave the Cmpany. Building yur HSA Yu can earn an emplyer cntributin t yur HSA by cmpleting tw healthy actins. Actin 1: Cmplete a health assessment Fr 2016, yu can earn an annual emplyer HSA cntributin f up t $200 fr emplyee nly cverage r $400 fr all ther cverage categries by cmpleting a health assessment. Please nte that if yu enrll in medical cverage after January 1, 2016, the amunt f yur emplyer 6

7 2016 Emplyee Benefits Guide cntributin will be pr-rated based upn bth yur effective date f cverage as well as the cverage tier yu select (e.g., emplyee nly cverage, family cverage, etc.). Fr example, if yur effective date f cverage is April 1, 2016 and yu elect emplyee nly cverage and cmplete the health assessment, yur prrated emplyer cntributin will be: ($200 annually 12 mnths) x 9 remaining mnths = $150 emplyer cntributin. The health assessment is an nline questinnaire that helps yu learn abut yur current health status. Answer questins abut yur lifestyle, health histry and recent health screenings (like the bimetric screening). Yur infrmatin will be kept strictly cnfidential and will nt be shared with the Cmpany. When yu have cmpleted the questinnaire, yu will receive a persnalized health reprt. It is a great way t learn what yu are ding right and where there is rm fr imprvement. Visit and lg in t Aetna Navigatr t cmplete the health assessment. If yu are a first-time user, yu will need t use the Register nw link t get started. Yu can access the health assessment by click n the I want t... Take a Health Assessment which is lcated n the left side n yur Aetna Navigatr hme page. Yu will receive the emplyer cntributin the mnth fllwing the cmpletin f yur health assessment. Yu have until December 15, 2016 t cmplete the nline health assessment in rder t earn an emplyer cntributin t yur HSA. Actin 2: Cmplete a bimetric screening Yu can earn mre dllars tward yur HSA by cmpleting a bimetric screening fr metablic syndrme. The 2016 emplyer cntributin fr cmpleting the bimetric screening will be $200 fr emplyee nly cverage r $400 fr all ther cverage categries. If yu enrll in medical cverage after January 1, 2016, the amunt f the emplyer cntributin will be prrated. The amunt will depend upn bth yur effective date f cverage as well as the cverage tier yu select. Fr example, if yur effective date f cverage is April 1, 2016 and yu elect individual cverage and cmplete the bimetric screening, yur prrated cntributin will be: ($200 annually 12 mnths) x 9 remaining mnths = $150 emplyer cntributin. Metablic syndrme is a set f five risk factrs that, tgether, can set the stage fr serius health prblems. The risk factrs are: high bld pressure, high bld glucse, lw HDL chlesterl, high triglyceride levels and central besity (as indicated by waist measurement). When yu have yur bimetric screening, bld will be drawn and yur bld pressure and waist circumference will be measured. Get yur screening: At a Quest Diagnstics Patient Service Center: T make an appintment, visit and register. Use the registratin key: crprisk. Yur ID is yur date f birth (MM/DD/YYYY) fllwed by the last fur digits f yur Scial Security number. Fr example, if date f birth is December 1, 1982 and Scial Security number is , the ID wuld be Yu may als call

Just remember t bring a Physician Results Cllectin frm. Dwnlad the frm frm r frm under Abut My Prgram. T qualify fr the 2016 emplyer HSA cntributin, yur cmpleted Physician Results Cllectin frm must be received by Quest Diagnstics befre Nvember 1, Yu will need t fast (n fd r drink, except water) fr 9 12 hurs prir t yur screening. It is als imprtant t drink plenty f water and take all medicatin accrding t yur regular schedule prir t yur screening appintment. Yu will be able t view yur results n the Quest Diagnstics website 3 5 days after yu cmplete yur screening. Yu will als receive a reprt via regular mail 2 3 weeks after yur screening. All infrmatin btained in yur Blueprint fr Wellness experience is persnal and cnfidential. The emplyer cntributin will be depsited t yur HSA the mnth after yu cmplete the bimetric screening Imprtant facts yu shuld knw abut the HSA The IRS sets the rules fr health savings accunts. Here is what yu shuld knw: Yu may cver a dmestic partner under yur health care plan, but his r her qualified health care expenses may nt be reimbursed frm an HSA unless he r she is a qualified dependent fr federal incme tax purpses. Yu may still cntribute the IRS maximum fr family cverage if yu cver a dmestic partner. While yu may cver adult dependent children fr health care up t age 26 (regardless f student r marital status), using an HSA t pay fr qualified health expenses f dependent children requires he r she be a qualified dependent fr federal incme tax purpses. A qualifying Dependent Child includes: Dependent children up t age 19, r age 24 if full-time students. Dependent children up t the age f 26 that yu will be prviding mre than 50% f this persn s supprt fr the calendar year and can claim them n yur federal incme tax return. Yu cannt use yur HSA t pay the qualified expenses f adult dependent children wh yu dn t claim n yur taxes. Hwever, if yu have an adult child and are unable t claim them n yur taxes, the adult child may pen their wn HSA t pay thse expenses as lng as they are enrlled in an HSA plan. If an adult child pens his r her wn HSA, he r she can cntribute up t the full family amunt f $6,750 fr

9 2016 Emplyee Benefits Guide Sme circumstances that impact yur eligibility fr an HSA: If yu are enrlled in Medicare r Medicaid, yu are nt eligible fr an HSA. Yu cannt have bth an HSA and a health care flexible spending accunt (FSA). If yu have an HSA, yur spuse can nt be enrlled in a health care flexible spending accunt r HRA with his r her emplyer. If yu have ther medical cverage thrugh Tricare r have received VA benefits within the past three mnths, yu cannt have an HSA. If yu have cverage under yur spuse s medical plan, yu cannt have an HSA. The IRS sets limits n ttal cntributin amunts. Fr 2016, these limits* are: $3,350 fr individual cverage $6,750 fr all ther cverage levels Emplyees age 55 r lder during 2016 can make additinal catch-up cntributins up t $1,000. *Cntributin limit includes bth emplyee and emplyer cntributins. Limit des nt include catch-up cntributins, which can be made in additin t the IRS-allwed maximum. If yu are a new hire and cntributed t yur prir emplyer s HSA Plan during the calendar year, please nte that the IRS annual limits apply t the ttal emplyee cntributins yu made under yur previus emplyer s plan and the Crprate Risk Hldings HSA Plan. Steps t use yur HSA at the dctr Here is an example f hw yu receive and pay fr care with a health savings accunt (HSA). Fr mre infrmatin n hw t use yur HSA, see the Get the Mst frm Yur Aetna HSA Plan guide available n Step 1: Receive health care services. If yu use an in-netwrk prvider, yu d nt need t pay at the time the service is prvided. Simply shw yur Aetna ID card and yur prvider will file a claim with Aetna. There are n cpayments in this plan, s please remind the receptinist if yu are asked fr ne. Step 2: Review yur claim after it is prcessed. If yu are registered with Aetna Navigatr, yu will get an when yur claim has been prcessed. Simply click n the link t see an Explanatin f Benefits statement that shws hw the claim was prcessed and hw benefits (if any) were paid. Step 3: Watch fr the bill frm yur dctr. The amunt yu we n yur dctr s bill shuld match yur amunt wed n the Explanatin f Benefits (EOB). If the amunts d nt match, call Aetna Member Services at Step 4: Decide hw t pay the bill. Yu can pay it ut f yur wn pcket r yu can use yur HSA. Remember, yu wn yur HSA s yu decide when t use it. Pay yur current expenses frm yur accunt r let the accunt grw fr future expenses it is yur chice. 9

10 Step 5: Pay the bill. Once funds are available in yur HSA, PayFlex makes it easy t pay fr yur eligible expenses. Use the PayFlex debit card: expenses are paid autmatically and there is n paperwrk t cmplete. Pay yurself back: Pay fr eligible expenses with cash, check r yur persnal credit card. Then withdraw funds frm yur HSA t pay yurself back. Yu can even have yur payment depsited directly int yur checking r savings accunt. Pay yur prvider: Use PayFlex s nline feature t pay yur prvider directly frm yur accunt. Step 6: Keep yur receipts. Make sure t keep yur receipts fr all expenses in case yu are audited by the Internal Revenue Service (IRS). What are qualified medical expenses? Teladc dctrs can: Diagnse yur cnditin Recmmend treatment Prescribe medicatin if necessary Teladc dctrs can treat nn-emergency cnditins like cld and flu symptms, respiratry and ear infectins, skin cnditins, sinus prblems, allergies, brnchitis and mre. Each Teladc cnsultatin is $40 and the fee applies t yur deductible, cinsurance and ut-f-pcket maximums. Yu can use yur HSA t pay fr Teladc services. T register fr Teladc and fr mre infrmatin, g t Yu may call Teladc at TELADOC ( ). * Teladc perates subject t state regulatin and may nt be available in certain states. Infrmed Health Line If enrlled in an Aetna medical plan, yu and yur eligible dependents als have 24/7 access t health infrmatin and answers with Aetna s Infrmed Health Line. Smetimes yu just need a quick answer frm a health care prfessinal r sme reliable advice t use until yu can see yur dctr. With the Infrmed Health Line, yu have this and mre. When yu call the tll-free number, yu will talk with a trained, registered nurse wh can: Answer health-related questins and explain medical terms Help yu decide whether r nt t g t a hspital emergency rm Tell yu hw t take care f a health prblem until yu can get t a dctr Give yu gd questins t ask yur dctr and help yu make the mst f ffice visits Yu may call the Infrmed Health Line at Prescriptin Drug Cverage If yu enrll in the Aetna 2000 r Aetna 1300 medical plan, yu will als receive prescriptin drug benefits administered thrugh CVS Caremark. Thrugh CVS Caremark, yu may fill yur prescriptins: At participating retail pharmacies. CVS Caremark is ne f the cuntry s largest pharmacy benefit managers, giving yu access t a netwrk f apprximately 68,000 pharmacies, including mre than 7,000 CVS pharmacy stres where yu can fill shrt-term prescriptins. Using the mail-rder service. After yu enrll, yu will receive infrmatin abut using CVS Caremark s mail-rder service. Yu can rder up t a 90-day supply f medicatin and request refills nline, by phne r by using a paper rder frm. Once yu are enrlled, yu can register at t use the mail-rder service, get 11

12 health infrmatin, use nline tls and get answers t medicatin questins. Value Frmulary A frmulary is a list f medicines that are cvered under the prescriptin drug plan. The Cmpany s prescriptin drug plan is called a Value Frmulary because the medicatins cvered under the Plan are high value-- they prvide great health benefits and are priced right. The Value Frmulary cvers generics. And, sme brand name drugs are cvered when a generic alternative is nt available. A list f cvered drugs is available n the CVS s Caremark website at Yu may als call CVS Caremark anytime at fr infrmatin. If yur current medicatin is nt n the Value Frmulary, ask yur dctr t write a prescriptin fr an alternative that is n the Value Frmulary. Yu can cntinue t take the nn-cvered medicatin; hwever, yu will pay the full cst f the medicatin. If yur dctr believes yur current nn-cvered medicatin is medically necessary, have yur dctr cmplete the CVS Tier Exceptin Frm and return it t CVS Caremark at the fax number n the frm. A cpy f the frm may be btained n Hw yur prescriptin drug benefits wrk with yur medical deductible Until the deductible is met, yu pay the full cst f any prescriptin. Once the deductible is met, yu then pay a cpay (flat fee) f $5 fr generic medicatins and cinsurance (a percentage) fr brand-name medicatins that are n the Plan s Value Frmulary. The Maintenance Chice Prgram Yu can fill yur 90-day supply f maintenance medicatins at a CVS pharmacy r thrugh mail rder service. Yu save mney because the cst f a 90-day supply is essentially the same as tw mnths f a 30-day supply at the retail pharmacy price. 13 2016 Emplyee Benefits Guide Hspital Indemnity Plan There are tw Hspital Indemnity Plan (HIP) ptins: HIP 2000 HIP 1300 The HIP prvides extra help when yu r a cvered family member needs inpatient hspital care. It pays a cash benefit directly t yu that is in additin t any benefits yu may receive frm yur medical plan. And, there is n additinal paperwrk r claim filing required if yu are enrlled in the Aetna 2000 r Aetna 1300 medical plans. Tw types f benefits are paid: A lump-sum benefit f $1,300 (if enrlled in the HIP 1300) r $2,000 (if enrlled in the HIP 2000) fr ne stay in the hspital during the cverage year; plus A daily benefit f $100 per day, fr up t 100 days that a member is an inpatient in a hspital per cverage year. Which HIP yu may enrll in depends upn yur medical plan electin: If yu are enrlled in the Aetna 2000 medical plan, yu may nly enrll in the HIP If yu are enrlled in the Aetna 1300 medical plan, yu may nly enrll in the HIP If yu d nt have medical cverage thrugh the Cmpany (e.g., yu are enrlled in yur spuse s medical plan), yu may enrll in either HIP ptin. Yu pay fr this benefit thrugh after-tax payrll deductins during the year. This benefit can be used t help pay ut-f-pcket expenses including yur plan s deductible.
