The goods and services shown below are those we can provide to families. You may choose only the items you desire, however, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements state you must buy items you did not specifically ask for, we will explain the reason in writing on the statement that is provided describing the funeral services selected. SERVICES PROVIDED BASIC SERVICES OF FUNERAL DIRECTOR & STAFF $ Arrangements and planning of the funeral through consultation with family and clergy, shelter of remains, authorization and permits, coordination with the cemetery, or other third parties. The fee includes a proportionate share of our basic overhead cost. EMBALMING $ Law does not require embalming. Embalming may be necessary, however, if you select a funeral with viewing. If you do not want embalming you have the right to choose an arrangement that does not require you to pay for it, (i.e. Immediate burial or direct cremation.) OTHER PREPARATION OF BODY $ (i.e. dressing and casketing) TRANSFER OF REMAINS TO FUNERAL HOME $ Within a 25 mile radius of Baldwin. Beyond 25 miles we charge $2.50 per mile. USE OF STAFF & EQUIPMENT FOR CHURCH SERVICE $ HEARSE $400.00

RECEIVING CASKETED REMAINS FROM ANOTHER FUNERAL HOME $ Charges include: Basic services of funeral director and staff, a proportionate share of overhead cost, care of remains, and arranging funeral services. IMMEDIATE BURIAL $ Charges include: funeral director and staff, a Proportionate share of overhead costs, removal of remains and local transportation of remains and basic 20 gauge metal casket.** If the purchaser desires another casket, the casket price will be added to the cost of the Immediate burial. ** If the coroner holds the death certificate after thirty (30) days pending results of autopsy finding, an additional charge of $20.00 charged by the state for late filing. DIRECT CREMATION $ Charges include: basic services of funeral director and staff, a proportionate share of overhead costs, removal of remains from place of death inside the city limits. An additional $50 for cremation release from coroner, cardboard container provided
