Funeral homes must price funeral services similarly to give the consumer their General Price List as requested. From our price list for 2018, you may choose from our packages and services and a detailed list of those charges will be provided to you. 2 THE TRIBUTE FUNERAL HOME General Price List These prices are effective January 1, 2018 and are subject to change without notice. THE GOODS AND SERVICES SHOWN BELOW ARE THOSE WE CAN PROVIDE TO OUR CUSTOMERS. YOU MAY CHOOSE ONLY THE ITEMS YOU DESIRE. HOWEVER, ANY FUNERAL ARRANGEMENTS YOU SELECT WILL INCLUDE A CHARGE FOR OUR BASIC SERVICES AND OVERHEAD. IF LEGAL OR OTHER REQUIREMENTS MEAN YOU MUST BUY ANY ITEMS YOU DID NOT SPECIFICALLY ASK FOR, WE WILL EXPLAIN THE REASON IN WRITING ON THE STATEMENT WE PROVIDE DESCRIBING THE FUNERAL GOODS AND SERVICES YOU SELECTED. PROFESSIONAL SERVICES Basic services of the Funeral Director and staff....$1, Our services include but are not limited to our personnel being available to assist our families as necessary to respond to your initial request for services and respond to your needs: Conducting the arrangement conference; Act as a consultant in planning a funeral or memorial service; Consult with family and clergy as necessary; Preparing and filing the necessary notices, authorizations, certificates and permits; Coordinating services of a cemetery, crematory or third party. Also included in this charge are overhead expenses, administrative costs and equipment, inventory expenses and 2 certified copies of the death certificate. The fee for basic services and overhead will be added to the total cost of the funeral arrangements you selected. (This fee is already included in our charges for direct cremations) Embalming..$1, Except in special certain circumstances embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as funeral with a viewing. If you do not want embalming, you have the right to pay for an arrangement that does not require you to pay for it. Dressing and Casketing..$ Refrigeration... $ Up to 5 days/ per day after. If the distance requires our funeral director to stay overnight (Overnight occurs when the travel time is equivalent to a 2 hour trip or longer and begins after 4pm), an additional $ will be charged. ALTERNATIVE SELECTIONS Direct Cremations...$1, to $1, Includes the cost for removal of the remains, transportation of the remains, obtaining necessary authorizations and a proportional amount of our basic services of staff which includes a share of the overhead costs. When choosing a direct cremation, you may choose to use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The minimum alternative container we provide is made of fiberboard. Direct Cremation with Crematory Fee and Alternative Container (Fiberboard)....$1, Direct Cremation with Crematory Fee and without the Alternative Container (Alternative Container provided by the purchaser)......$1, Direct Cremation without Crematory Fee and Alternative Container.....$1, Immediate Burials.. $2, to $4, Includes initial removal of the deceased, refrigeration of the deceased, transportation to a cemetery and a proportionate amount of our basic services of funeral director and staff which includes a share of overhead cost. 2

Immediate Burial With Container Provided By The Purchaser $2, Immediate Burial With Minimum Casket. $4, Forwarding Of Remains To Another Funeral Home....$2, Includes the initial removal of the deceased, embalming and shipping to a local shipping point, obtaining necessary authorizations and permitting and a proportional amount of our basic services of funeral director and staff which includes a share of the overhead cost. THIS CHARGE DOES NOT INCLUDE SHIPPING COSTS BY COMMON CARRIER, CASKET, SHIPPING CONTAINER, DRESSING, CASKETING, COSMETICS OR ANY TYPE OF VIEWING, CEREMONY OR VISITATION PRIOR TO SHIPMENT. Receiving Of Remains From Another Funeral Home..$2, Includes the initial removal from a local airport, care of remains to a local cemetery and a proportionate amount of our basic services of funeral director and staff which includes a share of overhead cost. Additionally, the purchase of a memorial service from The Tribute Funeral Home does not cover the cost of outside funeral directors, clergy, catering, flowers and decorations or any other services requested outside of the scope of the general price list. The guarantees, expressed in connection with the memorial services sold are the guarantees, extended by The Tribute Funeral Home to provide contracted staff to be present for a minimum of two hours to aid in the facilitation of the memorial service purchased by the client to help ensure the service is memorable and reverent. 4

In most areas of the country, state or local law does not require that you buy a container to surround the casket in the grave. However, many cemeteries require that you have such a container so that the grave will not sink in. Either a grave liner or burial vault will satisfy these requirements.
