2 GENERAL PRICE LIST *Prices are effective May 21, 2018 and are subject to change without notice. The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. *Certain funeral services may be provided off-premises by other funeral service providers. Basic Professional Services of Funeral Director and Staff $3,695 Our basic service charge includes but is not limited to the arrangement conference, coordinating service plans with cemetery, church, hospital or other institution, 24 hr. availability, preparation and filing of any necessary permits and certificates relevant to the funeral process, placement of death notices and obituaries, bookkeeping, accounting and other administrative expenses. This fee also includes overhead expenses relative to our facility such as wages, insurance, facility maintenance, equipment costs, utilities and government compliance. *This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. *This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains. *This fee is non-declinable. Embalming $1,195 Embalming is the process of chemically treating human remains to reduce the presence and growth of microorganisms, to temporarily inhibit organic decomposition, and to restore a natural physical appearance for funeral services within a reasonable period of time. *Required for services with an open casket, per Fairfax Memorial Funeral Home policy. *Embalming is not required by law. Embalming may be necessary, however if you select certain funeral arrangements such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. 3 Facilities, Staff, and Equipment Our funeral home and facility charges include, but are not limited to, placement of remains and set-up of service room, display of floral arrangements and religious equipment, supervision, coordination and direction of services and assistance of staff. 1. 5 Additional Services Forwarding Remains to Another Funeral Home (within the United States of America) $4,990 This charge includes basic services of funeral director and staff, embalming, dressing, casketing, grooming and cosmetology, transfer of remains to funeral home, and transportation to airport. *Does not include any funeral home facility and staff charges, merchandise, airfare, death certificates or cash advance items and services. *Any transportation in excess of 30 miles add $3.00 per loaded mile. *For International Forwarding of Remains: Add consulate courier vehicle and international shipping container. Receiving Remains from Another Funeral Home $3,145 This charge includes basic services of funeral director and staff, transfer of remains from airport, and funeral hearse for funeral services. *Does not include any funeral home facility and staff charges, dressing, casketing, grooming, cosmetology, merchandise, or cash advance items and services. Immediate Burial $3,145-$17,240 This charge includes basic services of the funeral director and staff, transfer of remains to funeral home, funeral hearse for transfer to cemetery. *Does not include any funeral home facility and staff charges, cemetery charges, burial vault, merchandise, or cash advance items and services. *Any transportation in excess of 30 miles add $3.00 per loaded mile. *The burial will be scheduled at the convenience of the funeral home and cemetery with no religious ceremonies or services. 1. Immediate Burial with a casket provided by purchaser $3, Immediate Burial with the use of any casket desired is this fee plus the casket price as selected by the purchaser (Please refer to our casket price list) $3,145 Direct Cremation $3,995-$18,090 This charge includes: basic services of the funeral director and staff, transfer of remains to funeral home, crematory fee, refrigeration up to 5 days. Daily fee will apply after 5 days. *Does not include any funeral home facility and staff charges, additional preparation of remains, merchandise, death certificates or additional cash advance items and services. *Any transportation in excess of 30 miles add $3.00 per loaded mile. *The cremation will be performed at the convenience of the funeral home with no religious ceremonies or services. *The cremation will be performed in approximately 3-5 business days if all authorizations and permits are received by the funeral home. *Additional days of refrigeration are $105 each day. *State law requires the legal next of kin or legal representative to sign a cremation authorization and the deceased remains to be positively identified before the cremation takes place. *State and local laws do not require a casket for direct cremation. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are heavy cardboard. 1.
