2 GENERAL PRICE LIST These prices are effective April 30, 2009 but are subject to change without notice. The goods and services shown below are those we can provide to our customers. You may choose only those items you desire. (However, any funeral arrangements you select will include a charge for our basic services and overhead.) If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. You may choose only the items you desire. If you are charged for items you did not specifically request, we will explain the reason for the charges on the written memorandum. Please note that there may be charges for items such as cemetery fees, flowers, and newspaper notices. SERVICES OF FUNERAL DIRECTOR AND STAFF $ 1,895 Our professional service charge includes but is not limited to consultation with family and clergy, arrangement and direction desired preparation and filing of necessary notices, authorizations and consents, and other services and attendance prior to, during and following the funeral including coordination with those providing other portions of the funeral (cemetery, crematory, florist, vault companies, musicians, etc.). Also included in this charge are overhead expenses relative to our facility such as insurance, maintenance and utility expenses, administrative cost, equipment and inventory expenses. This fee for our services will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremation, immediate burial, forwarding and receiving of remains.) EMBALMING $ 350 Embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with visitation. If you do not want embalming, you have the right to choose an arrangement which does not require you to pay for it, such as direct cremation or immediate burial. OTHER PREPARATION OF BODY $ 100 This service includes the usual cosmetology, dressing and casketing of the body or washing, disinfecting and casketing of the remains if there is no embalming at the family s request. 6 4 OUTER BURIAL CONTAINER PRICE LIST Effective April 30, 2009 In most areas of the country, state or local law does not require that you buy a container to surround the casket in the grave. However, many cemeteries require that you have such a container so that the grave will not sink in. Either a grave liner or a burial vault will satisfy these requirements. These prices are effective but are subject to change without notice. 1. 7 5 CREMATION DIRECT CREMATION $ 1,495 Our charge for a direct cremation without any ceremony includes removal of and temporary shelter of remains, necessary service of staff and authorization and transportation to crematory within 25 miles of Waco. (Additional miles above 25 are $ 1.75 per mile one-way) This price does include Crematory Charges. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The container we provide is a fiberboard box. 1. 9 SPECIAL SERVICES Forwarding of remains to another funeral home $ 1,375 This charge includes transfer of remains to funeral home, embalming, cosmetology, dressing and casketing of remains and limited services of funeral director and staff and necessary authorizations. It does not include casket, and air tray, any transportation other than initial transfer to funeral home nor any of the services of the staff or use of facilities for any ceremonies prior to forwarding the body. Receiving of remains from another funeral home $ 1,175 This charge includes limited services of funeral director and staff, shelter of remains, and transportation to cemetery within 40 miles radius of Grace Gardens. Disinterment $ 350 This is our professional charge for securing permits, arranging disinterment, state health department fee ($25), etc. Additional charges may include the opening and closing of the grave at disinterment site, container, the opening and closing of the grave at interment site, mileage, cemetery fees, etc. Immediate Burial $ 1,075 Our charge for an immediate burial without ceremony includes removal and temporary shelter of remains, transportation to cemetery (40 mile radius) and necessary services of staff and authorizations. Embalming if desired, and cemetery charges are not included in this price. Immediate burial with container provided by purchaser $ 1,075 Immediate burial with casket selected from our funeral home in addition to the price of the casket. $ 1,075
