Intrductin Kansas Health Fundatin Healthy Living Grants - Request fr Prpsals Thrugh this Request fr Prpsals (RFP), the Kansas Health Fundatin (Fundatin) will fund rganizatins t implement evidence-based strategies that prmte and supprt healthy living in Kansas, specifically activities that supprt ne r mre f the fllwing: a) healthy eating, b) active living, and/r c) tbacc use preventin. The Fundatin is particularly interested in strategies that will imprve the health f lw-incme Kansans and/r ther ppulatins disprprtinately affected by besity/tbacc-related chrnic diseases. Smking and besity 1 remain the leading causes f preventable death and illness in Kansas, csting ur state ver $2 billin 2 annually in unnecessary health care csts. Nearly ne-in-three (29.9%) Kansas adults is bese and ne-in-five (19.4%) smkes 3. Amng Kansas high schl students, ne-in-ten (10.2%) is bese and ne-in-five (20.8%) smke r use ther tbacc prducts. 4 The prblems f besity and tbacc use are increasingly cncentrated amng Kansans f lwer sciecnmic status, further cmpunding health and ecnmic disparities in ur state. This RFP represents ne cmpnent f the Fundatin s effrts t address these prblems and is intended t supprt the wrk f rganizatins cmmitted and well-psitined t imprve the health f all Kansans. T make greater prgress n these critical health issues, this RFP will supprt implementatin f evidence-based healthy eating, active living and tbacc preventin strategies in Kansas. Fr purpses f this RFP, evidence-based strategies refers t thse recmmended by leading health rganizatins (i.e., the Centers fr Disease Cntrl and Preventin, the Institute f Medicine, the Cmmunity Preventive Services Taskfrce) fr the preventin f tbacc use and besity, r are therwise supprted by strng research and evaluatin findings. The Fundatin is particularly interested in supprting the evidence-based strategies identified by Kansas stakehlders during the 2012 Kansas Obesity Summit and thrugh develpment f the Kansas Chrnic Disease State Plan ( ), and thse strategies recmmended t achieve the Healthy Kansans 2020 bjectives. Applicants will describe and substantiate the evidence base fr their prpsed strategies. PSE Strategies vs. Prgrams fr Individual Behavir Change While there are a wide range f ptential evidence-based strategies that supprt healthy living, the Fundatin recgnizes that persnal changes are made in the cntext f the larger envirnment and that many health prblems are related t cnditins utside individuals cntrl. As a result, the Fundatin is particularly interested in funding strategies that lead t implementatin f plicy, systems and envirnmental (PSE) interventins that supprt healthy eating, active living and tbacc preventin. Fr purpses f this RFP, applicants shuld use the fllwing definitins fr PSE interventins: 1 The Fundatin uses the term, besity as the prxy fr the many negative health effects assciated with lack f physical activity and an unhealthy diet. We believe increased physical activity and healthy eating are healthful regardless f weight classificatin, but als recgnize the fields f medicine and public health utilize the term and indicatr besity t represent a leading health prblem in the United States. 2 Surces: Campaign fr Tbacc Free Kids and Kansas Department f Health and Envirnment (KDHE) 3 Surce: 2012 Behaviral Risk Factr Surveillance System (BRFSS) 4 Surce: 2011 Yuth Risk Behavir System (YRBS) 1

2 Plicy strategies include laws, rdinances, reslutins, regulatin r written rules. Examples include rganizatinal plicies that prvide time ff during wrk hurs fr physical activity r ensure healthy fd and beverage items are available in wrksite cafeterias. Systems strategies impact pssible chices and behavirs acrss an rganizatin, institutin r system. Examples include changes t the schl system t supprt physical activity thrughut the schl day, and changes t health systems t ensure that each tbacc using patient is given evidence-based supprt. Envirnmental strategies invlve physical r material changes t the ecnmic, scial r physical envirnment. Examples include incrprating sidewalks, paths and recreatin areas int cmmunity design. The Fundatin views PSE interventins as benefiting any individual wh interacts with a brader system r envirnment. Prgrams fr Individual Behavir Change, n the ther hand, are designed t supprt behavir change amng a specified number f individuals thrugh the delivery f infrmatin r services. Thrugh this RFP, the Fundatin will nly fund Prgrams fr Individual Behavir Change that supprt brader changes in plicies, systems r envirnments. Fr example, training health care prviders t screen fr tbacc use and prvide brief cunseling/referral is a prgram that may be eligible fr funding if the training is part f a brader effrt t institute an evidence-based interventin within a health care system. It is the respnsibility f the applicant t clearly describe any Prgram fr Individual Behavir Change being prpsed and the extent t which it is related t a PSE interventin. Thrugh this RFP, grant funds can supprt activities t implement a PSE interventin that has already been adpted by a decisin-making bdy and activities likely t result in PSE interventins being adpted ver the curse f the grant term. The Fundatin anticipates funding a range f cmmunicatin and advcacy effrts thrugh this RFP that will strengthen supprt fr PSE interventins. II. Required Letter f Intent and Applicatin Deadline Applicants are required t ntify the Fundatin f their intent t submit a prpsal by 5 p.m. n February 25, Prpsals submitted by rganizatins that d nt submit a letter f intent by the deadline will nt be reviewed. Organizatins submitting letters f intent are nt required t submit a full prpsal. The letter f intent must include: 1. Name and address f the applicant rganizatin; 2. Primary cntact persn s name, title and cntact infrmatin; 3. Identificatin f the fcus area(s) that will be the subject f the prpsal (applicants may chse multiple fcus areas): Healthy eating Active living Tbacc preventin 4. Descriptin f prpsal (shuld be tw paragraphs r less). Submit letters f intent via t Elissa Buller, grants fficer, at ebuller@khf.rg, with the subject Healthy Living Grant Letter f Intent. Grant prpsals must be submitted via the Fundatin s electrnic submissin prcess by 5 pm n March 28, Applicants shuld plan t submit the prpsal prir t the deadline t accmmdate any technical issues that may arise. 2

Wh May Apply The Fundatin anticipates a brad range f rganizatins will be supprted thrugh this RFP, which may include: Organizatins representing perspectives f ppulatin grups disprprtinately affected by besity and tbacc use; Health care prviders and systems; Lcal and state gvernment; Academic institutins; Husing authrities; Organizatins with expertise in chrnic disease preventin and health prmtin; Lcal tbacc r chrnic disease preventin calitins; Organizatins serving schl systems and childcare prviders; Membership rganizatins; Parent rganizatins; and Neighbrhd assciatins. Eligible applicants are nnprfit and gvernment rganizatins well-psitined t implement strategies related t the RFP bjectives. Applicant rganizatins must be classified as taxexempt under Sectin 501(c)(3) f the Internal Revenue Cde. Private fundatins and Internal Revenue Cde Sectin 509(a)(3) rganizatins are nt eligible fr funds thrugh this initiative. T better ensure sustainability f the grant-funded activities, applicant rganizatin s annual revenue (minus any existing grants frm the Fundatin) must exceed the requested annual amunt. T avid any real r perceived cnflicts f interest, the Fundatin reserves the right t nt fund rganizatins that cntract with r receive funding frm, including grants r spnsrships, tbacc cmpanies r sft drink manufacturers. Applicants must include a signed statement describing any financial relatinship with tbacc cmpanies r sft drink manufacturers. IV. Available Funding and Grant Perid Applicants are eligible t apply fr a ttal award amunt up t $100,000. The maximum grant perid is 24-mnths beginning June 1, 2014 and ending May 31, If desired, applicants may prpse a shrter prject perid than the maximum 24-mnths. The full grant payment will be made at the start f the grant term. The Fundatin anticipates awarding up t $2 millin thrugh this RFP. V. Grant Exclusins Grant funds may nt be used t supprt: Lbbying as defined by the US Internal Revenue Cde (IRC), sectin 4945(d)(1). A wide range f advcacy activities that d nt represent lbbying may be supprted thrugh Healthy Living funding. Prgrams fr Individual Behavir Change (i.e., weight lss prgrams, chrnic disease selfmanagement prgrams, r the prvisin f tbacc cessatin services) will nt be cnsidered unless they are clearly part f an verall PSE strategy. Cmmunity Design Cnstructin (i.e., pedestrian walkways and bicycle paths) will nt be cnsidered unless it is clearly part f an verall PSE strategy. Any f the fllwing activities: Medical research 3

Financial Dcuments (if applicable) When reviewing prpsals, preference will be given t thse that clearly demnstrate ne r mre f the fllwing: A strng multi-sectr apprach; Strategies that leverage r build upn effrts funded by ther state, federal r fundatin grants; Strategies that will imprve the health f lw-incme Kansans and/r ther ppulatins disprprtinately affected by besity/tbacc-related chrnic disease. A. Cver Page The Cver Page is cmpleted nline and requires the fllwing infrmatin: Organizatin infrmatin (including rganizatin name, address, phne, , etc.) Primary cntact infrmatin Prject Term B. Prpsal Narrative The Prpsal Narrative is limited t 15 single-spaced pages and must be develped using 12- pint Times New Rman fnt and ne-inch margins. Clearly rganize the Prpsal Narrative using the fllwing six sectins. 1. Executive Summary (up t ne single-spaced page) This sectin is nt scred, but shuld prvide the Fundatin and reviewers with an verview f the prpsed activities and intended utcmes. 4

5 Prvide the fllwing n the Executive Summary page: i. Prject title. ii. Requested funding amunt. iii. Fcus f the grant prpsal: healthy eating, active living and/r tbacc preventin/cessatin. iv. Brief narrative verview f the prpsal. The Fundatin will likely draw frm this narrative verview when annuncing grant awards. 2. Applicant Capacity and Experience (up t three single-spaced pages) Describe applicant rganizatin s missin and hw the Healthy Living prpsal is related t that missin. Describe applicant s capability and resurces t ensure timely start-up and implementatin f the prpsed activities. Identify and describe qualificatins and related experience f key persnnel wh wuld direct/versee the grant-funded activities. Describe applicant s experience with similar prjects. Identify any ther rganizatins that will serve in key rles fr the prject. 3. Ppulatin and Statement f Need (up t tw single-spaced pages) Identify and describe the ppulatin and/r rganizatins that wuld be impacted by the prpsed activities. Describe the impact f besity and/r tbacc use n this ppulatin. Substantiate statement f need using relevant data. 4. Prpsed Prject Plan and Timeline (up t five single-spaced pages) Prvide a narrative descriptin f the prpsed strategies and implementatin plan. If yur prpsal includes prgrams fr individual behavir change r cmmunity design cnstructin, clearly describe hw these activities will supprt a brader systems and/r envirnmental change. The Fundatin will nt prvide grant funds fr the sle purpse f supprting prgrams r cmmunity design cnstructin. Discuss hw the effrts and/r anticipated utcmes will be sustained beynd the funding perid. On a single page, included in the five-page ttal fr this sectin, prvide a timeline that clearly identifies the majr activities that wuld ccur during each quarter (three-mnth perid) f the grant term. This sectin has ne f the highest pint values fr the prpsal. Ensure adequate detail is prvided t allw reviewers t clearly understand the prpsed prject. 5. Prpsed Prject Objectives (up t ne single-spaced page) Prvide at least five SMART (specific, measurable, attainable, relevant and time-bund) bjectives fr the prpsal. Refer t the Fundatin s guidance dcument fr develping SMART bjectives which is attached t the RFP. At least ne bjective must be related t the anticipated impact the grantfunded initiative will have n the ppulatin. Prpsed bjectives shuld reflect what applicant rganizatin expects t achieve and will be able t measure ver the curse f the grant term. Grantees will reprt prgress tward the prpsed bjectives n tw ccasins: 1) after the first 12 mnths (July 1, 2015) and at the cnclusin f the grant term. 5

6 Grantees prpsed bjectives will be used by external evaluatrs and Fundatin staff t assess the impact f funded grants. 6. Summary f Evidence fr Prpsed Strategies (up t three single-spaced pages) Prvide an verview f the evidence t supprt the prpsed strategies. Describe why applicant rganizatin believes the prpsed strategies are evidence-based. Cite reprts, publicatins r evaluatin findings that supprt the prpsed strategies. C. Budget and Justificatin Template The Budget and Justificatin Template must be submitted with the prpsal. Prvide a clear budget narrative and justificatin fr all ptential csts. D. Financial Relatinship Statement Template Applicants must submit a signed Financial Relatinship Statement Template t verify the rganizatin has n cntracts with and receives n funding frm, including grants r spnsrships, tbacc cmpanies r sft drink manufacturers r describe any such relatinships. E. Acknwledgement f Agreement Review T ensure applicant rganizatin is able t accept a grant frm the Fundatin, the rganizatin must submit a signed Acknwledgement f Agreement Review. 12 Develping SMART Objectives SMART Objectives One way t develp well-written bjectives is t use the SMART apprach. Develping specific, measurable bjectives requires time, rderly thinking, and a clear picture f the results expected frm prgram activities. The mre specific yur bjectives are, the easier it will be t demnstrate success. SMART stands fr Specific Measurable Attainable/Achievable Relevant Time bund Specific - What exactly are we ging t d fr whm? The specific part f an bjective tells us what will change fr whm in cncrete terms. It identifies the ppulatin r setting, and specific actins that will result. In sme cases it is apprpriate t indicate hw the change will be implemented (e.g., thrugh training). Crdinate, partner, supprt, facilitate, and enhance are nt gd verbs t use in bjectives because they are vague and difficult t measure. On the ther hand, verbs such as prvide, train, publish, increase, decrease, schedule r purchase indicate clearly what will be dne. Measurable - Is it quantifiable and can WE measure it? Measurable implies the ability t cunt r therwise quantify an activity r its results. It als means that the surce f and mechanism fr cllecting measurement data are identified, and that cllectin f these data is feasible fr yur prgram r partners. A baseline measurement is required t dcument change (e.g., t measure percentage increase r decrease). If the baseline is unknwn r will be measured as a first activity step, that is indicated in the bjective as, baseline t be determined using XXX database, 20XX. The data surce yu are using and the year the baseline was btained is always specified in yur bjective statement. If a specific measurement instrument is used, yu might want t incrprate its use int the bjective. Anther imprtant cnsideratin is whether change can be measured in a meaningful and interpretable way given the accuracy f the measurement tl and methd. Attainable/Achievable - Can we get it dne in the prpsed time frame with the resurces and supprt we have available? The bjective must be feasible with the available resurces, apprpriately limited in scpe, and within the prgram s cntrl and influence. Smetimes, specifying an expected level f change can be tricky. T help identify a target, talk with an epidemilgist, lk at histrical trends, read reprts r articles published in the scientific r ther literature, lk at natinal expectatins fr change, and lk at prgrams with similar bjectives. Cnsult with partners r stakehlders abut their experiences. Often,

13 talking t thers wh have implemented similar prgrams r interventins can prvide yu with infrmatin abut expected change. In sme situatins, it is mre imprtant t cnsider the percentage f change as a number f peple when discussing impact. Will the effrt required t create the amunt f change be a gd use f yur limited resurces? Relevant - Will this bjective have an effect n the desired gal r strategy? Relevant relates t the relatinship between the bjective and the verall gals f the prgram r purpse f the interventin. Evidence f relevancy can cme frm a literature review, best practices, r yur thery f change. Time bund - When will this bjective be accmplished? A specified and reasnable time frame is incrprated int the bjective statement. This takes int cnsideratin the envirnment in which the change must be achieved, the scpe f the change expected, and hw it fits int the verall wrk plan. It may be indicated as, By December 2010, the prgram will r, Within six mnths f receiving the grant... Using SMART Objectives Writing SMART bjectives als helps yu t think abut and identify elements f the evaluatin plan and measurement, namely indicatrs and perfrmance measures. An indicatr is what yu will measure t btain bservable evidence f accmplishments, changes made, r prgress achieved. Indicatrs describe the type f data yu will need t answer yur evaluatin questins. A SMART bjective ften tells yu what yu will measure. A perfrmance measure is the amunt f change r prgress achieved tward a specific gal r bjective. SMART bjectives can serve as yur perfrmance measures because they prvide the specific infrmatin needed t identify expected results.
