Dental Dental is a mandatry benefit fr yu and yur family. T cverage yur family yu must enter yur spuse and dependent(s) infrmatin in the Upint prtal. Online dental enrllment/changes fr Delta Dental f Washingtn are dne by calling the WEA Select Custmer Service department tll-free at (recmmended) r by lgging int their prtal at Visin Visin is a mandatry benefit fr yu and yur family. There is n enrllment verificatin required. Life and LTD Life and LTD are mandatry benefits specific t each pling grup. 5 WAIVING MEDICAL Emplyees Waiving Medical Cverage STEP 1: All benefits eligible emplyees are t cmplete, annually, the District Medical Waiver if declining medical cverage. District Medical Waiver frm may be fund n the district website under Emplyee Resurces at the fllwing link: User Name: IssaquahSD Passwrd: IssaquahSD411 Return the frm t the Payrll Office n later than September 29, 2017 inter-district mail. Step 2: Mandatry benefits (DENTAL / VISION / LIFE / LTD) are cmpleted at this pint; hwever, yu must still click n the Submit Cverage Selectins t HR t cnfirm enrllment. During the pen enrllment perid yu can always make a change and elect t participate in medical cverage. Final deadline t submit a change is 9:59 p.m. PDT n September 29, Nte n waiving medical cverage: Please nte that unless yu experience a Qualified Life Event, yu cannt elect t participate in a district medical plan until the next Open Enrllment perid in At this time yur mandatry benefits enrllment is cmplete! Please verify: 1. MEDICAL plan electin 2. Name/DOB/SSN/Gender n Spuse/Dependent(s) 3. MEDICAL Cverage Level crrespnds with spuse / dependent(s) enrlled in yur MEDICAL plan electin. See table belw. Once yur have hit yur final Submit Cverage Selectins t HR the system will re-calculate and shw yu yur apprximate ut-f-pcket mnthly expense. Reminder: The ut-f-pcket mnthly expense is nly an estimate and will change during the pling and pen enrllment perids. Enter the fllwing infrmatin: First Name Last Name DOB Gender SSN Cverage Start Date: 11/01/2017 DO NOT TOUCH COVERAGE END DATE. Leave at 12/31/9999 Click Save after yu ve entered the required infrmatin. STOP! D NOT check any f these bxes STOP!
