2 Thank you... Dear family and friends, A funeral, memorial service, and visitation, is a time of reflection and celebration of a loved one. It is also an important time to feel the support of family and friends. Our professional staff is trained to work with you in designing a service that is as unique as the person you are honoring. When a death occurs, a family is faced with many decisions that need to be addressed in a relatively short period of time. These decisions can leave a family overwhelmed, especially with the emotional impact that they have just experienced. In this booklet you will find our price lists. We offer several choices to fit each family's needs. It is our desire for your family to understand all of your options. We feel the more informed families make better decisions. We have created different service options and offer a wide variety of merchandise. It is always our goal that level of personalized service, comfortable facilities and professional staff exceed expectations. 4 Funeral Expense Guidelines While your funeral director will guide you through the many decisions that will need to be made; this section will inform you on the three parts to a funeral contract and help you better understand the price lists on the following pages. The three parts to a funeral contract are: 1. Expenses associated with the funeral home services provided to your family. These charges are often referred to as service charges and include our professional fees, facilities, and transportation. 2. Expenses associated with the merchandise your family selects. This could include items such as a casket, outer burial container that the casket is placed in at the cemetery, clothing, register book package, etc. Please keep in mind that you will be required to pay Indiana s seven percent sales tax on these items. 3. Expenses associated with those people that play a role in the funeral service but don t work for the funeral home. These items are referred to as Outside Expenses or Cash Advance Items on the funeral contract and vary by each family s needs and wishes. These expenses might include the cemetery s fees to open and close the grave, florists, musicians, hairdressers, newspaper obituary fees, minister honorariums and any other people that a family might want to participate in the services. Be sure you fully understand all of your outside expenses as often times these items can add a significant amount to the total service costs.

5 Our Payment Policy The staff of Walton Funeral Service is committed to providing compassionate service to our area. We recognize that the families we serve may have had little time to prepare for the costs associated with the death of a loved one. Because of this, we offer several different payment options. Please indicate below which method of payment you will be using to pay for the funeral expenses for your loved one. I/We will pay the funeral expenses by cash or personal check. Note: An early payment discount may be given if payment in full is made by cash or check. I/We will pay the funeral expenses by credit card: MasterCard or Visa. I/We will pay the expenses using a Prepaid Funeral Insurance or Trust. I/We will pay the funeral expenses using an irrevocable assignment of proceeds of verified life insurance policy. I/We, as Responsible Party(ies), will complete and sign all insurance assignments forms prior to services. I/We will pay the expenses using a non-assignable verified life insurance policy. I/We agree to use the funds from (insurance company), policy # to pay the funeral expenses once I/We have received said funds. I/We will apply for an installment loan via a third party to pay for the funeral expenses. I/We will use the funds to pay for the funeral expenses once I/We have received the funds. Except in those cases where Prepaid Funeral Insurance or Trust, irrevocable assignment of proceeds of a verified life insurance policy, and/ or non-assignable verified life insurance policy is/are being used to pay the entire funeral expense, payment in full (or payment of the balance remaining after insurance) is required the day of the service. Should you need assistance with payment, please alert your Funeral Director immediately, so appropriate arrangements can be made. We appreciate the opportunity to serve you and your family.

6 Our General Price List These prices are effective as of January 1, 2018 and are subject to change without notice. The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Basic Services of Funeral Director & Staff $2, Our charge includes funeral director and staff to respond to the initial request for service, necessary arrangement conference with family or responsible party, preparation and filing of necessary certificates and permits, recording vital statistics, preparation of necessary notices, and coordination of service plans with parties involved in the final disposition of the deceased, e.g. clergy, cemetery personnel, crematory and others. Also included in this charge are overhead expenses relative to our facility such as insurance, maintenance and utility expenses, secretarial and administrative costs and equipment and inventory expenses. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials and forwarding or receiving remains.) Embalming $ Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming; you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. Other Preparation of Body: Disinfecting, Dressing, Cosmetizing, Grooming, Casketing $ Special Care for Organ & Tissue Donors $ Minimal preparation of remains $ Refrigeration of remains $ (Per day charge for unembalmed remains after 48 hours.)

9 Our General Price List Continued Forwarding Remains to another Funeral Home $3, This charge includes the basic services of funeral director and staff, transfer of remains to the funeral home, embalming, other preparation of the remains, necessary authorizations and local transportation to the airport. This does not include any visitation or service prior to forwarding the body, a casket, or a standard shipping container. (If no casket, add shipping tray $350.00) Receiving Remains from another Funeral Home $2, This charge includes basic services of funeral director and staff, pick up and transportation of remains from the airport to the funeral home, and graveside service package at the cemetery. This does not include any casket. Direct Cremation (range of prices) $2, to $2, Our charge for a direct cremation includes transfer of remains to funeral home, sanitary care of unembalmed remains, shelter of remains for 48 hours as required by law, use of crematory, and basic services of funeral director and staff. This charge does not include any visitation or ceremonies. If you want to arrange a direct cremation you can use an alternative container. Alternative containers encase the body and can be made of materials such as fiberboard or composition materials (with or without an outside covering). The containers we provide are made of cardboard and fiberboard. (48 hours of refrigeration included. Daily charge of $145 applies for each days thereafter.)
