1 Disability 1 PLAN HIGHLIGHTS Prvide Cverage at N Cst As part f the USEC Inc. Leave Plicy, Full-time Emplyees are prvided up t 10 days f sick leave per calendar year. Part-time Emplyees are prvided prated sick leave based n the prptin f 40 hurs that make up their regular schedule. If yu are a regular Full-time Emplyee, yu are autmatically prvided sht term disability and lng term disability benefits. If yu are a Part-time Emplyee, yu are nt eligible f sht term disability lng term disability benefits....cntinue Percentage f Pay f up t 25 Weeks If yu are disabled and have cmpleted the seven day eliminatin perid, yu are eligible f a sht term disability benefit f 66 2/3% f yur weekly earnings up t a maximum weekly benefit f $2,000 f up t 25 weeks f disability....replace Part f Yur Pay f Disabilities that Cntinue After Yur Sht Term Disability Benefits End If yu cntinue t be disabled after yur sht term disability benefits end (, if later, 180 days), the lng term disability benefit cntinues t pay 60% f yur mnthly earnings up t a maximum mnthly benefit f $10,000. Generally, lng term disability benefits cntinue until yu reach age 65 until yur disability ends, if earlier. SHORT TERM DISABILITY PLAN Sht term disability benefits are prvided thrugh insurance. The insurer will decide if yu are disabled.

2 Disability 2 Yu must be cntinuusly disabled f seven days befe sht term disability benefits are payable. Exclusins Sht term disability benefits are nt prvided f any disabilities caused by, cntributed t, resulting frm: Occupatinal illness injury, with the exceptin f disabilities due t ccupatinal illness injuries f partners sle prpriets wh cannt be cvered by a wkers cmpensatin claim Intentinally self-inflicted injuries Active participatin in a rit Lss f a prfessinal license, ccupatinal license certificatin Attempt t cmmit cmmissin f a crime under state federal law Cmmissin f a crime f which yu have been cnvicted under state federal law Disability due t war, declared undeclared, any act f war Yu will nt be paid f a benefit f any perid f disability during which yu are incarcerated. Benefit Amunt Yur sht term disability benefit is 66 2/3% f yur weekly earnings up t a maximum weekly benefit f $2,000. Weekly earnings means yur grss weekly incme in effect just pri t yur date f disability. It includes yur ttal incme pri t any deductins made f pre-tax cntributins and befe taxes. It des nt include incme received frm cmmissins, bnuses, vertime pay, any ther extra cmpensatin, incme received frm surces ther than the Cmpany. In sme cases, yu may receive sht term disability benefits even if yu wk while yu are disabled. Refer t the benefit bklet prvided by the insurer t determine hw yur benefit is calculated in this situatin. Reductin f Benefits Due t Deductible Surces f Incme Yur sht term disability benefits are reduced by deductible surces f incme that yu receive are entitled t receive because f yur disability. Deductible surces f incme that will reduce yur sht term disability benefits include, but are nt limited t:

3 Disability 3 Wkers cmpensatin benefits benefits prvided under a similar law, state disability benefits and ther statuty benefits f disability, retirement unemplyment Benefits prvided frm a third party (after subtracting attney s fee s) by judgment, settlement, therwise Disability retirement payments under the Cmpany s retirement plan (including the Pensin Plan, but nt the Savings Prgram). If subtracting deductible surces f incme results in a zer benefit, the minimum weekly payment is $25. Claiming Sht Term Disability Benefits Sht term disability benefits cannt begin until the claim fm has been satisfactily cmpleted by yu and yur physician and accepted by the Cmpany and the insurer. Yu can cntact the insurer yur lcal Human Resurces t request a claim fm f assistance in filing yur claim. If yu d nt receive a claim fm within 15 days f yur request, send the insurer written prf f claim withut waiting f the fm. Yu must send written prf f yur claim t the insurer n later than 90 days after yur seven day eliminatin perid. If it is nt pssible t give prf within 90 days, it must be given n later than ne year after the time prf is therwise required, except in the absence f legal capacity. Yu may be required t be examined by a physician, ther medical practitiner and/ vcatinal expert f the insurer s chice as ften as it is reasnable t d s, at their expense. Yu may als be required t be interviewed by an authized representative f the insurer. When Sht Term Disability Payments Begin Yu will begin t receive payments when yur claim is apprved and the eliminatin perid has been met. Payments will be sent weekly. When Sht Term Disability Payments End Prvided yu are cntinuusly disabled, yur sht term disability benefit will be paid weekly f up t 25 weeks. Yur sht term disability benefits will end n the earliest f the fllwing: When yu are able t return t wk in yur regular ccupatin n a part-time basis but yu chse nt t When yu have received sht term disability benefits f 25 weeks When yu fail t submit prf f yur cntinuing disability When yu are n lnger disabled under the terms f the plan, unless yu are eligible t receive benefits under the Rehabilitatin and Return t Wk Assistance prgram

4 Disability 4 When yu fail t cperate participate in the Rehabilitatin and Return t Wk Assistance Prgram When yur disability earnings exceed the amunt allwable under the Plan When yu are cnsidered t reside utside the United States Canada, but nly after 12 mnths f payments. Yu will be cnsidered t reside utside these cuntries when yu have been utside the United States Canada f a ttal perid f six mnths me during any 12 cnsecutive mnths f benefits. When yu die. Successive Disabilities Refer t the benefit bklet prvided by the insurer f special rules that apply when yu return t wk n a full time basis and yu are disabled again. Rehabilitatin and Return t Wk Assistance Benefit The insurer may, at its discretin, select individuals t participate in its Rehabilitatin and Return t Wk Assistance Benefit. If yu are selected t participate in this prgram, yu must participate and cmply with the prgram in der t receive sht term disability benefits. F participating in this prgram, yu will receive: 10% f yur grss disability payment up t a maximum benefit f $250 per week (nt subject t reductin f deductible surces f incme) Weekly sht term disability payments f three weeks fllwing the date yur disability ends if yu are n lnger disabled while: Yu are participating in the Rehabilitatin and Return t Wk Assistance prgram and Yu are nt able t find emplyment. Refer t the benefit bklet prvided by the insurer f me infmatin regarding the Rehabilitatin and Return t Wk Assistance Benefit.

5 Disability 5 LONG TERM DISABILITY PLAN Lng term disability benefits are prvided thrugh insurance. This sectin prvides a brief summary f the lng term disability benefits; hwever, yu must refer t the benefit bklet prvided by the insurer f cmplete infmatin regarding yur lng term disability benefits. When Yu Are Disabled Yu are disabled if yu are limited frm perfming the material and substantial duties f yur regular ccupatin due t yur sickness injury and yu have a 20% me lss in yur indexed mnthly earnings due t the same sickness injury. After 24 mnths f payments, yu are disabled if, due t the same sickness injury, yu are unable t perfm the duties f any gainful ccupatin f which yu are reasnably fitted by educatin, training, experience. The insurer will decide if yu are disabled. Yu must be cntinuusly disabled thrugh the date yur sht term disability benefits end, if later, 180 days befe lng term disability benefits are payable. Yur disability will be treated as cntinuus if yur disability stps f 30 days less during the eliminatin perid. The days yu are nt disabled will nt cunt tward yur eliminatin perid. Exclusins Lng term disability benefits are nt payable f disabilities caused by, cntributed t, resulting frm yur: Intentinally self-inflicted injury Active participatin in a rit Lss f prfessinal license, ccupatinal license certificatin Attempt t cmmit cmmissin f a crime under state federal law Cmmissin f a crime f which yu have been cnvicted under state federal law Due t war, declared undeclared, any act f war Due t a pre-existing cnditin. Pre-existing cnditin means yu received medical treatment, cnsultatin, care services including diagnstic measures, tk prescribed drugs medicines in the three mnths just pri t yur effective date f cverage; yu had symptms f which an dinarily prudent persn wuld have cnsulted a health care prvider in the three mnths just pri t yur effective date f cverage; and the disability began in the first twelve 12 mnths after yur effective date f cverage. Yu will nt be paid a benefit f any perid f disability during which yu are incarcerated.

6 Disability 6 Benefit Amunt Yur mnthly lng term disability benefit is 60% f yur mnthly earnings, up t a maximum mnthly benefit f $10,000, reduced by deductible surces f incme and disability benefits. Mnthly earnings means yur grss mnthly incme frm the Cmpany in effect just pri t yur date f disability, including yur ttal incme befe taxes (i.e., pri t any deductins f pre-tax cntributins t a qualified deferred cmpensatin plan, sectin 125 plan, flexible spending accunt). Mnthly earnings des nt include incme received frm cmmissin, bnuses, vertime pay, and any ther extra cmpensatin incme received frm surces ther than the Cmpany. In sme cases yu may receive lng term disability benefits even if yu wk while yu are disabled. Refer t the benefit bklet prvided by the insurer t determine hw yur benefit is calculated in this situatin. Reductin f Benefits Due t Deductible Surces f Incme Yur lng term disability benefits are reduced by deductible surces f incme including, but nt limited t, the fllwing: Amunts yu receive are entitled t receive frm wkers cmpensatin laws, ccupatinal disease laws, laws with similar intent Amunts yu receive are entitled t receive as disability incme payments under a state cmpulsy benefit act law, a grup insurance plan, gvernmental retirement system as a result f yur jb with the Cmpany Amunts that yu, yur spuse and yur children receive are entitled t receive as disability payments because f yur disability under Scial Security, the Canada Pensin Plan, the Quebec Pensin Plan, any similar act plan The amunt that yu receive as retirement payments the amunt that yur children receive as retirement payments because yu are receiving retirement payments under Scial Security, the Canada Pensin Plan, the Quebec Pensin Plan, any similar act plan The amunt that yu receive as disability payments under the Cmpany s Pensin Plan, the amunt that yu vluntarily elect t receive as retirement payments under the Cmpany s Pensin Plan, and the amunt yu receive as retirement payment when yu reach the later f age 62 nmal retirement age as defined in the Cmpany s Pensin Plan and The amunt that yu receive under the Jnes Act. If subtracting deductible surces f incme result in a zer benefit, the minimum mnthly payment is the greater f $100 10% f yur grss disability payment.

Limited Pay Perid f Certain Disabilities Disabilities, due t sickness injury, that are primarily based n self-repted symptms, and disabilities due t mental illness have a limited pay perid up t 24 mnths. Yu will cntinue t receive payments beynd the 24 mnth perid if yu meet ne bth f the fllwing cnditins: If yu are cnfined t a hspital institutin at the end f the 24 mnth perid, yu will cntinue t receive payments during yur cnfinement If yu are still disabled when yu are discharged, yu will receive payments f a recvery perid f up t 90 days

9 Disability 9 If yu becme re-cnfined at any time during the recvery perid and remain cnfined f at least 14 days in a rw, yu will receive payments during that additinal cnfinement and f ne additinal recvery perid up t 90 me days In additin t the abve, if, after the 24 mnth perid f which yu have received payments, yu cntinue t be disabled and subsequently becme cnfined t a hspital institutin f at least 14 days in a rw, yu will receive payments during the length f cnfinement. Yu will nt receive payments beynd the limited pay perid as indicated abve, the maximum perid f payment, whichever ccurs first. The mental illness limitatin will nt apply t dementia if it is a result f: Strke Trauma Viral infectin Alzheimer s disease Other cnditins nt listed which are nt usually treated by a mental health prvider using psychtherapy, psychtrpic drugs, ther similar methds f treatment. Successive Disabilities If yu receive lng term disability benefits, return t wk and have a recurrent disability, the disability will be treated as part f yur pri claim and yu will nt have t cmplete anther eliminatin perid if: Yu were cntinuusly insured under the plan f the perid between yur pri claim and yur recurrent disability and Yur recurrent disability ccurs within six mnths f the end f yur pri claim. Yur recurrent disability will be subject t the same terms f the plan and maximum perid f payment as yur pri claim. Any disability which ccurs after six mnths frm the date yur pri claim ended, is due t a different cause than yur pri disability, will be treated as a new claim. The new claim will be subject t all f the plicy prvisins. If yu becme entitled t payments under any ther grup lng term disability plan, yu will nt be eligible f lng term disability payments under this Plan.

10 Disability 10 Rehabilitatin and Return t Wk Assistance Benefit The insurer may, at its discretin, select individuals t participate in its Rehabilitatin and Return t Wk Assistance Benefit. If yu are selected t participate in this prgram, yu must participate and cmply with the prgram in der t receive lng term disability benefits. F participating in this prgram, yu will receive: 10% f yur grss disability payment up t a maximum benefit f $1000 per mnth (nt subject t reductin f deductible surces f incme) Mnthly lng term disability payments f three mnths fllwing the date yur disability ends if yu are n lnger disabled while: Yu are participating in the Rehabilitatin and Return t Wk Assistance prgram and Yu are nt able t find emplyment. Upn satisfacty prf that payments have been made t a child care prvider, child care expenses f $250 per mnth, per child (nt t exceed $1,000 per mnth f all eligible child care expenses cmbined) will be paid f eligible dependent children under age 15 dependent children incapable f prviding their wn care n a daily basis due t their wn physical handicap mental retardatin Refer t the benefit bklet prvided by the insurer f cmplete infmatin regarding the Rehabilitatin and Return t Wk Assistance Benefit. Wk Life Assistance Prgram Available Life Balance, an additinal service available as part f yur lng term disability benefit, is a wkplace resurce that yu can get cnsultatin and services regarding addictin and recvery, parenting and childcare, legal issues, assistance frm the insurer s Scial Security claimant advcacy prgram, emtinal well-being, financial infmatin, elder care and nursing hmes, schling and educatin, etc. Life Balance is available via face-t-face assessment, telephne interview, educatin materials, wkshps, seminars and the Internet.
