2 NOTES: This booklet contains our standard offering of funeral services and related products. If you do not find what you are looking for in this selection guide, please let us know. We will make every effort to find the exact product or service you may desire. PAYMENT POLICY At Arkansas Cremation, every effort will be made to accommodate you with a method of payment responsive to your personal situation. Our options for payment are: Payment by cash or check. Assignment of a valid life insurance policy. Use of any major credit card. Prearrangement or burial insurance. It is the policy of our company for these arrangements to be handled at the arrangement conference or no later than 24 hours prior to the service.

3 INFANT AND CHILDREN S PRICING Effective: August 1, 2012 INFANT Graveside Service Only (including a 21 casket) $ 495 If the family would like to add additional services, they will be applied at a rate of 50% of the published price in the GPL. CHILD / YOUTH SERVICE (requiring a 2.6 foot to 5.6 foot size casket) The child / youth service includes any of the services selected by the purchaser from among those found on the General Price List. The price for these services will be applied at the rate of 50% of published price. This does not include the casket or vault. See casket prices below. CHILDREN AND INFANT CASKETS The following caskets are normally available from our supplier(s) within 24 hour notice. You may choose only those items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any item for which you did not specifically ask, we will explain in writing on the statement we provide describing the funeral goods and services you selected. Prices are for our normal operational week, Monday through Saturday. Sales tax is applied at the legal rate set by state and local law. The tax is computed based upon the value of the tangible property of the funeral. Unless otherwise indicated the prices shown upon this price list do not include these taxes. FOR THOSE DESIRING ITEMIZED PRICING Minimum services of funeral director and staff...$825 Our fee for the services of funeral director and staff includes, but is not limited to, staff to respond to initial request for service; arrangement conference with family; arrangement of funeral; preparation and filing of necessary authorizations and permits; recording vital statistics; preparation and placement of obituary notices; staff assistance prior to, during and following the funeral, e.g. cemetery, crematory and others. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials and forwarding or receiving remains.) Embalming...$400 Except in certain special cases, embalming is not required by law. Embalming may be necessary however if you select certain funeral arrangements such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. Receiving of deceased from another funeral home...$895 This charge includes local transfer to the funeral home within 25 miles, basic services of funeral director & staff, use of facilities for care and custody (excluding visitation, ceremony, and hearse). Direct cremation - No service, no viewing...$995 Our charge for direct cremation (no ceremony) includes removal of deceased to funeral home, basic services of funeral director & staff and equipment, necessary authorizations and permits, transportation to crematory and cremation fee. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are made of cardboard. Direct cremation with purchaser provided container... $945 Direct cremation with alternative container...$995 Immediate burial available from (No ceremony, No viewing)...$1,795 Our charge for immediate burial (no ceremony, no visitation) includes removal of deceased to funeral home, basic services of funeral director & staff, necessary authorization and permits, and hearse to local cemetery. Immediate burial with purchaser provided casket... $1,795 Immediate burial with Copper Hammertone casket... $2,495 OTHER CHARGES Rush cremation (included in Memorial & Funeral Cremation Package)...$150 All cremations will be performed within 5-10 days of receiving the deceased contingent upon obtaining all necessary authorizations. If the ashes are needed within a week, the rush charge is applied to the basic cremation package. 5 CREMATION OPTIONS Effective: January 1, 2015 Cremation Packages For All Cremation Packages that include a Discount, The discount does not apply if the package is changed. Celebration Funeral Cremation Package... $2,495 The funeral cremation includes the entire basic cremation package plus embalming, dressing and cosmetology, use of ceremonial rental casket, stationery package, funeral coach transportation, a funeral ceremony in a church or other facility within 50 miles of Little Rock, and urn. Simple Memorial/Graveside Cremation Package...$1,795 The Memorial/Graveside cremation Package includes the entire basic cremation package plus a memorial ceremony in a church or other facility; or grave-side service with burial of ashes within 50 miles of Little Rock, stationary package, and urn. Private Witness/Viewing Cremation Package...$1,795 Includes the entire Basic Cremation Package, but instead of a basic cremation container, we provide an upgraded cremation container suitable for viewing and our private family viewing fee. Basic Cremation Package (No Services or Viewing)....$995 Direct Cremation with container provided by purchaser... $945 The basic cremation includes removal of remains, basic services of staff, necessary authorizations, obtaining of death certificates, transportation to crematory, the cremation, and a minimum cremation container. This charge does not include any rites or ceremonies, obituary preparation, certified copies of the death certificates, or an urn. BURIAL OPTIONS Traditional Service (with visitation)...$2,995 Our Charge for a traditional burial (with visitation) includes: basic services of funeral director and staff; a proportionate share of overhead costs: removal of remains; funeral coach transportation, a funeral ceremony in a church or other facility within 50 miles of Little Rock, and stationery package.
