2 RIGHT OF SELECTION The goods and services shown below are those we provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. DISCLAIMER OF WARRANTIES Regency Mortuary makes no representation or warranties regarding caskets, urns, or other merchandise. The only warranties, expressed or implied, granted in connection with caskets and/or containers sold are the express written warranties, if any, extended by the manufacturers thereof. Regency Mortuary hereby expressly disclaims all warranties expressed or implied, relating to the caskets, urns, or other merchandise, including, but not limited to, the implied warranties of merchantability and fitness for a particular purpose. NOTICE The prices on this General Price List are effective for the dates shown and are subject to change after that date. The prices shown on this list are the prices of our mortuary and do not include any costs associated with another mortuary. Caskets or containers provided by the purchaser must meet requirements of the cemetery or crematory. We reserve the right to refuse to handle any casket or container which does not meet our requirements. Prepackaged offers are designed to offer the lowest cost funerals available at this mortuary. No substitutions may be made at these prices. Regency Mortuary reserves the right to choose in what manner we handle remains that die from contagious diseases. This is because some diseases could possible infect our personnel even after death has occurred. we will not refuse to handle any deceased remains, but if the cause of death is contagious we reserve the right to use safeguards to protect our personnel, as well as the general public. PAYMENT POLICY Payment must be made in full 48 hours prior to the funeral, memorial, or cremation service taking place. Payment can be made in the form of cash, check, or credit card (Visa, Mastercard, American Express, Discover). Checks returned by banking institutions for insufficient funds or other reasons are subject to a $25 return check fee. Payment can also be made by verified and assignable insurance or other third party payor for a fee of 8% of total charges (minimum $75 charge). 2

3 MINIMUM SERVICES OF THE FUNERAL DIRECTOR AND STAFF...$1, This fee for our basic service and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving the remains.) The fee includes the following: Personnel available 24 hours a day, 365 days a year to respond to initial call Arrangement conference Coordinating service plans with cemetery, crematory, and/or other parties involved in the final disposition of the deceased Clerical assistance in the completion of various forms and permits associated with a funeral Also covers overhead, including facility maintenance, equipment and inventory costs, insurance and administration expenses, and governmental compliance PREPARATION OF THE BODY Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. 5 FORWARDING REMAINS TO ANTOHER FUNERAL HOME... $ This charge includes removal of remains, basic service of staff, necessary authorization, embalming and transportation to Phoenix Sky Harbor Airport (excluding shipping charges and shipping containers). This charge does not include visitation or ceremonies and applies to shipping within the continental U.S. only. RECEIVING REMAINS FROM ANOTHER FUNERAL HOME... $ This charge includes basic service of staff, transportation from local airport or other shipping destination, and transportation of remains to cemetery or crematory. This does not include visitation or ceremonies IMMEDIATE BURIAL... from $ to $4, Our charges for immediate burial (without any rites or ceremonies at the funeral home, graveside or elsewhere) include removal of remains, transportation to cemetery, service vehicle and basic services of staff.
