3 These prices are effective April 23, 2014, and are subject to change without notice. The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. (However, any funeral arrangements you select will include a charge for our basic services and overhead.) If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement describing the funeral goods and services you selected. This list does not include prices for certain items you may ask us to buy for you, such as cemetery or crematory services, flowers, and newspaper notices. The prices for those items will be shown on the statement describing the funeral goods and services you selected. Please note: Service charge for children 5 to 12 years will be adjusted by 50%; children 3 to 5 years by 75%. No service fee for infants from birth to 3 years. In all such cases, merchandise and cash advance items will be added as applicable.

4 Itemized Selections Basic Services of Funeral Director and Staff...$ Our basic service charge includes, but is not limited to, staff to respond to initial request for service; arrangement conference with family or responsible party; preparation and filing of necessary certificates and permits; placement of obituary notices, planning of funeral arrangements; and coordination of services with cemetery, crematory, vault companies, transportation and other as required. Also included in this charge are the overhead expenses relative to our facility such as insurance, maintenance and utility expenses, secretarial and administrative costs, and equipment and inventory expenses. The fee for our basic service and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremation and immediate burial and forwarding and receiving remains.) Embalming..$ Except in certain cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with a viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. (Does not include use of facilities and staff for ceremony public or private in other location or at graveside; merchandise or cash advance items.) Package J: Green Burial..$ This eco-friendly burial includes removal of remains within 40- mile radius; necessary authorizations and basic services of funeral director and staff; refrigeration of remains for up to three days; placement of remains in basic particleboard tray with cardboard lid and delivery to Sunnyside Cemetery for burial.
