You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. PROFESSIONAL SERVICES Basic services of Funeral Director and staff... $750 Our fee for the basic services of funeral director and staff includes, but is not limited to, staff to respond to initial request for service; arrangement conference with family and responsible parties; arrangement of service/tribute including preparation and filing of necessary authorizations and permits, recording vital statistics, preparation and placement of obituary notice and coordination with those providing other portions of the funeral, e.g., cemetery, crematory, vault companies and others as required. Also included in this charge are overhead expenses relative to our facilities such as professional licensing, legal and accounting fees, insurance, building and utility expenses, parking lot and grounds, maintenance, taxes, equipment, furnishings, inventory costs, record keeping, secretarial and administrative expenses. The fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) ACREMATION PRICE LIST PAGE 1

2 PREPARATION OF DECEASED Embalming... $475 Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. Other preparation of the deceased: Cosmetology, dressing and placement of deceased in casket... $125 B. Sanitary procedures for deceased when no embalming has been performed... $325 C. Necessary care following organ, bone or tissue donation... $295 D. Refrigeration of unembalmed remains *... $195 (In accordance with Texas Law, unembalmed human remains sheltered by the funeral home shall be placed in refrigeration after 24 hours from time of death until final disposition. Texas law does not require embalming. However, it is the policy of ACremation.com that any human remains that has not been embalmed may not be viewed under any circumstance, unless such viewing is required for the purpose of positive identification. Further it is the funeral home s policy that no unembalmed body is dressed prior to placement in a casket or other burial container in clothing other than which death occurred. If embalming is declined solely due to financial hardship, please discuss this with our staff and we will present any appropriate options available.) *Normal charge for refrigeration while securing required documents for cremation or burial. Refrigeration beyond time needed to secure documents, when requested by family will be charged at the rate of $100 per day. FACILITIES, EQUIPMENT AND STAFF A. Visitation or private viewing at other location... $295 Our services include: visitation set-up, use of visitation area (up to 3 hours) between hours of 9:00 a.m. to 4:30p.m., display of floral arrangements, and supervision and attendance during the viewing, visitation or wake. Additional viewing/visitation will be charged at the rate of $125 per hour. A facilities charge of $150 will be added to the cost of the visitation for use of Rest Haven Funeral Home Royse City. B. Private Witnessing of Cremation or Second Identification Viewing... $295 Our services include: Witnessing at our crematory the start of the cremation process between hours of 8:00a.m. to 10 a.m., private viewing for up to 5 people at other facility if viewing is required after initial identification viewing. Initial identification viewing will be allowed at no charge if no family members or close loved ones were present when death occurred. C. Funeral service at other facility... $295 Our services include: coordination of the funeral arrangements, supervision of funeral and staff to coordinate the funeral ceremony, supervision of committal or other disposition service, equipment and additional staff necessary to conduct funeral at other facility. D. Memorial service at graveside... $295 Our services include: coordination of the memorial service, supervision of the memorial service, staff to attend the memorial service, set-up and use of facilities for memorial service. E. Memorial service at other facility... $295 Our services include: coordination of the memorial service, supervision of the memorial service, and additional staff necessary to conduct memorial service at other facility. Graveside service... $295 Our services include: coordination of the funeral arrangements at graveside, supervision of the graveside service, equipment and staff necessary to conduct the graveside service. (No services will be scheduled on Christmas Day, New Years Day, Thanksgiving, or Easter Sunday) G. Automotive equipment (35 mile radius) includes vehicle and personnel a. 5 Immediate Burial...$1305 to $4,500 This charge for immediate burial includes transfer of deceased to funeral home, local transportation to the cemetery, necessary authorizations and basic services of staff. This charge does not include the use of facilities and staff or any visitation or ceremony, public or private, prior to burial. The time of burial will be determined by the funeral home dependent upon current scheduling and commitments. a. Immediate burial with casket provided by purchaser... $1,305 b. Immediate burial with casket selected from our funeral home... $1,580 - $4,500 Direct Cremation...$995 to $4,190 This charge for a direct cremation includes transfer of deceased to funeral home, necessary authorizations, basic services of the staff and the crematory fee. This charge does not include the use of facilities and staff, any visitation or ceremony public or private prior to the cremation. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are made of heavy Michelman coated cardboard with no liner. a. Direct Cremation with container provided by purchaser... $995 b. Wilbert holiday charges are $500 and include Memorial Day, July 4 th Labor Day, Thanksgiving, and New Years. An additional $50.00 delivery fee for all liners and steel vaults will be added for burial that occurs in all outside cemeteries. Additional setting fee may apply in each cemetery. In most areas of the country, state or local law does not require that you buy a container to surround the casket in the grave. However, many cemeteries require that you have such a container so that the grave will not sink in. Either a grave liner or a burial vault will satisfy these requirements. Overnight lodging and meal fee for driver on overland transportation of remains (per night)... $150 The State Registrar s Office of Texas has initiated a new electronic death registration system, which is currently used by medical certifiers on a voluntary basis. When a death certificate is initiated in the electronic system by a Medical Examiner, and the initial cause of death is pending, the initial certified copy of the death certificate may not be accepted for use in filing a death claim by your insurance provider. Regrettably, the initial cost for the first copy (when ordering multiple copies) will be charged when the death certificate is filed and again when the amended death certificate certified copies are ordered. Even if you are not filing a death claim with an insurance provider, state law requires the funeral home to file the original death certificate within ten days of the date of death regardless of the cause of death, or its pending status. 7 DISCLAIMER OF WARRANTIES Our funeral home makes no representations or warranties regarding caskets, outer burial containers, or other funeral goods sold by the funeral home. The only warranties, expressed or implied, granted in connection with goods sold with this funeral service are the express written warranties, if any, extended by the manufacturers thereof.
