1 TWU OFFICE OF RESEARCH & SPONSORED PROGRAMS INSTRUCTIONS FOR USING THE TWU PROPOSAL APPROVAL ROUTING FORM Phne: (940) Website: WHEN TO SUBMIT THROUGH RESEARCH & SPONSORED PROGRAMS In accrdance with TWU Prpsal Submissin Plicy 1.25, all prpsals submitted fr external funding are ruted thrugh either the Office f Research & Spnsred Prgrams (ORSP) r Institutinal Develpment. Prpsals incrprating the fllwing elements shuld be ruted thrugh ORSP: gvernment funding surce (city, state, r federal); research; indirect (facility & administrative) csts; use f human subjects, animals in research, radiactive materials r devices, bihazardus agents, and/r recmbinant DNA materials; peer review; cntracts r subcntracts; and/r strict reprting requirements. Prpsals fr private funding that d nt include the abve elements are typically submitted thrugh Institutinal Develpment. ORSP can assist in determining which ffice t submit the prpsal. REVIEW, ROUTING, APPROVAL AND SUBMISSION TIMELINE The items belw shuld be submitted t ORSP at least 10 days prir t the submissin date f the prpsal. This will allw ORSP the apprpriate amunt f time t check fr cmpliance against the guidelines and btain the final apprval f the apprpriate vice president. 1. Prpsal Apprval Ruting Frm signed by the PI, C-PI, Chairs/Directrs/Assciate Deans, and Academic Deans (as applicable); 2. Abstract; 3. Prpsal narrative and/r methdlgy; and 4. Budget and budget narrative. If the prject requires technlgy supprt beynd that which is nrmally prvided by TWU, the prject must be reviewed by the Office f Technlgy. Facilities changes r large equipment purchases need t be reviewed by Facilities Management and Cnstructin in rder t determine whether existing facilities can accmmdate the new equipment. The prpsal must be apprved by ORSP and a vice president prir t submissin. ORSP can assist in btaining the signature f the apprpriate vice president. Typically, ORSP submits the prpsal electrnically r via mail. The riginal Prpsal Apprval Ruting Frm, a cpy f the submitted prpsal, and the guidelines will be filed in the Office f Research & Spnsred Prgrams.

2 COMPLETING THE FORM Please type. 1. TEXAS WOMAN S UNIVERSITY PRINCIPAL INVESTIGATOR/PROJECT DIRECTOR The term Principal Investigatr (used interchangeably with Prject Directr) refers t the designated faculty r staff member wh will be in charge f the prject if it receives funding. Generally, the Principal Investigatr (PI) is a tenured / tenure-track faculty member at TWU. The PI must be a salaried emplyee f TWU at the time the prpsal is submitted and when the award is funded. The PI is respnsible fr entering the infrmatin requested n the apprval ruting frm and initiating the ruting fr signatures. Cmplete the name, title and cntact infrmatin fr all TWU Principle Investigatrs and all TWU C- Investigatrs. 2. Prject Infrmatin 2 A Prpsal title Enter the title f the prpsal prject. This title will be used n award dcumentatin, TWU reprts, and pssibly news releases. When chsing a title, please cmply with the spnsr guidelines. 2 B Prject Type Research Prject is directed tward 1) increasing knwledge in science, 2) designing, develping, r testing f specific items r prcesses, r 3) determining the ptential f discveries in technlgy, materials, prcesses, methds, devices r techniques. If research, indicate whether r nt this prject is a clinical trial. Training/Instructin - Prjects that incrprate teaching as a main bjective. Service - Prject invlves the perfrmance f wrk ther than instructin/training r research, i.e. health and cmmunity service prjects Schlarship r Fellwship 2 C Prject Start and end dates Enter the prject start date and end date. This perid shuld cver all budget perids. 2 D Ttal Amunt Requested Fill in all spaces. Funding infrmatin is fr this prpsal nly. This infrmatin must be cnsistent with the budget spreadsheet develped by ORSP. If F&A (Indirect Csts) are nt being charged, please enter Zer.

3 3. Funding Agency Infrmatin 3A Funding Agency Name Type in the cmplete name f the Spnsr t which the prpsal will be submitted. 3B Is this a Subcntract? Indicate whether r nt this is a subcntract t TWU. If it is a subcntract, please give the fficial name f the subcntracting agency. 3C Cmpetitin/RFA Enter the Slicitatin Name and Number and/r the Slicitatin URL. 3D Spnsr Cntact Infrmatin Prvide the spnsr cntact infrmatin, including the cntact Name, phne number, , and web address. 3E Spnsr Deadline Enter the spnsr's due date and time fr submissin f this prpsal. 3F Methd f Submissin Check the crrect bx f the methd f prpsal submissin. 4. Institutinal Cmmitment f Resurces If cst sharing r matching is part f this prpsal, please indicate whether it is required by the spnsr, prvide a cncise descriptin f the methd f cst sharing, and the accunt numbers which will be used fr the cst sharing cmmitment. Please describe any wrklad reassignment r curse release fr any f the faculty listed in the prpsal. If yu anticipate rental space, cnstructin r renvatin will be required t huse this prject r any equipment acquired fr this prject, yu must crdinate an advance apprval with the Department Chair r Dean's ffice. Large equipment purchases may need t be reviewed by Facilities Management and Cnstructin (FMC) t determine whether existing facilities can accmmdate the new equipment. If this prject requires the purchase f infrmatin technlgy equipment r has any special infrmatin technlgy requirements, please explain. The Office f Technlgy may need t review and apprve the arrangements. 5. Cmpliance Requirements Check the crrect bx fr each f these items, as applicable t this prject.

4 6. Descriptin Enter a lay descriptin f n mre than 2-3 sentences. This descriptin may be used n award dcumentatin, TWU reprts, and pssibly news releases. 7. Investigatr Certificatins 8. Apprvals The TWU PI must sign in this sectin. When multiple PIs are prpsed in an applicatin, all named investigatrs must sign. Investigatr signatures certify agreement t abide by current University and federal plicies n financial cnflict f interest, intellectual prperty, the use f human subjects, and vertebrate animals in research, and ther University research plicies as apprpriate. Each investigatr (1) certifies that the infrmatin submitted within the applicatin is true, cmplete and accurate t the best f the investigatr's knwledge, and (2) agrees t accept respnsibility fr the scientific cnduct and/r carrying ut f the prject. The PI s Department Chair (Department Head, r Directr) and the PI s Dean, as wells as the Department Chairs and Deans f all ther cllabrating TWU departments and clleges (if applicable) must sign the apprval ruting frm t certify they have reviewed the prpsal and apprve the prject. ADDENDUM FOR SIGNIFICANT FINANCIAL CONFLICT OF INTERESTS The addendum page must be cmpleted fr Public Health Service (including NIH) and Natinal Science Fundatin prpsals. List all prject persnnel n the addendum frm. The date f the mst recent significant financial cnflict f interest (SFCOI) disclsure frm n file must be included fr each persn listed as a prject directr, principal investigatr, r ther key persnnel in the grant applicatin r wh is respnsible fr the design, cnduct, r reprting f research. Their SFCOI frm must have been filed within the last 12 mnths. A prpsal t the PSH r NSF may nt be submitted until this addendum is cmplete. DEFINITION Significant Financial Interest (SFI): A financial interest, as defined by the Cde f Federal Regulatins (42 CFR 50), cnsists f ne r mre f the fllwing interests f the investigatr (and thse f the investigatr's spuse and dependent children) that reasnably appears t be related t the investigatr's institutinal respnsibilities: With regard t any publicly traded entity, an SFI exists if the value f any remuneratin received frm the entity in the 12 mnths prir t the disclsure and the value f any equity interest in the entity as f the date f disclsure, when aggregated, exceeds $5,000.
