We are very excited abut this year s event, and anticipate that mre than 20,000 peple will enjy these festivities lcated in Memrial Park and thrughut dwntwn Wheatn. This year s festival will ffer a 5/10K Run fr the Animals t benefit Csley Z, and expanded art fair, children s entertainment, carnival rides, beer tent, business exp and a fine entertainment line-up. One f the key elements f the fest that participants lk frward t is ur Fd Vendr rw, featuring lcal and fest treats. We are currently accepting applicatins fr fd vendrs fr the event. We will have limited number f electrical hk-ups available n a first-cme, first served reservatin basis. 2 The Taste f Wheatn is a high quality event and we are attempting t prvide a level f exclusivity fr fd items prvided by each vendr. The items will be determined by the Taste f Wheatn Cmmittee, based n member affiliatin, past participatin and the menu items yu list n yur applicatin. Yu will be able t list up t six items n the applicatin but will nly be able t sell three (3) items per fd bth. We are asking fr additinal items s that we can avid duplicate items at the fd bths. If yu want t sell mre than three (3) items, yu will need t purchase an additinal bth. We will begin t assign bths by Tuesday, April 24, We encurage yu t return yur applicatin t us befre this date t ensure cnsideratin. Applicatins received after this date will incur a $40 late fee. Accepted vendrs will be cntacted by Tuesday, May 8, 2018 with a cnfirmatin . Guidelines t fllw: - Yur payment (in the frm f a certified check, mney rder, credit card r cash) must accmpany yur applicatin. Payment shuld be made t: Wheatn Park District. Failure t cmply with this will result in dismissal frm the event withut a refund. - Yu must cmplete the electric needs and utlet request n yur applicatin. - Yu must cntact the DuPage Cunty Health Department prir t the event t register fr yur permit and ensure yu are aware f any changes in regulatins (infrmatin included in this packet). - The Wheatn Park District and the Wheatn Chamber f Cmmerce will be selling f sda and water! 4 DuPage Cunty Health Department Permit Applicatin Steps 1. Review the Temprary Fd Service Requirements Bklet nline t learn abut fd safety at temprary fd events. 2. Cmplete the Temprary Fd Service Applicatin. 3. Submit the applicatin frm and fee (see belw) t the apprpriate Public Health Center at least 10 days prir t the event. 4. After yur permit applicatin is prcessed, the permit will be given t yu n-site after an inspectin. Fees and requirements fr Temprary Fd Service Permits are based n a risk type. Yur risk type is determined by the menu items and the amunt f fd preparatin t be dne n-site (see chart belw). Final determinatin f risk type and fee are made by the area Sanitarian.
