The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Basic Services of Funeral Director & Staff... $1,815 The services of the funeral director and staff include, but are not limited to, 24-hour availability, consultation with the family to determine their wishes and acquiring the information needed to carry these out. It includes conducting the arrangements conference, planning the funeral, securing the necessary permits, preparing the notices, and coordinating the cemetery or crematory arrangements. Also included in this charge are overhead expenses relative to our facility such as insurance, maintenance and utility expenses, secretarial and administrative costs and equipment and inventory expenses. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is included in our charges for direct cremation, immediate burials and forwarding or receiving remains.) Embalming... $745 Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. Regulation (d) of the Kansas State Board of Mortuary Arts requires that a body that is not buried or cremated within 24 hours of death must be embalmed or refrigerated. Other Preparation of the Body... $245 This service provides professional cosmetology, dressing and casketing. Sheltering of the Remains... $245 This service is the sheltering of an unembalmed body in refrigeration for up to 72 hours. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The container we provide is made of fiberboard and wood. 7 Vaults In most areas of the country, state or local law does not require that you buy a container to surround the casket in the grave. However, many cemeteries require that you have such a container so that the grave will not sink in. Either a grave liner or burial vault will satisfy these requirements. 8 PROVISIONS FOR PAYMENT AND ESTATE CHARGES The death of someone you love, a devastating human experience, can impact judgment. We believe in the importance of celebrating the life of a loved one and will assist in any way we can in arranging an appropriate service to meet your individual needs. However, it is important to be realistic with funeral expenditures, as it is with any expenditure. We will strive to help you carry out your wishes within your financial means. Our provisions for payment are: 25% of total funeral bill or total of cash advances whichever is greater (to be paid at time of arrangement conference) Social security number of buyer is required if payment in full is not received at arrangement conference. Balance is due in 30 days. A late charge of 1.5% per month will be added to all balances not paid within 30 days from the date of death. If the funeral bill is not paid within 60 days from the date of death and an estate is filed, a $500 estate filing fee will be added to the contract at the time of filing the estate with the county. It is also understood that all financial arrangements are made with the buyer and not with the estate of the deceased. For your convenience, we accept cash, check, American Express, Visa, MasterCard & Discover Card. VARIABLE TERM PRENEED GUARANTEE If an individual chooses to pre-pay a funeral before need, Downing & Lahey offers a variable term preneed guarantee. Basic guarantee 1 year from time of contract (No extra cost) Good guarantee..up to 5 years (5% of services and merchandise cost) Better guarantee.up to 10 years (10% of services and merchandise cost) Best guarantee Lifetime (15% of services and merchandise cost) These extra funds go towards covering the increase in services and merchandise costs that will occur in the future. If the extra funds are not needed, they will be refunded to the estate of the deceased or to the state of Kansas if the deceased was on state assistance.
