The goods and services shown on this list are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangement you select will include a charge for our basic services. If legal or other requirements mean you must buy any item you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. This list does not include prices for certain items that you may ask us to buy for you (cash advances), such as, cemetery lots, open & close of the grave, tent set up and lowering device (when not provided by cemetery), flowers, music, beautician and honorarium for ministers. The prices for those items will be shown on your bill or the statement describing the funeral goods and services you selected. Methods of Payment Payment for goods and services is due when services are rendered, unless other arrangements have been made prior to need. For your convenience, we accept the following methods of payment: Cash Personal or Cashier s Check Secured Life Insurance Proceed Assignments Credit or Debit Cards (Should you require any third party financing, we will work with your banker or financial institution.)

2 Basic Services of Funeral Director & Staff: $ 1, Our fee for the services of funeral director and staff include, but not limited to, twenty-four (24) hour availability for staff to respond to initial request for service; arrangement conference with family or responsible party; clerical assistance in the completion and filing of necessary certificates and permits; and consultation of service plans with cemetery, crematory and others as required. Other services included are receiving and arrangement of flowers, completion and placement of obituary notices. Embalming: $ (Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement, which does not require you to pay for it, such as direct cremation or immediate burial.) SELECTION F : Receiving of Remains $ Our charges include necessary services of funeral director and staff, temporary shelter of remains, and funeral coach to cemetery. This charge does not include transportation or mileage fees for removal from airport or other establishment to our funeral home, nor any funeral rites or ceremonies. (This selection would be necessary if deceased was already in a casket and prior services were held by the forwarding funeral home.) SELECTION F : Receiving of Cremains $ Our charges include necessary services of funeral director and staff, temporary shelter of cremains and utility vehicle to local cemetery. This charge does not include any funeral rites or ceremonies. (This selection would be necessary if deceased was cremated by another crematory or funeral home.) The following schedule of fees shall apply to infants and children. The fee is based on the same service fee schedule as an adult with the exception that only the following percentage of the full adult fee will be applicable: Stillbirth 10% of Adult Fee Birth to 2 years. 20% of Adult Fee 3 years to 8 years. 40% of Adult Fee 9 years to 15 years... 80% of Adult Fee 16 years and over. 100% of Adult Fee * Discount packages available to those who purchase casket and outer burial container (if applicable) from Hart-Wyatt Funeral Home, Inc. * Discount packages available to those whose method of payment to Hart-Wyatt Funeral Home, Inc., is either cash, personal check, cashier s check or secured life insurance proceeds assignment. 4

7 OUTER BURIAL CONTAINER PRICE LIST & Description These prices are effective as of April 1, 2015, but are subject to change without notice. In most areas of the country, no state or local law requires you to buy a container to surround the casket in the grave. However, many cemeteries require you to have such a container so that the grave will not settle as much. Either a burial vault or a grave liner will satisfy these requirements.
