2 General Price List Prices are effective as of August 4, 2016 but are subject to change without notice. The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. You may choose only the items you desire. If you are charged for any items you did not specifically request, we will explain the reason for the charges on the written memorandum. Please note that there may be charges for items such as cemetery fees, flowers, newspaper notices, certified death certificates, police escorts, etc. SERVICES OF FUNERAL DIRECTOR AND STAFF: $ This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving of remains.) This charge is also included in the other packaged funeral offerings as allowed by law. This fee includes but is not limited to the following: Personnel available 24-hour hours per day. Availability includes that of funeral director and/or staff to respond to initial request for services, arrangement conference with family or responsible party, arrangement of service, tribute or memorialization. Staff to advice, assist and recommend appropriate personalization and individualization of the services to commemorate the unique life of your loved one. Obtaining and recording vital statistics and biographical information. Death certificate preparation, filing and obtaining necessary permits, authorizations, consents and assignments. Coordination with other parties such as hospital, doctor s office and staff, cemetery, crematory, clergy, and others involved in the services and disposition. Sheltering and care of decedent Also included in this charge are the overhead expenses relative to our facilities such as professional licensing, administrative services, insurance, maintenance, building, and utility expenses, taxes, legal and accounting fees, record keeping, equipment and inventory expenses. ADDITIONAL SERVICES OF FUNERAL DIRECTOR AND STAFF Use of Facilities and Staff for a Funeral Ceremony or Memorial Service at our Facility (per two hours). (This includes services Monday- Friday 10:00 a.m. to 4:00 p.m.) Our services include coordinating the funeral arrangements, supervision of funeral ceremony, display of floral arrangements, and staff to assist with the funeral ceremony. Additional time is available at the rate of $ per hour. Coordination or Direction of a Funeral Ceremony or Memorial Service at a Facility other than our Facilities (per two hours). Our services include coordinating the funeral arrangements, supervision of funeral ceremony, display of floral arrangements, and staff to assist with funeral ceremony. Additional time is available at the rate of $ per hour.

3 Equipment and Staff for a Graveside Service (per two hours). Our services include accompaniment of cremains to cemetery, supervision of graveside service, display of floral arrangements, and staff to assist with service. Additional time is available at the rate of $ per hour Coordination and Direction of a Simple Farewell Service for immediate family or friends. This takes place in a designated area at our crematory or funeral home for the intent of providing the opportunity to pay last respects to a loved one. Services include coordinating the arrangements, viewing of unembalmed remains, supervision of farewell ceremony, staff to assist with farewell ceremony. A $250 Setting of Features charge will be added to this cost so that mouth & eye closure procedures may be performed. Due to the nature of this intimate viewing time, this service is limited to no more than 10 family members and/or friends. $ Use of Crematory and Staff to Witness Cremation. This takes place in a designated area at our crematory for the purpose of witnessing the cremation. Services include coordinating the arrangements, viewing of unembalmed remains, and staff to assist with cremation procedures. This service is scheduled according to our crematory s availability and is limited to no more than 10 family members and/or friends. Use of Facilities and/or Staff for Viewing/Visitation (per two hours). Our services include setup of visitation area, placement of remains, display of floral arrangements, supervision of, and attendance during the visitation. Additional time is available at the rate of $ per hour. $ Use of Facilities and/or Staff for a Reception at our Facility (per two hours). (This includes receptions Monday- Friday 10:00 a.m. to 4:00 p.m.) Our services include coordinating the reception arrangements, supervision of reception, set up of the reception foods/beverages, display of floral arrangements, and staff to assist with the reception. Additional time is available at the rate of $ per hour. Use of Tahara Room $ Coordination and Direction of a Veteran s Honors Service at the DFW National Cemetery. This service offers Veterans families the opportunity to pay last respects to a loved one who qualifies for burial or inurnment in the National Cemetery. Services include coordinating the arrangements, obtaining the veteran s burial flag, coordinating military honors, supervision of the burial or inurnment, and staff to assist with the ceremony. Coordination and Direction of a Private Veteran s Inurnment Service at the DFW National Cemetery. This service includes a private and supervised inurnment of cremated remains by our Funeral Director. This allows Veterans who qualify for inurnment in the National Cemetery to be honored through that choice although family cannot be present for the inurnment. Services include coordinating the arrangements, supervision of the inurnment, and staff to assist with the ceremony. $ Immediately Expediting the Preparation and Filing of Necessary Certificates and Permits Needed for cremation or burial Shipping of Cremated Remains by North Dallas Funeral $ Delivery of Cremated Remains Locally by North Dallas Funeral Home (within 35 miles) Mailing Certified Death Certificates Per additional mailing address Delivery outside of a 35 mile radius is subject to an additional mileage fee of $2.25per mile thereafter one way. Embalming may be necessary however, if you select certain funeral arrangements, such as a funeral with a viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. 7 FILING OF LIFE INSURANCE CLAIMS Individuals wishing to use the proceeds of a life insurance policy to pay for funeral goods and services, in lieu of paying cash, may elect to have a third party assist you in the filing of a claim on behalf of the beneficiary (ies) of the policy. Please note that if you utilize a third party to file a claim seeking expedited receipt of life insurance proceeds on behalf of a beneficiary, there will be a fee to be paid associated with the filing of such a claim. Any such fees will be deducted from the proceeds of the life insurance policy. Filing Insurance Assignment - 8% of the total contract cost or a minimum of $ whichever is greater SPECIAL SERVICES Forwarding Remains to another funeral home This service includes removal of remains to funeral home, basic services of funeral director and staff, embalming or additional care of unembalmed remains, dressing and casketing, and transfer to common carrier within 35 miles of our facility. $1, Receiving Remains from another funeral home This service includes transfer from common carrier within 35 miles of our facility $ IMMEDIATE BURIALS This charge includes removal of remains within 35 miles, basic services of funeral director and staff, transportation to local cemetery, and sanitary care of unembalmed human remains. It does not include any visitation, services, or use of facilities. Immediate Burial with Casket provided by the Purchaser Immediate Burial PLUS Cost of Casket purchased through North Dallas Funeral Home $4, Range from $5, to $20, DIRECT CREMATION This charge includes removal of remains within 35 miles, basic services of funeral director and staff, refrigeration up to ten days, processing of death certificates and cremation permits, and cremation. This charge does not include any type of ceremony, viewing of remains, or any type of preparation made by North Dallas Funeral Home for the purpose of viewing. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are the minimum cardboard containers without outside covering or interior linings. Direct cremation includes reduced minimum services of funeral director and staff, transport from place of death to funeral home (within 35 mile radius), refrigeration, sanitary care of unembalmed remains, crematory fee, service vehicle, and processing of permits.
