The goods and services shown below are those we can provide to our customers. You may choose only those items you desire. However, any funeral arrangements you select will include a charge for our basic services. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. SERVICES OF FUNERAL HOME AND STAFF: ITEMIZED FULL SERVICES TRADITIONAL AVAILABLE SERVICE PROFESSIONAL SERVICES: A. BASIC SERVICES OF THE FUNERAL DIRECTOR AND STAFF: Personnel available 24 hours a day, 365 days a year to respond to initial call, arrangement conference, coordinating service plans with cemetery, crematory, and/or other parties involved in the final disposition of the deceased. Clerical assistance in the completion of various forms associated with funeral arrangements. Securing and recording the death certificate and disposition permit. In addition, this fee includes a proportionate share of our basic overhead cost. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charge for direct cremations, immediate burials, and forwarding or receiving remains). (Non-declinable Charge) **$ GPL-1

If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. 4 NOTE: A Police Escort is a safety factor that will be utilized for Funeral Processions traveling within the city limits. This will be considered a Cash Advance Expense item added to the funeral purchase agreement. The cost will be determined according to local Police Department pricing policies. E. This charge does not include visitation, rites or ceremonies, and shipping charges. RECEIVING REMAINS FROM ANOTHER FUNERAL HOME: $ Our charge includes: Basic services of funeral director and staff; a proportionate share of overhead costs; necessary authorizations; local transportation of remains to cemetery or crematory (within 35 mile radius). This charge does not include visitation, rites or ceremonies, and shipping charges. GPL-4

5 DIRECT CREMATIONS: $ to $12, Our charge for a direct cremation includes: Basic services of funeral director and staff; a proportionate share of overhead costs; removal of remains; transportation to crematory; necessary authorizations, and applicable crematory charges. If you want to arrange a direct cremation, you may use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). This charge does not include visitation, rites or ceremonies. The containers we provide are: Minimal Alternative Container / Cremation Casket of Your Choice 1. Direct cremation with container provided by the $ Purchaser (all such merchandise must meet any applicable state or crematory requirements). 2. Direct cremation with: Minimum Alternative Container $ required for cremation (corrugated fiberboard container). 3. Direct cremation with: Shaker Pine Cremation Casket $ Other cremation caskets are available, and may be chosen with prices listed on the Casket Price List. IMMEDIATE BURIAL: $ to $11, Our charges for immediate burial, which does not include visitation, rites or ceremonies, includes: Basic services of funeral director and staff; a proportionate share of overhead costs; removal of remains; and local transportation to cemetery. 1. FINANCIAL ARRANGEMENTS: INSURANCE ASSIGNMENTS - A third party assignment company, Express Funeral Funding, is utilized to secure funds for the total amount of the Statement of Goods and Services Contract. The total fees charged is a basic $ minimum charge or 3.50% of the Assignment Total when over the minimum, which will be deducted from the life insurance policy value. This takes a major financial burden off of the family while taking care of the account for services rendered. GPL 7
