2 Mark Holmes began the Holmes Funeral Home in 1925 in the north end of Manchester, relocating to the present location in In 1977, his sons Howard, Arthur, and Norman Holmes purchased the Watkins Funeral Home from the Watkins Family, maintaining the history of family-owned businesses and community involvement in Manchester. In 2017 the four members of the third generation: Scott, Howard, Richard, and Mark Holmes, have entrusted the continuation of Mark Holmes vision to Glenn Blumenstein and Brian Romanowski. They, along with a staff of licensed funeral directors / embalmers and administrative staff members, are always available for assistance and guidance before the death occurs, at the time of the death, and the days, weeks, and months following. We encourage you to call us with any questions or situation you feel we may be able to help you with. As our motto says, Where Personal Consideration is still a Tradition.

3 Payment Policy We understand that losing someone you love is an emotional and life changing experience. Consideration of any financial ramifications is very difficult. We hope to help you plan a Tribute in a way that is both meaningful to you and your family and that honors the life of your loved one. Payment prior to cremation or burial: Prior to the day of your loved one s cremation or interment we ask for a deposit. The deposit should be equal to the amount of cash advances and merchandise selected. Any balance will be billed to you at your home within 14 days. The balance will be due in 30 days. If for any reason you are unable to pay the balance in that time, late payment and interest charges will be applied. Some of the payment methods we accept include: 1. Cash, Check, Money Order, or Cashier s Check 2. Credit Card (Visa, MasterCard, Discover, and American Express) 3. Insurance Policy with the beneficiary s consent. The policy can be used as payment for the funeral expenses. Insurance assignment is acceptable if the following criteria is met: The policy is verified by us, and all documentation is signed preceding the funeral. The policy is assignable and payable directly to the funeral home. The policy must be non-contestable (if in the contestable period the policy will not be accepted as a form of payment). Pre-Arranged and Pre-Paid Funerals 4. In some cases, the deceased s estate must pay for funeral expenses, and in these instances we request that a family member take the financial obligation for funeral expenses and then file for reimbursement with the estate. 5. Financing is available with approved credit. As a final note, it is our policy to provide funeral services to all those within our community, and we do not wish for anyone to be limited by finances; if financial hardship is an issue, we ask that you contact us to speak more about the options that we can provide.

4 General Price List Prices are effective January 1, 2019 and are subject to change without notice. The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Holmes-Watkins Funeral Home is not a lender and we do not participate in any aspect of securing a loan on your behalf. We are not compensated in any manner and only provide you with information about qualified lenders as an accommodation only and at no additional charge. 5 Professional Services A. Basic Services of Funeral Director and Staff... $2, Our charge includes consultation with the family or responsible party; staff and facilities to respond to request for services, coordinating service plans with cemetery, crematory, and/or other parties involved in the final disposition of the deceased; securing official documents and necessary registrations; and sheltering of the deceased. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burial, and forwarding or receiving remains.) B. Embalming by Licensed Embalmer... $ Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. C. Other Preparation of the Body... $ Included are cosmetology, dressing, and casketing of the deceased. Additional Charges as Needed: Post Autopsy Care... $ Post Organ Donation charge.... $ D. Essential Care of Deceased When Not Embalmed... $ Includes use of refrigeration facility for up to 3 days. ($95.00 for each day thereafter) E. Bathing, disinfection, dressing, and casketing When Not Embalmed...$ Includes use of our facilities when family or chevra kadisha is involved. F. 7 Alternative Selections A. Forwarding of remains to another funeral home... $3, Our charge includes: basic services of funeral director and staff; proportionate share of overhead costs ($2,995.00); removal of remains, preparations of remains {including embalming}and local transportation. This charge does not include shipping casket or required shipping enclosure, viewing, air transportation, or funeral service. B. Receiving of remains from another funeral home... $1, Our charge includes; basic services of funeral director and staff; a proportionate share of overhead costs; care of remains; transportation of remains to funeral home and to the cemetery or crematory. It does not include viewing or funeral service. C. Direct Cremation (Total charge with all required items and cash advances)... $4, Our charges for a direct cremation (without ceremony) include removal of remains (within 3 hour total transfer time to funeral home. $65.00/hr after 3 hours), use of refrigeration facility, proportionate required staff services ($ ), acquiring necessary authorizations, service vehicle, transfer of remains to local crematory. If you want to arrange a direct cremation, you can use an alternative container at additional cost. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are constructed of cardboard with wood and plywood base and cost $ D. Immediate Burial... $3, Our charge for an immediate burial (without ceremony) includes removal of remains (within 3 hours total transfer time to funeral home. $65.00/hour after 3 hours), care of remains, basic services of funeral director and staff ($ ), necessary authorizations, service vehicle, transportation to cemetery within 50-mile radius of funeral home. It does not include viewing or casket.
