1 2012 FITNESS INCENTIVE PILOT PROGRAM FOR PSHMC HEALTH PLAN MEMBERS The fitness incentive fr Medical Center health plan member is changing. Currently health plan member are reimbursed if they attend a gym at least 100 times per calendar year. This prgram nly engaged abut 300 emplyees and there were instances reprted f emplyees signing int their gym and leaving just t btain 100 visits. Under the guidance f the Medical Center s Wellness Advisry Grup, the prgram has been revised t prmte healthier habits and wellness amng Medical Center health plan members. Eligible participants must be cvered under the Medical Center health plan. The new plan fr calendar year 2012 allws each health plan member/spuse/partner, and dependents (age 12 and up) t attend a gym fr three free cnsecutive mnths (ninety days) and prmtes the fllwing bjectives: Remve barriers t participatin in a fitness facility. Use the allcated funds t engage mre health plan members. Encurage mre persns t start a new, regular exercise rutine. Encurage mre members t cntinue their exercise rutine by prviding a benefit that will be utilized and valued. HOW TO PARTICIPATE The prgram begins n January 1, 2012 and cvers membership dates frm Jan. 1 Dec. 31, The three mnths is defined as 90 days and must be used cnsecutively. The chice f facility is nt limited t certain gyms. Qualifying facilities fr the new plan have been expanded t include swimming, yga, persnal training, and martial arts studis. There are three ptins under the new prgram. Chse the ne that wrks best fr yu: Optin #1: University Fitness Center at main Hershey campus Existing* UFC member with payrll deductin: Yu d nt need t make any changes t yur membership if yu have autmatic payrll deductin. Yur 3-mnth membership credit will arrive in a check frm Highmark Blueshield t yur hme address by June 1, Health plan member must be emplyed thrugh June 1, 2012 t receive the credit. Existing* UFC member with direct pay: Pay yur UFC membership as usual. Yur 3-mnth membership credit will arrive in a check frm Highmark Blueshield t yur hme address by June 1, Health plan member must be emplyed thrugh June 1 t receive the credit. Yu must verify with UFC that they have yur 5 digit Lawsn ID if yu carry the insurance r yur spuse s/partner s Lawsn ID if he/she carries yu n their insurance. New UFC member: Sign up and pay fr whichever UFC plan best suits yu (quarterly, semi-annual r annual). Yur 3-mnth membership credit will arrive in a check frm Highmark Blueshield t yur hme address after the 3-mnth mark has ccurred, allwing 60 days t prcess. Health plan member must still be emplyed at the time the credit is due. 1

3 Optin #3: Out-f-Netwrk Gym If yu belng t r jin an ut-f-netwrk gym (nt the University Fitness Center r a facility included n the in-netwrk list) yu will need t cmplete a reimbursement frm t receive credit fr up t three mnths. A reimbursement will nt be issued until the cmpletin f the three mnth membership perid. The frm may be btained frm the Blueprints fr Wellness website at pennstatehershey.rg/wellness r by cntacting wellness@hmc.psu.edu. It will als be available n the Human Resurces Infnet page under Frms. Yes, a spuse/partner cvered under the medical center grup health plan is als eligible fr this benefit. Can my child receive this benefit? Yes, any dependent age 12 and lder cvered under the medical center grup health plan is eligible fr this benefit. Nte: minimum age requirements fr children vary frm gym t gym. My gym is nt n the in-netwrk list. Can I still g and get my three mnth credit? Yes, please see ptin #3. Yu will need t cmplete a reimbursement frm t receive credit fr up t three mnths. Where is the reimbursement frm? When will it be psted? N reimbursements will be issued until the cmpletin f the three mnth membership perid. Therefre the earliest pssible date t submit a cmpleted frm is March 30. T minimize cnfusin between frms, the 2012 reimbursement frm will be published after the cmpletin f the 2011 reimbursement prcess. Sme facilities were nt able t participate as an in-netwrk gym due t cst restraints, peratinal limitatins, r lgistic cnstraints. Every effrt was made t include gyms in a variety f gegraphic lcatins and appealing t a variety f participants. 4

5 D persnal training studis cunt? Yes. There are nt persnal training studis in-netwrk. Yu will need t cmplete a reimbursement frm t receive credit fr up t three mnths. D martial arts studis cunt? Yes. There are nt martial arts studis in-netwrk. Yu will need t cmplete a reimbursement frm t receive credit fr up t three mnths. Des swimming cunt? Yes, fitness swimming cunts in lap pls and pls assciated with fitness facilities. The benefit will nt cver a summer membership t a pl fr seasnal recreatin purpses. If any gym cunts, why is there even a list f in-netwrk facilities? The in-netwrk facilities have agreed t wrk directly with the Medical Center t administer the benefit. This arrangement shuld simplify the prcess fr participants t participate in an in-netwrk gym. Additinally, sme f the in-netwrks gyms have negtiated a special membership rate beynd the three mnth trial perid shuld a participant decide t jin that facility. D I nly have t exercise fr 3 mnths/year t be healthy? Fr the best benefits yu shuld engage in physical activity several times per week thrughut the year. Fr thse new t exercise, the three mnth benefit is designed t help yu get started with a new exercise rutine and hpe that yu will cntinue the remainder f the year. Fr seasned gym-gers this benefit is designed t supprt yur regular active lifestyle in a way that is meaningful. All participants must attend the fitness facility at least nce during the 3 mnths t receive credit. Des this benefit expire?
