Imprtant Ntes: Chapter Dues: If APhA is cllecting yur Chapter Dues, they will be included in the ttal amunt due in additin t Natinal Dues. State Dues: If yur schl and state have agreed t have APhA cllect yur State Assciatin Dues, they will als be included in the ttal amunt due in additin t Natinal and Chapter Dues. 1. PAC (Plitical Actin Cmmittee) dnatins are accepted when submitting membership renewals nly. NEW MEMBERS WILL NOT BE GIVEN THE OPTION TO MAKE THIS DONATION; this ptin will nly be available t renewing members. 2. Membership Uplad Files can be prvided by chapters that can submit ONE PAYMENT (either check r credit card) alng with a file using a specific template that includes student member cntact infrmatin. APhA cannt prcess yur payment unless every student has an address, member ID, and a graduatin date listed. 3. Chapter Administratr Accunts were created fr each chapter and include nline access t all chapter rsters and reprts. Chapter Advisrs shuld designate a chapter leader t generate reprts and files and/r update chapter fficers in essence, share the lad! 4 Submitting Student Membership Enrllment Frms Please d nt ffer paper enrllment frms unless a student encunters a prblem with Online Enrllment. Cmpleted paper frms shuld be mailed back t APhA (at the address belw). It is imprtant t ensure that all f the requested infrmatin n the frm is filled ut. If the frms are nt prperly cmpleted, they will be returned t the MVP and the delay may negatively impact yur chapter enrllment prcess. Cmpletely fill ut the Student Membership Enrllment Frm and be sure that all infrmatin is legible. All the infrmatin listed belw is required! (Fr example: Receiving 50 enrllment frms with a check amunt fr 51 students can result in a delay f prcessing fr all student members.) 3.
