The Trillium Cmmunity Health Plan Innvatin Fund is designed t help prviders and cmmunity stakehlders drive innvative slutins that imprve the health f Trillium members. Trillium is a Crdinated Care Organizatin that prvides Oregn Health Plan (Medicaid) benefits fr ver 90,000 residents f Lane Cunty and the Reedsprt cmmunity f Duglas Cunty. We partner with lcal rganizatins t prvide integrated primary and behaviral healthcare fcused n addressing the scial determinants f health t remve barriers and imprve health utcmes. Wh can apply? We are seeking prpsals frm individual agencies r multiple agencies that wuld like t partner tgether n prjects that address ne r mre f the fllwing Innvatin Fund Fcus Areas: Behaviral health integratin Oral health integratin Scial determinants f health CCO quality metrics Alternative payment mdels Value-based prgrams Cst cntainment An agency may submit a prpsal bth individually and as a member f a partnership with ther agencies. An agency r partnership may apply fr 2018 and/r 2019 funding. Prpsals fr 2018 funding are due Octber 31, Prpsals fr 2019 funding may be submitted at any time between January 1 and July 31, 2019 r until the available funding is exhausted, whichever is earlier.

2 What are the funding amunts? Funding amunts fr 2018 range frm a minimum f $50,000 t a maximum f $600,000. Funding cannt be carried ver int Funding amunts fr 2019 range frm a minimum f $50,000 t a maximum f $1 millin. Funding terminates after 12 mnths f prject peratin, r the end f calendar year 2019, whichever is sner. Any required funding after year 1 f the prject will depend n the prpsal s business plan, results during year 1, and/r securing the necessary cntracts/agreements with Trillium r ther entities. Depending n the structure f the prject, funds may be disbursed at the beginning f the prject r peridically thrughut it. Funding may be linked in whle r in part t meeting gals r metrics in the prpsal. Cmpleted prpsals submitted by the deadline will be screened by Trillium Strategic Surcing and evaluated by the Trillium Leadership Team and Finance Advisry Cmmittee. Their recmmendatins will be frwarded t the Trillium Bard f Directrs fr final review and decisin.
