1 PG&E's Paid Family Leave & Disability Benefits Utility IBEW and SEIU--represented emplyees 2017 and 2018 Paid Family Leave and Medical /Disability Benefits Overview PG&E s Paid Family Leave wage cntinuatin benefits will be a supplement t PG&E s Vluntary Disability & Paid Family Leave Benefit Plan (VP) benefits nly. Fr qualifying events beginning n r after January 1, 2018, emplyees must be cvered under the PG&E VP t be eligible fr wage cntinuatin benefits. Emplyees wh pt-ut f VP cverage will remain eligible fr the Califrnia State Plan benefits. PG&E pays the full cst f supplemental wage cntinuatin benefits. Yu pay fr the cst f cverage fr the Vluntary Plan thrugh payrll deductins (same cst as State Plan). PG&E PFL Wage Cntinuatin prvides supplemental incme replacement up t 100%* (pre-tax) f yur basic wage rate (the wage as f the date prir t yur first date f leave) fr eligible emplyees wh take apprved time away frm wrk t care fr a seriusly ill family member r t bnd with a new child. Wage cntinuatin supplements the weekly Vluntary Plan benefits. Yu may be eligible fr up t eight (8) weeks f PFL wage cntinuatin benefits/leave. Medical Leave and Vluntary Disability Insurance review details in the chart n page 2 and in the medical leave sectin starting n page 6. Lng-Term Disability benefit prvides yu with incme replacement if yu becme disabled because f an accident r a lng-term illness and yu are unable t wrk. The benefit under the new LTD Plan IV, when cmbined with certain ther surces f incme that may be payable after the disability ccurs, will be equal t 70% f yur basic mnthly pay. 3 Paid Family Leave Wh is eligible? If yu are an IBEW- r SEIU-represented emplyee, yu becme eligible under the PFL plicy upn cmpletin f ne day f emplyment. The PFL plicy applies t eligible emplyees wh have a new qualifying event n r after January 1, Time ff t bnd with yur new child must be taken within the first year f the child s birth, adptin r fster care placement. If yu are an eligible emplyee and did nt exhaust yur six (6) weeks f CA PFL Insurance benefits fr a 2017 qualifying bnding leave event and yu have time available under the Family and Medical Leave Act (FMLA), Califrnia Family Rights Act (CFRA), r Cmpany leave, yu may be eligible t receive PFL Wage Cntinuatin benefits fr qualifying bnding events in Yu are nt eligible fr the VPFL benefits with respect t the 2017 qualifying bnding leave event. If yu are a Hiring Hall, temprary r intermittent emplyee wh has nt attained regular status, yu are eligible fr Vluntary PFL benefits (55% benefit) and are nt eligible fr PG&E PFL wage cntinuatin benefits/leave r PG&E s cmpany leaves. VPFL is a wage replacement benefit nly; yu may be eligible fr a leave f absence under the FMLA, CFRA, r ther similar state r lcal leave law. Hw it wrks If yu will be absent t care fr a family member with a Serius Health Cnditin r t bnd with yur new child within the first (1st) year f the child s birth, adptin r fster care placement, yu must apply fr Paid Family Leave by cntacting the leave and disability administratr, Sedgwick. Fr absences in which yu are als eligible and qualify under the FMLA, CFRA, Cmpany Child Care/Bnding leave r any ther similar r lcal leave law, PFL runs cncurrent with and des nt extend the maximum perid available. Ntice and Claim prcessing Yu must prvide 30 days advance ntice fr a planned PFL leave perid ne call is needed fr VPFL, PG&E PFL wage cntinuatin and any ther leave fr which yu may be eligible and qualified. Fr unfreseeable events, yu need t prvide ntificatin as sn as the need fr leave is knwn. Cmplete medical certificatin (fr family care events) r dcumentatin t supprt yur bnding leave is required t be submitted t the leave and disability administratr, within the timeframe specified. Once cmplete dcumentatin is received, a decisin will be made within 2 business days t determine yur eligibility fr VPFL and Wage Cntinuatin benefit payments and leave. Yu re required t take leave under the FMLA and CFRA at the same time yu are receiving PFL benefits. FMLA, CFRA, ther lcal state/lcal leave laws r cmpany leave, VPFL and wage cntinuatin benefits all run cncurrently with and d nt extend leave under federal, state, and lcal leave laws r cmpany leave plicies. After yur PFL leave perid, yur family care r bnding leave may be extended under the FMLA, CFRA r Cmpany Child Care leave, if yu are eligible. If yu are a CA emplyee and yu pt ut f the VP, yu are nt eligible fr the wage cntinuatin benefits under the PFL plicy. Yu will cntinue t fllw the leave f absence prcess and apply fr the cmpany child care/bnding leave and leaves prvided under the FMLA, CFRA, PDL r lcal leaves, if eligible. 4 Paid Family Leave, cntinued Yu will need t apply fr CA PFL thrugh the Emplyment Develpment Department (EDD). Payments fr CA PFL benefits will be made directly by the EDD and via a debit card. Benefits and Payments There is n waiting perid required fr VPFL and PG&E PFL wage cntinuatin benefits. VPFL and PFL wage cntinuatin benefit payments will be made accrding t yur nrmal pay cycle and the methd f payment yu ve elected thrugh PG&E s payrll department (i.e. direct depsit r live check). VPFL benefit payments will be issued n a separate paycheck frm any ther pay type and issued accrding t yur current pay designatin (direct depsit r live paycheck). PFL Wage Cntinuatin supplements VPFL, r ther similar state benefit, up t an equivalent f 100% f yur basic wage rate n the day prir t yur first day f family r bnding leave. There is n cap t the 60% VPFL r 100% PFL wage cntinuatin benefits. VPFL is federally taxable and wage cntinuatin benefits are taxable. If yu are an intern, hiring hall, temprary r intermittent emplyee wh has nt attained regular status, yu are eligible fr a weekly VPFL benefit f 55%. Fr each day f an eligible, full-time, cntinuus PFL perid due t caring fr a family member with a serius health cnditin r bnding with a new child which is less than a full week, the amunt f daily PFL benefit payable will be ne-seventh (1/7) f the amunt f the weekly benefit. Yu may be eligible t receive PFL benefits n an intermittent basis fr absences in which yu are suffering a wage lss during an eligible PFL benefit perid. Amunt f PFL wage cntinuatin is reduced by VPFL benefits, and tgether with VPFL, will nt exceed 100% f pre-disability wage rate. There is a wage lss calculatin t determine bth VPDI and wage cntinuatin benefits, which means partial-day absences are generally cunted as full-day absences. If yu re taking leave fr bnding purpses, the minimum PFL benefit perid in which PFL wage cntinuatin can be received is 2 weeks. Hwever, n 2 ccasins, a leave and PFL benefit perid f less than 2 weeks can be taken. Other Benefits When apprved fr PFL leave/benefits, yu cntinue t receive health care benefits as if yu were at wrk. Payments fr these health care premiums csts will be autmatically deducted frm available wage cntinuatin benefits received. If yur wage cntinuatin benefits payments are nt sufficient t cver the full amunt f yur benefit premiums, yu have the ptin t request that a prtin f certain emplyee-paid benefit premiums be deducted frm yur VPDI benefit payments. Eligible emplyees will t cntinue t accrue vacatin during perids f PFL leave in accrdance with current rules fr unpaid leaves f absence (see yur respective labr agreement, mypgebenefits.cm r the Summary f Benefits Handbk fr additinal details and limitatins fr vacatin accrual during perid f leave). While apprved t receive VPFL and wage cntinuatin benefit payments, 401(k) cntributins and cmpany match will cntinue at yur current designatin unless yu elect therwise pursuant t nrmal 401(k) electin prcedures. The emplyee becmes a new dad and requests 8 weeks f leave t bnd with his new child. Leave starts n May 15, State Plan 2018 PG&E Paid Family Leave (PFL) Benefits Emplyee is eligible fr CA PFL benefits and federal/state leaves and PG&E s cmpany child care/bnding leave. Beginning the first day f leave, he wuld receive a weekly wage replacement f apprximately 60% under CA PFL: The state uses a base perid t determine benefit; base perid is January 1, 2017 December 31, 2017: 2017 annual salary = $85, ($1,634.61/week) 60% f eligible base perid earnings (using highest quarter): apprximately $ Week 1 8: $980.76/week CA PFL Ttal amunt fr 8 weeks = $7, Emplyee is eligible fr VPFL + PG&E PFL Wage Cntinuatin benefits and federal/state leaves and PG&E s cmpany child care/ bnding leave. Leave starts n March 20, State Plan 2018 PG&E Paid Family Leave (PFL) Benefits Emplyee is eligible fr CA PFL benefits and federal/state leaves and PG&E s cmpany child care/bnding leave. Beginning the first day f leave, she wuld receive a weekly wage replacement f apprximately 60% under CA PFL: The state uses a base perid t determine benefit; base perid is Octber, 2016 September 30, 2017: 2017 annual salary = $112,000 ($2,153.84/week) 60% f eligible base perid earnings (using highest quarter): apprximately $1,292.30, but capped at state maw f $1, Week 1 6: $1,216.00/week CA PFL max Ttal amunt fr 6 weeks = $7, Emplyee is eligible fr VPFL + PG&E PFL Wage Cntinuatin benefits and federal/state leaves and PG&E s cmpany child care/bnding leave. 6 Medical Leave with Vluntary Plan Disability Insurance Wh is eligible? If yu are a Utility emplyee wrking in Califrnia, yu are eligible fr cverage under the Vluntary Disability and Paid Family Leave Benefit Plan. VPDI is a wage replacement benefit nly; yu may be eligible fr a leave f absence under the Family and Medical Leave Act (FMLA)/ Califrnia Family Rights Act (CFRA), Pregnancy Disability Leave (PDL) ther similar state r lcal leave law r PG&E s cmpany medical leave. Medical Leave: FMLA/ CFRA/PDL: FMLA/ CFRA Yu ve wrked fr the cmpany fr at least ne year and wrked at least 1250 hurs in the 12 mnths immediately preceding the leave start date. FMLA/ CFRA Yu have nt exhausted yur FMLA/CFRA entitlement within the 12 mnths prir t the requested leave date. CFRA and PDL eligible if yu wrk in Califrnia. PDL Immediately eligible (n service r hurs wrk requirement). Cmpany Medical Leave: Absences must be greater than tw weeks. Leave cannt be taken n an intermittent basis r n a reduced schedule. Yu need t have attained regular status. Hiring Hall emplyees are nt eligible fr this leave. If yur medical leave began in 2017, yu remain eligible under State Plan benefits with respect t such leave. Hw it wrks If yu will be absent due t yur wn disability (a physical r mental illness r injury, including any illness r injury resulting frm pregnancy, childbirth, r a related medical cnditin) fr mre than seven (7) cnsecutive calendar days (waiting perid) r any length f time that may be cvered under FMLA/CFRA/ PDL, yu must apply fr medical leave by cntacting the leave and disability administratr, Sedgwick. Yur call t Sedgwick t reprt yur medical leave will initiate yur disability claim and start the VPDI eligibility review prcess. Pregnancy is a cvered event and PDL and the VPDI prvides benefits t prepare fr childbirth and recvery frm delivery. Generally, yu may begin receiving VPDI benefits up t 4 weeks prir t yur expected delivery date t prepare fr delivery. Fr recvery frm delivery, yu may begin receiving VPDI benefits frm the date f birth (regular delivery is cvered fr 6 weeks; C-sectin is cvered fr 8 weeks). Fr absences fr which yu are als eligible and qualify under the FMLA, CFRA, PDL, cmpany leave plicy r any ther similar r lcal leave law, VPDI runs cncurrent with and des nt extend the maximum perid available. After satisfying the waiting perid and exhausting sick pay, VPDI prvides a weekly benefit f 60% f yur basic wage rate n the day prir t yur first day f disability. There is n cap t the 60% VPDI. VPDI nt taxable. If yu are an intern, hiring hall, temprary r intermittent emplyee wh has nt attained regular status, yu are eligible fr a weekly benefit f 55%. Fr each day yu are unable t wrk during any eligible perid f disability which is less than a full week, the amunt f daily VPDI benefit will be ne-seventh (1/7), f the amunt f the weekly benefit. If yu return t wrk and are wrking part-time r intermittently and still suffering a wage lss during an eligible disability benefit perid, yu may be eligible fr VPDI benefits. Waiting Perid There is a wage lss calculatin t determine bth VPDI and wage cntinuatin benefits, which means partial-day absences are generally cunted as full-day absences. There is a seven cnsecutive calendar day waiting perid t be eligible fr VPDI benefits. Sick pay will be used during the waiting perid. Once sick pay is exhausted, yu can chse t use vacatin pay, flating hlidays r in- lieu f days r be unpaid during the waiting perid. If n sick pay is available and if apprved fr VPDI, benefits will begin n the earliest f: 1. The 8 th cnsecutive calendar day f disability; 2. The 1 st day f hspital cnfinement; r 3. The 1 st day f treatment in a hspital surgical unit r a surgical clinic, prvided yu re disabled fr a perid f at least 8 days as a result f such treatment. Other Benefits While apprved fr medical leave, yu cntinue t receive health care benefits as if yu were at wrk. Payments fr these health care premiums csts will be autmatically deducted frm available sick pay. If yur sick pay is nt sufficient t cver yur full absence perid, yu have the ptin t request that a prtin f certain emplyee-paid benefit premiums be deducted frm yur VPDI benefit payments. Eligible emplyees cntinue t accrue vacatin while receiving sick pay. Once sick pay exhausts, yu will t cntinue t accrue vacatin during yur medical leave in accrdance with current rules fr unpaid leaves f absence (see yur respective labr agreement, mypgebenefits.cm r the Summary f Benefits Handbk fr additinal details and limitatins fr vacatin accrual during perid f leave). The emplyee has an accident and is unable t return t wrk fr 12 weeks, beginning March 5, The emplyee has 5 weeks/200 hurs f sick pay available; required t be used first STATE PLAN Emplyee is eligible fr CA SDI benefits after all available sick pay ends. She may als be eligible under federal/state leaves and, nce sick pay ends, PG&E s cmpany medical leave. After sick pay ends, she wuld receive a weekly wage replacement f apprximately 60% under CA SDI. 9 Lng-term Disability Plan IV PG&E s Lng-term Disability (LTD) Plan benefits prvide yu with incme replacement if yu becme disabled because f an accident r a lng-term illness and yu are unable t wrk. The benefit under the new LTD Plan IV, when cmbined with certain ther surces f incme, will be equal t 70% f yur basic mnthly pay. Wh is eligible? Yu must be a regular emplyee wh satisfies the requirements set frth in the LTD Plan Dcument and Summary f Benefits Handbk in rder t be eligible t apply fr benefits under the LTD Plan. If yu are an IBEW- r SEIU-represented emplyee wh has attained regular status and yur nset f disability ccurred n r after January 1, 2018, yur LTD benefits will be determined accrding t the prvisins f Plan IV. Hw it wrks In general, yu will be cnsidered disabled if, by reasn f injury r illness, yu are ff wrk twelve (12) mnths r mre within an eighteen (18) mnth perid as a result f disability fr the same medical cnditin and: Are nt able t perfrm the duties f yur regular (base) classificatin; and The cmpany is unable t place yu in a psitin at PG&E fr which yu are skilled and qualified, that meets yur reduced wrk capacities, and pays at least 70% f yur basic wage rate f yur base classificatin at the time f disability. Yu must apply fr Lng Term Disability with Sedgwick, PG&E s leave f absence and disability administratr. Upn prviding a cmplete medical dcumentatin and required authrizatins, Sedgwick will determine if yur disability qualifies under the prgram. While n LTD, yu are required t cmply with all prgram prvisins in rder t remain n an apprved LTD status and receive benefits. Subject t the terms f the LTD Plan dcument, the Summary f Benefits Handbk and any Letter Agreement, yu are required t return t wrk frm LTD when: Yu are able t perfrm yur pre-disability psitins with r withut a reasnable accmmdatin; r Yu are able t perfrm anther psitin at PG&E that is vacant with r withut a reasnable accmmdatin, prvided yu are qualified t perfrm the duties f the psitin and the salary f the new psitin pays a wage that is at least 70% yur pre- disability pay rate, and the lcatin is within a cmmutable distance. If, upn return t wrk frm LTD, yu are placed in a different psitin that pays at least 70% f yur basic wage rate, the LTD Plan will pay the difference between yur pre-disability, base classificatin s basic wage rate and the new psitin fr up t 5 years. Benefits and Payments Once qualified, the LTD plan will pay up t a mnthly incme equal t 70% f yur basic mnthly rate f pay f yur base classificatin, minus any benefits yu receive frm ther surces. The perid f time yu can receive LTD benefits depends n yur disability and yu remaining qualified under all the prvisins f the LTD Plan (as discussed in mre detail in the LTD Plan dcument and the Summary f Benefits Handbk): Tw years, if yu are nt receiving Scial Security Disability Insurance benefits at the end f the tw years f LTD eligibility.
