The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Prior to drafting any contract for goods or services, the responsible party or the decedent s survivor who is handling the funeral arrangements is entitled to receive a copy of any preneed agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent. 2 SERVICES, FACILITIES AND EQUIPMENT 1. Basic Services of Funeral Director and Staff (Required)...$ 995. This is a basic fee that we charge every family for our professional expertise in assisting them with their preferences. It includes a portion of our overhead, which includes taxes, licenses, utilities and business expenses required to serve the public. Our charge includes, but is not limited to consultation with the family, administrative and clerical services, preparations of the death certificate for certification by the attending physician or coroner, local procurement of death certificate, preparation of required permits, filing the death certificate with appropriate agencies, and when necessary, notification to clergy, cemetery, crematory or common carrier. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) 2. Preparation of Remains Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as a direct cremation or immediate burial. A) Embalming... $ 525. 3 PROFESSIONAL FUNERAL SERVICE OPTIONS While the selection of an individualized Funeral Service may be accomplished through the use of the itemized General Price List, most families we have served indicate a preference for one of the funeral packages listed below. You may add to any of the packages with your choice of merchandise (such as a casket, alternative container, register book, or memorial folders), and cash advanced items (such as Health Department filing fees, clergy honorarium, or sales taxes). You may add to any of these plans with other items from the itemized price list. A complimentary Custom Video Montage and Website Death Notice and online grief support will be given on all Burial Packages. However, no credit can be made for substitutions or deletions from the funeral service packages. 4 MINIMUM SERVICES For those families that desire minimum services only, the following plans have been priced as packages, and include only those items listed. Merchandise (such as a casket or air shipping tray), and Cash Advances (such as legal filing fees, cemetery charges, or common carrier fees) are not included. No viewings or services are available with these packages. IMMEDIATE BURIAL... If you want to arrange a direct cremation you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are made fiberboard. Our direct cremation includes basic services of funeral director and staff, Refrigeration (first 96 hours), Transfer of remains to the mortuary, and Transfer of remains to the crematory.
