3 HOW TO ENROLL Hw t enrll during pen enrllment. Open enrllment will be cnducted via SAP Emplyee Self Service (ESS) t enrll/change benefits. 4 ELIGIBILITY REQUIREMENTS Emplyees Eligibility T be eligible fr mst benefits, yu must be a regular emplyee wrking at least 20 hurs per week. 5 LIFE/ACCIDENTAL DEATH & DISMEMBERMENT (AD&D) BENEFITS HIGHLIGHTS Remember t name a beneficiary Yur beneficiary will receive the benefit paid by yur life insurance plicy in the event f yur death. Yur beneficiary is the persn(s) wh will receive yur life insurance benefits when yu die. Yur beneficiary can be a persn r multiple peple, charitable institutins r yur estate. Once named, yur beneficiary remains n file until yu make a change. Withut clear directin n file, yur family culd end up fighting fr yur death benefit in curt. This can take time and mney, and it s the last thing yur lved nes will want t deal with after yur death. 6 DISABILITY BENEFITS HIGHLIGHTS Shrt Term Disability (STD) insurance prvides incme prtectin in the case f a shrt-term illness. Once yu meet the eligibility waiting perid, yu will be autmatically enrlled in the emplyer-prvided basic STD cverage.
