3 GENERAL PRICE LIST THESE PRICES ARE EFFECTIVE AS OF April 1, 2018 BUT ARE THEREAFTER SUBJECT TO CHANGE WITHOUT NOTICE ***************************************************************************** The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. As a tribute to veterans who have served our country we will discount any service selected by the family $ (Please provide honorable discharge form DD214 to receive discount) PACKAGED SERVICES AND MEMORIAL PRODUCTS PACKAGE are only available to those who purchase or rent the casket, cremation casket or alternative cremation container from Lowe Funeral Home & Crematory, Inc. The price of each package represents a discount for the same goods, services and merchandise if selected on an itemized basis. **Receptions are available with any service package chosen** PACKAGED BURIAL SERVICE $ Services of funeral director and staff, embalming, visitation, service at funeral home, other facility, or graveside, transfer of remains, funeral coach, service and utility vehicle. PACKAGED CREMATION SERVICE $ Services of funeral director and staff, embalming, visitation, service at funeral home, other facility, or graveside, transfer of remains, cremation fee, service and utility vehicle. (This fee does not include a cremation container for viewing, services and cremation). PACKAGED SERVICE OF REMEMBRANCE $ Services of funeral director and staff, transfer of remains, use of facilities (or other place) for memorial service, (this does not include a visitation), service and utility vehicle, anatomical donation/cremation fee. (This does not include a casket for viewing or services.) PACKAGED SIMPLIFIED FUNERAL PLAN $ Services of funeral director and staff, embalming, visitation ( 1 hour prior to the service), graveside service, 20 gauge steel casket, transfer of remains, funeral coach, service and utility vehicle. This does NOT include an outer burial container if required by cemetery, sales tax, cemetery fees, obituaries, flowers, death certificates or any other cash advance item. (This must be paid in full prior to the service or any information is released to the public or newspaper.) Memorial Products Package....$ Includes register book, memorial dvd, home equipment, acknowledgment cards and memorial folders. GREEN BURIAL This establishment is one of a select number of funeral homes who meet the criteria for providing green funeral service by the Green Burial Council; a tax-exempt, nonprofit organization working to encourage environmentally sustainability in the field of funeral service. These offerings allow for a funeral to take place without the use of toxic or non-biodegradable materials/chemicals. Caskets and shrouds are made available to enclose and transport the dead. Body preparation suitable for a green funeral service accommodated by this funeral home consists of refrigeration, the use of topical sprays/compounds, or arterial embalming with essential oils. Home wakes, public visitations, church funerals and graveside services are options that may be arranged within the time frame that a green funeral allows.

4 BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF... $ This fee for our basic services will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, forwarding or receiving remains.) Our fee for the services of funeral director and staff includes, but is not limited to, staff to respond to initial request for services; arrangement conference with family or responsible party; arrangement of funeral; preparation and filing of necessary authorizations and permits; recording vital statistics; preparation and placement of obituary notices; staff assistance prior to, during and following the funeral, including coordination with those providing other portions of the funeral, e.g. cemetery, crematory and others. Also included in this charge are overhead expenses relative to our facility such as insurance, maintenance and utility expenses, secretarial and administrative costs, and equipment and inventory expenses. EMBALMING $ Except in certain special cases embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. OTHER PREPARATION OF THE BODY Washing, disinfecting and dressing of remains for viewing when there is no embalming......$ If casketed then an additional $ Special care of autopsied remains..$ Refrigeration of remains (per day after 2 days)..... $ When you select a service without embalming we offer a private family only viewing of the deceased $ USE OF FACILITIES Use of facilities and staff services for viewing (per day). This charge does not include merchandise or cash advance items. FORWARDING OF REMAINS TO ANOTHER FUNERAL HOME (within the U.S.)......$ Services of funeral director and staff, transfer of remains to funeral home, embalming and transportation to the airport.
