2 Value Choice Cremation & Funeral Care PA General Price List These prices are effective as of September 13, 2018 but are subject to change without notice. Our Pledge We are committed to serving you with the utmost regard for your family. This price list reflects our commitment to provide information and a wide variety of options to those that we are privileged to serve. The goods and services shown below are those we can provide to our customers. You may choose only the items you desire, however, any funeral arrangements you select will include a charge for our basic services. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Please note that there may be charges for items selected by you that are not described in this price list. Prior to drafting any contract for goods and services, the responsible party or the decedent s survivor who is handling the funeral arrangements is entitled to receive a copy of any preneed arrangement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent. Our Value There are many options available to you when planning a final tribute. We value the services that we are privileged to provide and will explain all of your options so that you will be pleased with the services you are arranging. For your convenience, we have arranged a single charge for the basic services of our funeral directors and staff which includes, but is not limited to the availability of our personnel 24 hours a day, 365 days a year to respond to your initial call, the arrangement interview, bookkeeping, accounting, and any general clerical administration fees. In addition, it also provides for the securing and filing of the death certificate, any disposition permits necessary, and the coordination of your service plans with all parties involved in the final disposition, such as the cemetery or crematory. The fee for our Basic Services of Funeral Director and Staff is currently $1,595. This fee for our basic services will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains). 3 Gathering & Remembrance Whether you are a member of a synagogue or church, you may want a simple and dignified graveside funeral, a large or intimate chapel service, or a creative service of your own design. We have a small observation room available on the premises or you may select from the many organizational sites we have relationships with all around the Washington D.C. metropolitan area. Our hope is to provide you with a service that fulfills your wishes. Embalming $1,795 Embalming is not required by law. Embalming may be necessary if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. 4 Other Services Forwarding of Remains to another Funeral Home $4,500 This charge includes transfer of remains to our funeral home (within 30 miles), basic services of funeral director and staff, embalming or refrigeration and transfer to common carrier within 30 miles of our funeral home. In addition, an air tray or shipping container will be provided as required by a common carrier. Receiving of Remains from another Funeral Home $4,500 This charge includes local ground transportation from common carrier within 30 miles of our funeral home, basic services of funeral director and staff, and transportation to crematory or cemetery. 5 Cremation Services There are numerous and flexible options for a family wishing to do cremation. We are able to accommodate anything from a simple immediate cremation to a full service funeral followed by cremation. Cremation arrangements will require, according to state law, an actual physical identification. All applicable next of kin will also be required to sign the Cremation Authorization Form. The next of kin is the closest relation to the deceased and this is the person who typically has the primary right of disposition. For example, if the deceased person is married, his or her spouse would be the closest next of kin and would be required to sign the authorization. However, if the deceased person is widowed or divorced, then all children that he or she had would be required to sign the authorization. So, in cases of multiple persons that are next of kin as equal relation, all of those persons will be required to authorize cremation. The funeral home will be able to help in these circumstances with coordinating signatures with multiple and out of town family members. Whether you prefer a simple cremation or a tribute including family and friends, our professional staff will explain all of our options so that you can make informed decisions with every opportunity to create the final tribute that addresses your needs. Direct Cremation $1,595 to $3,895 This charge includes the transfer of remains to our funeral home from within 25 miles of our facility, basic services of funeral director and staff, refrigeration and nonscheduled cremation fee. If you want to arrange a Direct Cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composite materials. The basic alternative container we include in our Direct Cremation package has a wood base and cardboard top and cost $95. Alternatively, we offer a Deluxe Identification Alternative Container for $395. (The Recommended container when doing a small gathering identification). A complete price list will be provided at the funeral home. The crematory does not permit the use of alternative containers made of non-rigid materials or metals. No item deletions from packages are allowed. Direct cremation with container provided by the purchaser (All such containers must meet any applicable state or crematory requirements.) 9 General Information Third Party/Cash Advance Items Third party items/cash advance items are items that are not associated with the funeral home, but maybe necessary for the family at a time of death. These items include but are not limited to: cemetery costs (even though property may be owned, most cemeteries will still have opening and closing fees at the time of service), Rabbi or clergy honorarium, certified copies of death certificates, death notice or obituary in newspapers, Shomer fees based on an hourly rate, out of town funeral home fees, airline fees, Chevra Kadisha donations, and state associated permit fees. Funeral Service Facilities We have multiple chapels throughout the Washington Metropolitan area to suit any needs (subject to availability). Death Certificates Usual timeframes for receiving certified copies of death certificates: 7 to 10 days in Maryland; 10 to 14 days in Virginia; days in Washington, DC. In some circumstances, unforeseen issues may occur with death certificates at the state level during the filing process. During this time the state may research something listed on the certificate by the certifying physician. This may cause an unexpected delay of receiving certified copies. In the event that this rare situation happens, the family will be notified immediately. Unfortunately, these are state level occurrences and are out of the control of the funeral home. Families will have a choice whether to they would like to pick their certificates up from the funeral home or have them shipped via UPS for $75. Page 8
