1 PRICE LIST December 1, 2018 Prices subject to HST except where indicated Basic Funerals is a fully licensed funeral home. All of our directors are certified and exceptional at what they do. What s different about our company is that we do not own our own chapel or visitation space. Reducing what we have to physically maintain and minimizing overhead costs allows us to reduce the fees we charge to you and all of the families we serve. Our company rents existing public locations across the province to host the services we provide. Our staff is trained to identify the needs of families and offer them options that suit those needs. We work with them to provide what they want and what their loved one would have wanted. Our flexibility allows us to provide meaningful service at an affordable cost. Whether we meet in person, converse through , or discuss over the phone, we work to provide constant and effective communication every step of the way.

2 Standard Service Charges Professional Fees Includes the coordination of the process from start to finish with every step either completed or overseen by a licensed funeral director. This includes coordinating with all related parties and completing the required funeral documentation. This fee is charged based on the postal code of the deceased s last known address. Includes up to 10 days of storage. $100

3 Service Package Fees Not For Profit Service Package This package is intended for individuals that do not plan on viewing their loved one. Cremated remains will be scattered at a Provincial Park in either the spring or fall following cremation. All services are still overseen by a licensed funeral director and come with the care and dignity that every family deserves. There will be 3 Proof of Death Certificates issued. Cremation will take place at one of our preferred crematoriums. Furthermore, Professional Fees are discounted with this chosen option. Budget Service Package Designed for families who want nothing more than the cremation process. This simple and easy service provides you with only what you need with no hidden costs or fees Included is a temporary container for the cremated remains, CPP forms, 5 proof of death certificates and 24 hour services and support. Cremation will take place at one of our preferred crematoriums. The turnaround time for this service will be about 7 days. Basic Service Package The most common package chosen by our families. Includes the simplicity and ease of our Budget package with the following additional items: a dedicated online obituary page, custom obituary template, unlimited Proof of Death Certificates, our Basic Executor Guide and pre-filled CPP forms. The turnaround time for this service will be about 5 days. Priority Service Package All of the benefits of the Basic Package as well as the following additional services: In person arrangement and completion of documentation, delivery of cremated remains at the end of the process, submission of obituary to newspaper(s)*, complete aftercare program to assist with estate settlement in person**. The turnaround time for this service will be about 3 days. *Newspaper publishing fees apply $0 $0 $395 $690 **If an in person meeting is not available in your city, the aftercare meeting may take place remotely. 4 Staffing Fees: Memorial & Funeral Services Staff for 1st Hour for Visit. or Service (incl. setup of venue) The arrangement of the venue or location prior to family arrival. Includes the first hour of staff. Charged per location and/or day. Not available in Budget. Staff fee for additional hour with casket Staff for each additional hour for visitation or funeral service. Staff fee for graveside attendance Staff presence and direction at the burial location. Does not include officiant. Staff fee for memorial with urn (up to 3 hours) Staff for a memorial service with urn present, for up to 3 hours. Staff fee for additional hour with urn Staff for each additional hour for gathering or memorial service. Staff for final viewing (up to 30 minutes) Staffing and coordination for the final viewing. Does not include crematorium reservation fees, or chapel rental, if applicable. Preparation fees are not included. Staff for closed casket witnessing Staffing and coordination required to witness the beginning of the cremation process. There is no option to view your loved one. Staff for witnessing and final viewing (up to 30 minutes) Staffing and coordination required to witness the beginning of the cremation process, and a final viewing. Staff fee for unattended burial of cremated remains Staff to bring cremated remains to cemetery for burial with no family present. Pictures of the burial may be ed upon request at no additional charge. Does not include cemetery property or opening/closing fees. Staff fee for committal services Staff to attend a graveside committal service before interring the ashes. Does not include officiant honourarium or cemetery opening and closing fees. 5 Body Preparation Dressing Only This service includes the dressing of your loved one. Does not include any further preparations, such as bathing or the application of cosmetics. Alternative Preparation (with shroud) Bathing, shrouding, and application of cosmetics if required. No chemical embalming takes place. 6 Additional Services Document or Clothing Pickup One of our staff will visit your home to pick up completed documents, clothing, etc. This does not include staff for the full arrangement process. In Person Arrangement A meeting with one of our funeral directors to go over all your options and complete the appropriate documentation. Included in Priority service package. Expedited Burial In the event of a burial being required within 2 days from the time of signing our contract, we will provide expedited service when possible. Expedited Cremation In the event of a cremation being required within 2-3 business days, we will provide expedited service when possible. Not available for Budget service package. Canada Post Delivery Delivery of cremated remains to any address in Canada through tracked ExpressPost. Tracking number will be provided. Delivery of Cremated Remains Delivery of cremated remains by our staff, within available areas in Ontario. Mileage One-way outside 30km. Refundable Deposit for Cremated Remains When human remains are to be cremated, a refundable deposit may be required to be paid at our discretion. If the cremated remains are not claimed within one year of the cremation, the deposit will be used to inter the cremated remains in common ground. We supply pictures of the scattering (if requested) and a Certificate of Cremation. Obituary Submission Assistance with a newspaper obituary submission*. Not available for Budget service package. Aftercare Specialist Support One hour in-person meeting* to walk through the toolkit and online assistance. One year of phone support. *In select areas meeting may be handled remotely if not reasonably feasible. Unlimited Funeral Director s Proof of Death Certificates These Proof of Death Certificates are supplied by Basic Funerals and can be used for most estate related matters within Canada. Staff for expedited certified copies of the Death Registration Staff, documentation, and necessary vehicles required to apply for expedited provincial certificates. 10 Estimate of Disbursements Disbursements are items arranged for and possibly paid for on your behalf by us. The following are some items that are often identified as disbursements. Cremation fee Varies based on crematorium selected, HST included. Coroner s cremation certificate (HST exempt) This is a fee charged by the coroner for issuing a certificate that allows cremation to take place. The fee is set by the Chief Coroner for Ontario. Death registration (HST exempt) This is a fee charged by the local municipality to register the death. The fee is set by each municipality and may vary. Chapel Fee Varies based on chapel selected, HST included. $565 $75 $55 ~$300/hr

11 Caskets - Adult Sized Cardboard Container (Basic 101)* Reinforced cardboard cremation container for transportation to the crematorium. *If weight exceeds 300lbs, a wooden casket tray will be required (see below) Over-sized Care For people greater than 300lbs. This fee includes everything that may be necessary to handle an over-sized body with care and dignity. Includes any additional staff needed for dignified care of the body as well as an oversized casket or tray for transportation. 12 Payment, Financing and Funding Payment Unless otherwise specified, payment is due upon signing of the contract. Payment may be made by cash, cheque, money transfer, and most major credit cards. Beyond the allotted time, interest will be charged at 1.25% per month (equivalent to 15% per annum). At Need funding For your convenience, we will waive any interest fees if we receive confirmation that payment will be expected from life insurance, or an account in the beneficiary s name. If finances are an issue, we also have contact information for community and city programs which can assist with the payment of funeral expenses. Pre funding For your convenience, we offer two funding options when prepaying services or supplies. When prepaying, you pay for the services and supplies today and the money will be used to pay for the services and supplies when they are delivered at the time of death. Trust Upon signing a prepaid funeral service contract, your money will be placed in trust by us, where it will accrue interest until the contract is fulfilled (when the services and supplies are delivered) or the contract is cancelled. Insurance Upon signing a prepaid funeral service contract, we will facilitate your purchase of an insurance or annuity contract where the proceeds of the policy will be used to pay for the services or supplies at the time they are delivered.
