1 Cntract fr Lease f Lenv ThinkPad X230 Lenv ThinkPad X230 (as per specificatins listed n reverse page) This lease agreement is made and entered int n the day f,, by and between f (address) and Drset Primary Schl f Rescrla Ave, Crydn with respect t the lease f a Lenv ThinkPad X230 and hneycmb carry-case fr three (grade 4) / fur (grade 3) years (please circle). The ntebk described herein and frthwith is fr the sle use f: (student name) I agree that I will lease the Lenv ThinkPad fr a perid f fur years frm March 1 st 2013 until December 18 th 2015 (grade 4) / 2016 (grade 3) (please circle). I agree that the ttal rental payable ver this fur year term will be $ The amunt payable is fr the lease f a Lenv x230 and a rigid hneycmb carry-case which is valued at $25. 2 I understand that Drset Primary Schl will cnduct general maintenance, update and keep the ntebk hardware and sftware in gd wrking rder ver the fur year perid. Drset Primary Schl will nt be respnsible fr repairing damage t the ntebk due t misuse r damage (accidental r therwise). I agree that I will incur any csts relating t the repair f the ntebk due t misuse, lss r damage (accidental r therwise). Optins: Insurance Warranty - Upgrades I understand that I have the ptin t insure the Lenv ThinkPad thrugh ibrker Financial Services at a cst f: 1 year insurance -$70 2 years $110 3 years - $150 4 years- $220 ibrker Financial Services Details... ibrker Financial Services which is underwritten by QBE Insurance (Australia) Limited, specialise in insurance fr educatinal ntebk prgrams. Sum Insured: Up t $650 (ex-gst) per ntebk Excess - $50 per ntebk per claim. The schl strngly encurages parents t insure their ntebk. This insurance can be rganised thrugh the schl and is GST exempt. I understand that I have the ptin t upgrade the warranty frm the standard 3 year back t base (ntebk returned fr repairs) warranty t either a 3 year nsite (repairs dne at the schl) warranty. Csts described belw. I understand that I have the ptin t upgrade my ntebk frm 2gb RAM t 4gb RAM fr an additinal $31.00 If yu wish t insure yur ntebk, add additinal warranty r upgrades yu must indicate this either n this frm (which needs t be returned by the 14 th f December 2012), r at the schl ffice by Friday 8 th f February N ptins can added after this date. 3 Usage Guidelines: Students must adhere t the Drset Primary Schl Acceptable Internet Usage Cntract at all times (this will be signed at the beginning f the 2012 year with each child s parent). This includes usage f the ntebk during ut f schl hurs. If students have permissin frm their parents t take the ntebks hme, they will be required t bring their ntebks t schl fully charged every day. The ntebk cmes with a hneycmb case, prvided by the schl, and students must use it fr the purpse f prtecting the ntebk. Games are nt t be accessed thrugh the schl netwrk r played n the ntebk at schl. Deliberate attempts t avid the schl s restrictins f access t sites deemed inapprpriate is frbidden and will be dealt with under the Internet Usage Plicy. The set up and cnfiguratin f the ntebk is nt t be changed. This des nt preclude students frm installing hme printer drivers and cnfiguring the ntebk fr access t hme netwrks and hme Internet Service Prviders. Hwever, it is the respnsibility f the student t ensure the ntebk remains ready and able t be cnnected t the schl netwrk. Students are nt t install ther prgrams. Wrk can be saved n the schl netwrk when at schl and a memry stick at hme. Students may nt disassemble any equipment. All repairs will be rganised by the schl. If damage t the ntebk is suspected, then the classrm teacher needs t be alerted and ur technicians will inspect the ntebk and determine whether it is a simple technician issue r that it will require repair with supplier Netwrk Neighburhd. If the ntebk needs t be sent t Netwrk Neighburhd fr repair the student will receive ne f the schls ntebks t wrk n. There may be cst fr sme repairs deemed t be wear and tear. At schl, ntebks will and must be secured in strage lckers in the classrm whenever they are nt in the student s direct pssessin r being used, such as at recess and lunchtimes. These lckers will be prvided by the schl. Ntebks shuld nt be left unattended in bags r in classrms. If yur child attends OSHC, then the ntebk can be stred in a secure lcker at OSHC. All games and music n the ntebk must be legitimate and nt pirated. Yu must respect cpyright. The schl cannt permit students t use illegal sftware. All sftware n the ntebk must be licensed. Unauthrized cpying f sftware r infrmatin belnging t thers is prhibited. Students are respnsible fr backing up data Students are respnsible fr the safety and integrity f the data n the ntebk. Students are encuraged t use the schls netwrk strage area t back up their wrk. The schl accepts n respnsibility fr lst data. Refrmatting f the hard drive will be undertaken when deemed necessary by the schl s technicians. Refrmatting causes all prgrams and files n the cmputer t be erased and the cmputer t be returned t its riginal state. Netwrk administratrs may review files and cmmunicatins t maintain system integrity and ensure that users are using the ntebk respnsibly. Users shuld nt expect that all files and infrmatin n the ntebk is private, including infrmatin cntained n any ther strage device

4 cnnected t the ntebk. Ntebks are the prperty f the schl. The schl reserves the right t access the ntebk at any time. Students may nt add r remve any identifying labels n the ntebk, as this will vid any warranty. Misuse f the ntebk may result in the equipment being required t be returned t Drset Primary Schl. Ntebks may nly be retained by the student ver the summer hlidays if the first 3 installments are paid. Althugh this is t be decided by the parent, the schl des nt encurage students t take their ntebk hme during hliday time. Transprt t and frm schl: Drset Primary Schl strngly recmmends that n child shuld transprt their ntebk t and frm schl withut parental supervisin. This is fr the safety f the child. When travelling t and frm schl, students are t carry their ntebk s that it is nt visible t any casual bserver. Ntebks must be carried inside the student s schl bag r an apprved carry bag r cntainer. Ntebks must be carried at all times and nt left unattended. Ntebks must be remved frm the schl bag as sn as the student arrives at schl t be placed in a secure lcker and remved frm the bag again when they get hme. At hme: Ntebks are t be kept away frm the eating area, fd and drink. The ntebk shuld be kept away frm ht and cld surfaces. Students must nt eat fd r drink when using the ntebk at hme. The pwer crd shuld be used whenever pssible t avid the battery being run dwn. Ntebks must be stred flat. Slvents are nt t be used t clean the ntebk. The ntebk is prvided fr the sle use f the student named in this agreement. Faults: Any malfunctins must be reprted immediately t the classrm teacher and the schl s cmputer technicians. Lss/Damage/Theft f Netbks: Lst ntebks are nt cvered by warranty r cvered by the schl s glbal insurance. Students are respnsible fr knwing where the ntebk is at ALL times. If students damage their ntebk intentinally it will nt be repaired as part f the warranty. If yu have taken the individual insurance ptin, this damage wuld als be unlikely t be cvered. The ntebk must always be carried in the puch r case. This minimizes the chance f damage.

5 If the ntebk is stlen r accidentally damaged while a student is taking it hme, has it at hme, r is returning with it t schl it will nt be cvered by the schl s glbal insurance. If yu have taken the individual insurance ptin, this wuld be cvered. Stlen r Damaged at Schl If the ntebk is stlen at schl, accidently damaged (eg. liquid spills, run ver, crushed) r destryed (eg. electrical surges, shrt circuits), students, teachers and parents will be respnsible fr fllwing the prcedure listed belw: An explanatin f the cause f the damage will be required by the student; this may als require a statement frm the parent r teacher. The schl, and in sme cases the insurance cmpany, will further investigate each claim t determine the cause f the prblem. Stlen If the ntebk is stlen frm the schl, at n fault f the student, the schl will rganise fr a replacement ntebk. Accidental, Negligent r Intentinal Damage at schl If a child has damaged a ntebk at schl (either theirs r a ntebk f anther child) and it is nt able t be repaired by ur technicians, Parents/Guardians (f the child wh caused the damage) will be liable fr the csts incurred fr repairs t the ntebk.
