Though death is the cause of our meeting, let us celebrate life. Our hope is to help you fill the emptiness of loss by assisting you in giving expression to the richness and particular beauty of a life that was like none other. The 19th Century Fernwood Cemetery in Mill Valley, California has being recreated as a place of ecological restoration, community and remembrance. Fernwood is not only a crematory and a funeral home, but is also a memorial landscape where natural burial and cremation rituals are interwoven with land restoration and preservation. A memorial service, one of the most sacred events any of us will prepare, takes much thought and consideration. Whether you want to do something indoors or outdoors; whether you want it to be religious or non-denominational, we can help you create it. Our Remembrance Room is suitable for many different services from religious ceremonies to Circles of Friends to catered meals. Each person and family is invited to bring creativity and personal vision to this place. Fernwood is a unique site where families and histories connect through cycles of nature, memory and conservation. Fernwood provides a place for spiritual connection to our ancestors and to the earth. Please Note: This General Price List is for those considering using our Funeral Home services. Our funeral packages are available only when the casket has been purchased through our funeral home. If you purchase the casket elsewhere, please select from the itemized list of services and merchandise to complete the funeral service of your choice. This policy does not apply to urns and cremation packages. Caskets, urns, outer burial containers and cemetery options come in many varieties and are not included in our packages, but are offered on separate price lists provided at Fernwood. Our staff will gladly provide you with copies of these. Prior to drafting any contract for goods or services, the responsible party or the decedent s survivor who is handling the funeral arrangements is entitled to receive a copy of any pre-need agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent. 2 Fernwood Remembrance Room MEMORIAL SERVICE PACKAGES From our experience in helping families, we have created packages that incorporate the most selected service options. While we encourage you to customize your arrangements from the itemized list of options, we offer these packages to you as a means to simplify the funeral arrangement process and lower the costs. These service package prices represent a discounted price from the value price of the same goods and services if selected from our itemized list. Merchandise and services, whether included in a package or not, may be purchased separately. Because the options include various types of services, no credits for deletions or substitutions are possible. Prices DO NOT include casket or cemetery charges. 4 CREMATION PACKAGES All cremations (unless otherwise specified) will take place on-site at the Fernwood Crematory. Cremation Options available from $3,500 to $1,800. 5 The Horizon Crematory Fee Observed Cremation (Up to 30 minutes and 20 guests) Total Package Price $2,400 Direct Cremation Our charge for Direct Cremation (without ceremony) includes: Transfer of deceased from place of death to Funeral Home Crematory fee Direct Cremation with container provided by the purchaser $1,800 (Container must meet applicable state and crematory requirements) Direct Cremation with fiberboard alternative container $1,900 Note: If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide range from fiberboard (heavy cardboard) to hardwood (Poplar). A complete list is provided at the Funeral Home. Crematory Fee $425 (In addition to Funeral Services chosen by package or itemized. This fee is already included in our charge for direct cremation and our cremation packages.) Rush Cremation $300 (In addition to the Cremation fee cremated remains ready within 48 hours of obtaining permit.) Observed Cremation $385 (Observed Cremation takes place in our Crematory Remembrance Suite. Up to 30 minutes and 20 guests.) Additional fee if deceased weighs more than 250 lbs $350

6 GENERAL PRICE LIST This itemized price list offers an overview of the general goods and services offered by Fernwood Funeral Home. You may choose the items you desire from the list of goods and services provided. Please note that a charge for our Basic Services will be added to any funeral arrangements you select. This fee for Basic Services is included in our packages. Basic Services of Funeral Director and Staff This fee for Basic Services and overhead will be added to the total cost of any funeral arrangement you select. (This fee is already included in the charge of a direct cremation, immediate burials and our various packages). This is a non-declinable fee. This basic fee includes the services of the funeral home staff s professional expertise in assisting with all the requirements of a funeral arrangement. The funeral staff is made available to respond to the initial call for service. The funeral home staff is made available for the arrangement conference, in order to document the requested funeral rites and to then coordinate the details with clergy, cemetery, crematory, florists, escorts, musicians, Health Department, common carrier, any relevant publications for the placement of obituary notices, as determined by a family and/or other parties responsible for special services or disposition. This fee includes a proportionate share of the established overhead costs, city, county, state and federal indirect taxes, beyond our control. (This fee does not include sales tax on merchandise or business profit taxes.) Basic Services Fee $1,300 Embalming Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. All procedures are in strict compliance with OSHA guidelines. Embalming Fee $825 Refrigeration If embalming is declined and the final disposition of the deceased is not completed within 24 hours of the time of death, refrigeration is required by California State regulation, Title 16 Sec. 1223(b). Note: Our initial refrigeration fee includes 5 consecutive days. Beginning on the 6th day, the Funeral Home reserves the right to charge $200 for each additional day of refrigeration. Time restrictions may fluctuate due to daylight hours and disposition arrangements. There is an additional charge of $500 for use of the facilities, services and staff after 5:00 p.m., or on Saturday, Sunday or holidays. Visitations Use of Facilities and Staff for visitations in our Room of Remembrance or other onsite location. Our services include 2 staff members, set-up of the visitation area, placement of encased remains, display of floral arrangements, and supervision during the visitation. Post-autopsy care may be required at the sole discretion of the Funeral Director. $550 per hour Overnight Visitations Use of Facilities at Fernwood or off-site location Includes 2 staff members for the set- up of the visitation area, placement of encased remains and display of floral arrangements Limited supervision Conducted from 10:00 p.m. 8:00 a.m. Overnight visitation at an off-site location requires proof of liability insurance. Our services include accompaniment of decedent to interment site, supervision of graveside service, display of floral arrangements and staff to assist with service. 9 Private Viewing The fee for minimal preparation (required) and dressing (if desired) is included in this charge. Fernwood Funeral Home policy does not permit Private Viewings to exceed 30 minutes. The funeral home permits a maximum of 12 participants. Location will be chosen at the discretion of the funeral home. A Non-declinable post-autopsy care fee may be required at the sole discretion of the Funeral Director. Private Viewing up to 30 minutes $550 (Private Viewing must be scheduled between 9:00 a.m. and 3:00 p.m.) Identification Viewing Identification Viewing must be scheduled between 9:00 a.m. and 3:00 p.m. The fee for minimal preparation (required) is included in this charge. This does not include dressing, casketing and/or cosmetizing. Fernwood Funeral Home policy does not permit Identification Viewings to exceed 15 minutes. The funeral home permits a maximum of 6 participants. Location will be chosen at the discretion of the funeral home. A Non-declinable post-autopsy care fee may be required at the sole discretion of the Funeral Director. Identification Viewing up to 15 minutes$250 Immediate Burial Options The charge of any immediate burial option includes: Transfer of deceased from place of death to Funeral Home and utility vehicle and/or service vehicle for transportation to local cemetery as needed Price does not include casket or cemetery charges.
