1 Alpena Cunty Yuth & Recreatin Millage Grant Applicatin fr 2018 Funding Alpena Cunty welcmes relevant 501(c) 3 rganizatins t apply fr funds which are awarded n a calendaryear basis. This grant applicatin is due n later than 4 PM n May 1, 2017 with grant funds released fr prgrams and prjects after January 1, Hand-deliver r mail grants t: Alpena Cunty Yuth & Recreatin Cmmittee c/ Alpena Cunty Bard f Cmmissiners 720 W. Chishlm St., Suite 7 Alpena, MI T be cnsidered, grantees must: Be recgnized as a 501 (c) 3 nn-prfit charitable rganizatin. Schls & gvernment entities are als eligible. First time applicants must prvide cpies f by-laws, perating prcedures and perating bard structure, including a primary cntact persn and bard treasurer; Abide by the Cunty f Alpena s accuntability standards (attached) fr nnprfit rganizatins; Submit a fully cmpleted applicatin, including budget and budget narrative; Present yur prgram/prject prpsal (after May 1) t the Yuth & Recreatin Cmmittee; A cpy f Liability Insurance will be required by the Cunty if applicatin is apprved; Attend a mandatry Y&R Meeting n December 7, 2017 t fulfill yur funding cntract in rder t btain funding. Nn-attendance may result in frfeiture f grant funds. Cmplete a Funding Reprt (including phts) at the culminatin f yur prject/prgram. T be cnsidered, grants must: Prvide recreatinal benefits t citizens f Alpena Cunty; Be typed n this applicatin frm and three-hle punched; Cntain the riginal 3-hle punched applicatin and ten (10) additinal 3-hle punched cpies; Prvide at least tw (2) estimates, qutes r ther pertinent infrmatin regarding yur grant; Be submitted n r befre the deadline. Please d nt submit grants that are drilled, spiralbund r cntain staples. Use f temprary clips, such as paper r binder clips, are permitted when submitting yur applicatin and its 10 cpies. Mst applicatins are submitted in large manila envelpes.
