2 While you are free to select any of the individual services with a separate price for each item, to save money, most families select a funeral package below. These charges do not include cash advances. Cremation is performed within 5 working days after obtaining the signed Death Certificate from the attending physician or coroner, disposition permit, the completion of all required forms and payment in full. 3 The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services of funeral director, staff and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. ITEMIZED PRICE LIST 1. BASIC SERVICES OF FUNERAL DIRECTOR, STAFF AND OVERHEAD $ This is a fee which we charge every family for our basic services which include: consultation with the family or responsible party, clerical services, coordinating service plans with cemetery, crematory and/or other involved parties, obtaining required authorizations and permits(locally), and a fee necessary to cover a proportionate share of our overhead costs such as licenses, insurance, maintenance of facilities and equipment, utilities, and a host of other costs related to being in business to serve the public. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. This fee is already included in our charges for direct cremations. 2. *TRANSFER OF DECEDENT TO OUR FUNERAL HOME (LOCALLY) $ From San Bernardino County Coroner facility $ Additional personnel as necessary or required $ ( Additional charge for greater distances ) 3. PAYMENT OPTIONS Payment is due in full at the time arrangements unless other arrangements have been made. We accept the following forms of payments: Visa, Mastercard, American Express, Discover credit cards, cash, personal checks, cashier checks, or money order. There will be a $50 charge for all returned checks If check is not cleared within 7 business days the account will be placed on Check Systems for five (5) years and placed for collections with a third party company "Direct Collect" *

4 DIRECT CREMATION RANGE FROM $ TO $ If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The alternative container we provide is cardboard. Our charge for a direct cremation fee (without ceremony) includes: basic services of funeral director and staff; a proportionate share of overhead costs; local transfer of decedent to our funeral home, refrigeration of remains for the first 72 hours, local procurement of the death certificate, the preparation and the filing of permit of disposition and cremation performed at our convenience. This fee does not include the alternative container, California sales tax and regulatory fee, permit for disposition, a cremation urn or any certified copies of death certificate This cremation will be performed within 5 working days from date of signed death certificate and payment. 1. This option does not allow for a public viewing or any post autopsy care. MERCHANDISE Our facility does not offer for sale the following: Infant and child caskets; rental caskets; casket outer burial container; hearse or limousine. 1.
