The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Method of payment Payment for goods and services are expected at the time of arrangement conference. For your convenience we accept cash, check, or credit card. This facility is licensed and regulated by the Oregon Mortuary and Cemetery Board. 2 GENERAL PRICE LIST These prices are effective as of August 1, 2015 but are subject to change without notice. Basic Services of the Funeral Director and Staff and Overhead This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) This fee includes the following: Personnel available 24 hours a day, 365 days a year to respond to initial call Arrangement Conference Coordinating service plans with cemetery, crematory, and/or other parties involved in the final disposition of the deceased Clerical assistance in the completion of various forms and permits associated with a funeral Also covers overhead, including facility maintenance, equipment and inventory costs, insurance and administration expenses, and governmental compliance Preparation of the Body Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with public viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. 3 Use of Facilities and Staff Use of Facilities and Staff Services for Visitation (Per day or portion thereof) Our services include set-up of visitation area, placement of encased remains, display of floral arrangements, supervision of and attendance during the visitation. Use of Facilities and Staff Services for Funeral Service in Our Chapel Our services include coordinating the funeral arrangements, supervision of funeral, and staff to assist with the funeral ceremony. Staff Services for Funeral Service at Other Facility Our services include coordinating the funeral arrangements, supervision of funeral, and staff to assist with the funeral ceremony. Use of Facilities and Staff Services for Memorial Service in Our Chapel (without remains present) Our services include coordinating the memorial service arrangements, supervision of the memorial service, and staff to assist with the service. Staff Services for Memorial Service at Other Facility (without remains present) Our services include coordinating the memorial service arrangements, supervision of the memorial service, and staff to assist with the service. Equipment and Staff Services for Graveside Service Our services include accompaniment of remains to cemetery, supervision of graveside service, and staff to assist with the service. Special Charges Service of Remembrance (Burial/Cremation Provided By Another Company) Includes Use of Facilities and Staff for service, and Standard Memorial Package Overtime: Evening Service or Visitation (up to 8:00 PM) Saturday Service or Visitation (Service between 10:00 and 3:00 PM Sunday Service or Visitation (Service between 10:00 and 3:00 PM) Transportation Transfer of deceased from place of death to funeral home (within a 25-mile radius) Additional distance will be charged at 6.00 per mile one way Funeral vehicle (e.g. Hearse) (within a 25-mile radius) Additional distance will be charged at 6.00 per mile one way. 4 Direct Cremation to 5, If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are alternative containers and other cremation containers, including containers made of wood, containers made of fiberboard, and containers made of other composite materials. See descriptions below. We also offer a selection of cremation oriented caskets. Our charges for direct cremation (without ceremony) include removal of remains, transportation to crematory, refrigeration, basic service vehicle and reduced basic services of staff. The crematory fee is included. Direct cremation with container provided by the purchaser Direct cremation with a Cardboard Alternative Container Direct cremation with casket from our facility (in addition to cost of casket) Immediate Burial 1, to 12, Our charges for immediate burial (without any rites or ceremonies at the funeral home, graveside or elsewhere) include removal of remains, transportation to cemetery, refrigeration or embalming, if relevant, service vehicle and reduced basic services of staff. This charge does not include visitation or ceremonies. This charge applies to shipment within the continental U.S. only. International Forwarding of Remains Fee This fee includes international calls, documents, and carriers. It is in addition to the Forwarding of Remains to Another Funeral Home charges Receiving Remains from Another Funeral Home This charge includes reduced basic services of staff, service vehicle from local airport or other shipping destination within a 30-mile radius, and service vehicle to cemetery or crematory.
