2 ITEMIZED PRICES FOR OUR SERVICES The goods and services that follow are those we can provide to our families. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. PROFESSIONAL Services BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF $1,195 Our non-declinable charge for requesting the assistance of our Licensed Funeral Director and staff. This fee reflects the overhead of operating and maintaining our facility and having staff available 24 hours a day, 7 days a week. (Includes response to initial request for services desired and conference with responsible party to determine services desired and coordinating service plans with cemetery, crematory, and/or other parties involved for the final disposition of the deceased. This fee for our services will be added to the total cost of the funeral arrangements you selected. Other Services of Licensed Funeral Director $175/hour EMBALMING $450 Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. Our reception room can seat 40 guests comfortably, with room to spare. We also have a full kitchen available for those who need a stove or refrigerator. Charges include attendant. 2 hours use of reception facility, tables, chairs, and waste receptacles. Does not include catering charges. Basic cleanup is expected, otherwise a cleaning fee will be assessed. Step 2) Choose your Casket In this guide you will find examples of the caskets we offer, you can view our full line of caskets available online or come by our office to view our catalog. All of our caskets offered are of a high quality and finish. What makes each one different is the type and thickness of metal or type of wood as well as how much ornate trim and detail to interiors each one has and if they have a gasket or not. Step 3) Choose your Memorial Stationery We offer two packages, either complete or basic. You may also select just items you may desire ala carte. Step 4) Choose your Outer Burial Container Outer Burial Containers (vaults and grave boxes) are not required by law, but some cemeteries have a policy to require them to prevent the graves from collapsing over time. This is a personal preference and should be considered when selecting your casket. 11 A burial vault is a lined and sealed outer receptacle that houses the casket. It protects the casket from the weight of the earth and heavy maintenance equipment that will pass over the grave. It also helps resist water and preserves the beauty of the cemetery or memorial park by preventing the ground from settling. If it s not lined and sealed... it s not a burial vault. An outer burial container, or grave-box, is an unlined concrete box with drainage holes to allow ground water to escape. Burial vaults are constructed of high-strength concrete and reinforced with a metal or plastic liner. They provide the most lasting protection. By definition, if an outer burial container is not lined and sealed, it s not a burial vault. 12 Cremation Choices Choosing cremation offers several choices for service and memorialization. The following considerations will help you determine the type of event that would be most meaningful to you, your family, and friends. Do you prefer to offer friends and extended family the opportunity to say goodbye and offer their love and support with a public viewing or memorial service? Do you prefer to conduct ceremonial services at a church or place of worship? If family and friends are coming from a great distance, do you prefer to arrange for a memorial service following the cremation? Whatever your preference, our staff will work one-on-one with you to create an appropriate tribute. To help simplify the planning and arrangement process, the offerings described on the following pages represent the most popular choices of our client families who elect cremation. 15 BASIC CREMATION Family Viewing Family Farewell Cremation Package $2,395 When a simple cremation is requested, but the family would like to have an informal family gathering in our chapel with the body present prior to cremation. This package is not for public gatherings, and visitation must be in our chapel. Includes: Limited Services of Funeral Director and Staff; arrangement conference and intake of information; Assistance in writing obituary; Local Transfer from place of death; cardboard cremation container; plastic temporary urn; 5 days refrigeration; and Cremation Fee Time Frame for Cremation In order for the cremation to take place, the account must be paid in full; the death certificate must be signed by the doctor, coroner, or hospice RN, as well as have completed the Cremation Authorization by all legal next of kin, and have the cremation permit from the health department. After these documents are received by the crematory, then the deceased can be scheduled for cremation. Typical timing to receive cremated remains back is 4-7 days. Cardboard Cremation Container Temporary Urn Please Note: Prior to cremation, the deceased is placed in a corrugated container unless otherwise noted in the package, or you purchase/provide another combustible container for the body. After cremation, unless you purchase an urn from our funeral home, the Cremated Remains of the deceased will be returned in a temporary container which is a simple plastic container, with the remains in a bag.

At Sullivan Funeral Care you will always get the caring service you deserve at a price you can afford. Since 2014, our staff has been privileged to serve you, along with hundreds of other families of White County and surrounding areas. Our policy has always been to provide full information to the public concerning funeral services, arrangements and prices. To lose someone you love is a devastating human experience. It can be traumatic and seriously impact one s judgment. Therefore, it is important to be realistic with respect to funeral expenditures. We do not want your choices to be more than is affordable, nor, less than you would like to arrange. What is important is that arrangements are kept within your wishes and budget. We believe in the recognition and meaningful celebration of a life that has been lived and will assist in arranging services to meet your individual needs. In order to treat every family we serve equally and fairly, while ensuring our costs remain low and our business sustainable, we have set the same standard payment policy for each and every family served as outlined below. Confirmed provisions for Payment in Full is required at the conclusion of the arrangement conference or prior to scheduling services, submitting obituaries, or performing cremations. Financing by Lending USA o We offer our families the ability to pay for our services over time through monthly payments to Lending USA (with credit approval).
