We provide personal and compassionate service, giving the care to your family that we would want for our own family. Our professional, experienced funeral director will offer guidance so you may make an informed decision about the service options and related merchandise available. Our staff is ready to assist you in taking care of the many details that must be handled after death occurs. Because each family has its own traditions, we are committed to helping you plan services, celebrations and tributes which will reflect the life and interests of your loved one. Financial Arrangements Provisions for payment are due at the time of arrangements. To avoid any possibility of misunderstanding or embarrassment and for your convenience, we accept the following methods of payment. Cash, Check, Debit Card, Credit card (Visa, MasterCard, American Express & Discover) and Pre-arrangements. A five percent (5.0%) servicing fee will apply to credit card transactions of cash advance payments only. Insurance policies may be used as a source of payment. If the funeral account is to be paid from the benefits of an insurance policy, an Assignment of Proceeds of Insurance form must be completed and an Insurance Assignment Filing Fee charge will be incurred. Should the funeral account be paid by the estate of the deceased, we will make contact with the legal representative and an estate Filing Fee charge will be incurred. In all instances, cash advance items must be paid at the time of arrangements. Delinquent accounts will be assessed a finance charge of 2.5% per month, compounded monthly. Services may be cancelled if the cash advance fees are not paid in full. If you anticipate difficulty in meeting this obligation, please feel free to discuss it with us. The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

2 Professional Services 1. BASIC SERVICES OF FUNERAL DIRECTOR,STAFF OVERHEAD $ Our fee for the basic services of funeral director and staff includes, but is not limited to: Personnel available 24 hours a day to respond to initial request for service Arrangement interview with family or responsible party and recording vital statistics Securing and recording necessary authorizations and permits Sheltering of remains Coordination of service plans with cemetery, crematory, and/or other parties involved in the final disposition Overhead expenses relative to our facility such as professional licensing, staff services, bookkeeping, legal and accounting fees, insurance, building and utility expense, parking lot and ground maintenance, taxes, equipment, furnishings, inventory costs, administrative expenses, and governmental compliance. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving of remains.) 2. EMBALMING $ Use of the embalming room, professional services, equipment and materials involved Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as a direct cremation or immediate burial. Embalming may be required for placement into a mausoleum or shipping on common carrier. 3. Since many caskets that appear similar in appearance may differ greatly in quality and construction, Gasketed: These caskets are designed by the manufacturer to resist the entrance of air, water and other outside elements. These caskets may be constructed of varying gauges of steel, stainless steel, and copper of bronze. There is no scientific evidence that any casket with a sealing device will preserve human remains. Non-Gasketed: These caskets are not designed by the manufacturer to resist the entrance of air, water or other outside elements. These caskets may be constructed of metal, wood, or wood products covered with fabric. A complete price list will be provided at the funeral home. RECEIVING REMAINS FROM ANOTHER FUNERAL HOME $ Proportionate charge for basic services of funeral director and staff Transfer from common carrier(within 30 miles of our funeral home) Transportation to place of disposition(within 30 miles of our funeral home) Sheltering of embalmed remains for up to 3 days for placement at a cemetery This charge for receiving the remains does not include additional items that may be necessary such as mileage, casket, shipping or transfer of container, airline charges, viewing/visitation, rites or ceremonies etc.

9 If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The alternative containers we provide are made of cardboard. A complete price list is available at the funeral home. Our crematory does not permit the use of containers made of non-rigid materials. By discussing plans in advance, you can take all the time necessary to make the best decisions regarding the type of ceremony and disposition preferred. Pre-planning provides peace of mind for everyone involved. A carefully preplanned funeral service can be the most comforting to your family as it spares them from having to face decisions that can be both emotionally and financially difficult at the time of death. It can give them lasting peace of mind to know that they acted in accordance with your wishes. Another reason to pre-plan is to ensure that your family will receive all the benefits for which they are entitled. We can provide you with helpful information to guide your family through the details that will need to be addressed at the time of your passing. Although not required when making pre arrangements, most people choose to set aside funds that will cover future expenses. We offer a variety of pre-payment programs which allow you to control cost and avoid emotional overspending.
