2 Page 2 Cntact/Renter Initials: Date: CONDITIONS AND RESPONSIBILITIES OF RENTER: Please read the material belw t make sure all parties understand the requirements f prviding fr everyne s safety and keeping the Center fr African American Histry, Art and Culture (CAAHAC) a well maintained and safe lcatin fr future use. DEPOSIT/RENTAL FEES: a signed cntract and date hld depsit in the amunt f $ (per each rental perid) must be received t reserve yur date(s) and time(s). The balance f yur space rental fee is due thirty (30) days prir t yur event. Miscellaneus csts, such as bar, catering, rentals, etc., are due ten (10) days prir t yur event. INSURANCE: Special Event Liability insurance is required f all renters and is due n later than ten (10) days prir t yur event. The insurance must, at renter s sle expense, prvide and maintain public liability and persnal prperty damage insurance, insuring attendees, emplyees, cntractrs and cntracted vendrs against all bdily injury, prperty damage, persnal injury and ther lss arising ut f renter s use and ccupancy f the premises, r any ther ccupant n the premises, including appurtenances t the premises and sidewalks. The insurance required shall have a single limit liability f nt less than $1 millin, and general aggregate liability f nt less than $2 millin. CAAHAC shall be named as an additinal insured f said plicy. If alchl is t be served, please make sure a license is btained by the renter, which must be purchased and a cpy given t the CAAHAC, n later than 10 days prir t the event as yu are ultimately liable fr the safety f yur guests. Established catering services must prvide a cpy f their certificate f insurance and catering license t CAAHAC at least ne mnth prir t the event. LIABILITY: Renter agrees t indemnify, defend, and hld CAAHAC, its landlrd, building wners, fficers, emplyees, and agents harmless f and frm any liabilities, csts, penalties, r expenses arising ut f and/r resulting frm the rental and use f the premises, including but nt limited t, the persnal guarantee f prvisin, service, and dispensing f payment by renter, its emplyees, and agents f alchlic beverages at the CAAHAC. In the event the CAAHAC, its landlrd, building wners, fficers, emplyees and/r agents are required t file any actin in curt in rder t enfrce any prvisins f this agreement, renter agrees t pay the CAAHAC, its fficers, landlrd, building wners, emplyees and/r agents, all reasnable attrney fees, curt fees, and csts f suit incurred by tenant including all cllectin expenses and interest due. CATERING STANDARDS, we highly recmmend caterers have experience wrking and are familiar with state s laws, regulatins, plicies, and prcedures. All caterers wrking at Cultural Center are required t have a valid certificate f insurance. CAAHAC space and is t be used fr final fd presentatin, plating and bussing nly. Sterns may be used with clse supervisin. Please nte that CAAHAC des nt prvide dishes, glassware, pts, pans, knives r utensils. 2

3 Page 3 Cntact/Renter Initials: Date: CAAHAC will be in a clean cnditin, and the space shuld be returned t a clean cnditin immediately fllwing yur event. A final walkthrugh with a member f ur event staff is mandatry at the clse f yur event. Caterers /r renters must remve all trash and recyclables frm the site. All trash, including srted recyclables and prperty, must be cllected, prperly bagged and remved by the caterer /r renter. Failure t remve r clean will result in fees, and event party will be charged. CAAHAC encurages thse renting space t cnsider green, sustainable, fair trade, eclgically sund cleaning and zer waste slutins. CAAHAC prudly cmmits t as clse t zer waste events as pssible. CATERING/GUEST COUNT: Final guest cunt must be submitted via at least ten (10) days prir t yur event, althugh additins may be made t yur guest list up t tw (2) days prir t yur event. We have 120 chairs, five 6 rectangle tables, 15 6 rund tables. CAAHAC wants t make every event here a special and welcme experience. Therefre, every effrt will be made t allw renter t prepare decratins reflecting their creative requirements. We ask that nly n nails and thumbtacks are used n ur walls r flrs. NO TAPE OF ANY KIND IS ALLOWED ON THE WALLS. CONDUCT: There is abslutely n drug use r smking f any kind tlerated n premises including litering r cngregating utside n the sidewalk at any time during the event. Disparaging remarks r any type f physical vilence will nt be tlerated and will be cause fr immediate expulsin. Renter and guests shall use the premises in a cnsiderate manner at all times. During underage events, such as schl dances, we prefer that underage individuals d nt have in and ut privileges. Cnduct deemed disrderly at the sle discretin f the CAAHAC staff shall be grunds fr immediate expulsin frm the premises and cnclusin f the rental perid. In such cases n refund f the event csts shall be made. LIVE MUSIC/DJs are allwed. Hwever, please be aware that the premises are lcated near residential units, and therefre neighbrhd nise regulatins d apply. In the event that renter s event creates a disturbance due t high nise vlume, the nsite manager has full authrity t ask the renter, DJ r live music presenter t turn the entertainment dwn and/r ff. If repeated disturbances are created, at CAAHAC discretin, renter may be expelled frm the premises r the ffending nise will be ended. In the event f disturbances t the pint f expulsin, n prtin f the event csts will be refunded t renter. Lud music must end by 10 pm (Mnday thru Thursday and Sunday) and by midnight n weekends (Friday and Saturday). Additinal time can easily be added prvided the vlume is mdest and it cannt be heard beynd the bundaries f the prperty r by immediate neighbrs. 3

LOAD-IN/LOAD-OUT AND STORAGE: All lad-ins and lad-uts must take place within the designated time frame given by the CAAHAC. CAAHAC is nt respnsible fr checking in r handling any items brught int the venue by rental cmpanies. All external items must be checked in and signed fr by the client r client s representative. All excess material (such as bubble wrap, bxes, hangers, plastic, etc.) created by deliveries must be remved and dispsed f by rental cmpany, client, r client s representative. Limited strage is available upn request. Strage fees may be applied t any items left vernight r beynd nrmal lad-in/lad-ut times. This als applies t items left pst event fr shipment ut via curier. Nte: it is nt the respnsibility f the CAAHAC t ensure that pick-ups are scheduled and executed. CLEANING, TRASH AND EQUIPMENT REMOVAL: The CAAHAC will be in a clean cnditin prir t yur event. Within tw (2) hurs fllwing the event, yu are required t return the space t the same clean cnditin in which it was fund. CAAHAC encurages thse renting space t cnsider green, sustainable, fair trade, eclgically sund cleaning and zer waste slutins. This is a drug free and nn-smking facility at all times, NO EXCEPTIONS. Renter shall nt sell alchl n premises at any time withut a license. Renter may nt serve alchl t minrs n the premises at any time. Renter agrees, fr everyne s safety, t ensure alchlic beverages are cnsumed in a respnsible manner n the premises. The CAAHAC reserves the right, in its exclusive discretin, t expel anyne wh in its judgment is intxicated r under the influence f alchl r drugs, r wh shall in any manner d r participate in any act jepardizing the rights, use permit, r insurability f the CAAHAC. ENTRY AND EXIT: Fr the safety f its staff, guests, r building cntents, renter agrees that the CAAHAC staff may enter and exit premises during the curse f the event. A representative f the CAAHAC will be n site during yur entire event and will be checking peridically with the respnsible parties t insure everything is running smthly. We will als be checking the bathrm, the verall premises, replenishing hand twels and tilet paper, and will be available fr questins r t respnd t needs r issues that may arise at any time. LOST AND FOUND: The CAAHAC takes n respnsibility fr persnal effects and pssessins left n premises during r after any event. We d, hwever, maintain a lst and fund and will hld recvered items up t 30 days. Every attempt will be made t return any recvered item t its rightful wner. 4
