2 GENERAL PRICE LIST General Information/Disclosures: subject to change without notice. Prices are effective August 24, 2017 and are The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. The list does not include prices for certain items that you may ask us to buy for you such as cemetery or crematory services, flowers, and newspaper notices. The prices for these items will be shown on your bill or the statement describing the funeral goods and services you selected. We do not charge you for our services in buying these items. Certain funeral services may be provided off-premises by other funeral service providers. Professional Services of Funeral Director and Staff $2450 This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select (this fee is already included in our charges for direct cremation, immediate burials, forwarding and receiving remains). This charge includes, but is not limited to, arrangement conference; coordination of cemetery and/or crematory for final disposition of the deceased; procurement, preparation, and filing of necessary notices; general administrative and record keeping expense; and general overhead expense such as maintenance, equipment, inventory and insurance costs, and governmental compliance. Embalming Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it (such as direct cremation or immediate burial). 5 Page Four Direct Cremation $2300 to $4495 Simple cremation without any visitation or ceremonies; public or private. The direct cremation charge is a discounted/basic service that includes: Basic services of the Funeral Director and Staff; removal of remains from within our local service area, sanitary care, service vehicle, and transportation to the crematory. Direct cremation with container provided by the purchaser, $2300* Direct cremation with alternative container (particle box $275), $2575* Direct cremation with cremation casket, $2300* (plus cost of casket selected; a complete price list of caskets is provided at the funeral home; cremation caskets range from $425 to $2195) This charge does not include any cash advance payments including obituaries, certified copies of the death certificate, crematory fees, or any additional merchandise. It also does not include use of facilities and staff for any visitation or ceremony, public or private, prior to or after the cremation. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are unfinished boxes made of particleboard, pine and cardboard. Storage of the Cremated Remains: The cremated remains may be held at the funeral home at no charge for thirty (30) days until arrangements are made for proper release to the family or responsible party at our facility during our regular business hours. After thirty (30) days a $25 fee will be assessed per month, or any portion thereof, for shelter and storage of the cremated remains. After ninety (90) Days, cremated remains left at the funeral home will be considered as unclaimed and/or abandoned and the Jellison Funeral Home & Cremation Service will respectfully dispose of them according to NH RSA 325-A:27. Anatomical Donation to a Medical School $ 1,495.00* Medical Schools will typically compensate us for a portion of the removal and transfer of remains to the appropriate location. This fee includes the removal of remains and transfer only. All other services will be subject to pricing from the General Price List. Please note: Many Anatomical Donations are rejected at the time of death for various reasons by the medical schools. It is always wise to be prepared with alternate plans for final disposition in case rejection occurs.

7 Page Six Forwarding Remains to another Funeral Home This charge includes discounted/limited basic services of the Funeral Director and Staff, removal of remains within our service area, embalming, dressing and casketing of remains, service vehicle for necessary permits and certificates, transportation to Manchester airport. 1. With minimum airline approved shipping container: $ With casket selected from our funeral home or another source (plus cost of casket): $4240 This charge does not include the use of facilities and staff for any visitation or ceremonies, public or private, prior to or after forwarding of the remains. If the remains are to be transported to another funeral home outside of our service area or an airport other than Manchester, NH by automobile, additional mileage fees will be determined by location and distance. This price also does not include the airfare charges incurred for air shipment of the remains. An exact price will be provided to you prior to the service being provided. Receiving Remains from Another Funeral Home This charge includes basic services of funeral director and staff, and transportation of remains to the cemetery or crematory $2450 This charge does not include additional remains preparation, the use of facilities and staff for any visitation or ceremony, public or private, prior to or after the burial/cremation. This charge does not include any additional cash advance charges such as cemetery opening, vault charges, crematory charges etc. Additional charges for Custodial Care and Storage of remains may also apply. Receiving Cremated Remains With complete graveside committal service and family in attendance $695 (additional $250 for weekends and allowed holidays). Interment at our convenience and no one in attendance $ 275 This charge includes receiving of cremated remains, and local transportation to the cemetery. This charge does not include the use of facilities and staff for any visitation, rites, or ceremony, public or private, prior to or after the burial. This charge also does not include cemetery opening fees.

8 CASH ADVANCE PRICE LIST These prices are estimates as of October 26, 2017 and are subject to change without notice. The items shown below are Cash Advances. These are fees that we are able to pay on your behalf and then include in our statement. We do not charge you for this service. Cash advances are outside services which must be retained in order for many arrangements to be completed as requested. Since these prices are set by third parties, the prices on this list are subject to change without notice. The exact fees will be provided to you at the time of service. These items must be paid for in advance and include, but are not limited to: Grave Opening Charges.$ 100 to $ 2850 These rates vary according to which cemetery is selected, if it is a cremation urn or a casket burial, and if the cemetery has different rates for summer and winter burials, or weekend and holiday overtime charges. 9 Payment & Credit Policy We endeavor to provide you with appropriate services and ceremonies to meet your needs in celebrating the life of your loved one. It is important that the arrangements meet your wishes and budget. We are a family-owned business that treats every family we serve with the utmost compassion and respect. In an effort to provide the finest services possible, we are dependent upon the families we serve for making payment in a timely fashion. We realize that caring for the death of a loved one is often unexpected and unplanned. Please understand that the entire funeral amount is due and payable upon receipt of the final invoice, and before the burial or cremation takes place. For your convenience, we offer as many simple payment options as possible: Personal Check, Bank Check, Money Order, Cash, Visa, MasterCard, Debit Card The following are the only acceptable credit arrangements: Assignable Life Insurance: Life insurance may be used as a means of full or partial payment for funeral and other related expenses. (Must be verified in advance) We can assist the family with processing the various claims forms. Entitled Benefits: We will process on the family's behalf any death benefits from the Veterans Administration, Town or State Welfare Departments or Nursing Home Accumulated Funds Accounts as payment towards funeral expenses. The balance, however will be due upon receipt of the final invoice. We regret that we are unable to accept settlement of estates through attorneys or probate court as a means of financial arrangements. The family is responsible for the entire balance according to our payment policy. The family will be eligible for complete reimbursement from the estate. We also regret that we are unable to accept private or in-house payment plans as a means of financial arrangements. Loans may be available to you through your local bank or credit union. All unpaid balances after thirty days (30) from the date of death accrue late fee charges of 1.5% per month (18% per year). All late fees are incurred from the date of death. All unpaid balances after ninety days (90) from the date of death may be submitted to an agency for collection. If you have any questions, our staff will be available at any time to answer them.
