Statements Mission In response to Christ's mission to help others, Compassionate Hands serves people in temporary crisis, by addressing immediate needs, fostering hope and guiding towards self sufficiency. Background Compassionate Hands was organized in 1993 by the Yukon Ministerial Alliance, business leaders and concerned citizens. Compassionate Hands reaches out to Yukon to strengthen cooperation in local mission work. Compassionate Hands serves Yukon as a clearing house, networking service and referral agency to provide services, support and to foster hope for self sufficiency. We assist with: housing/emergency shelter; utility cutoffs; prescription needs; food & clothing referrals; Personal/baby care items; gas to work or doctor; rent free wheelchairs/walkers & affordable handicap accessible van for area elderly and disabled residents. Impact Accomplishments for 2012: A Long Range Planning Committee was established to address programs & services. An auditor was secured for preparation of a 2012 audit. A Limited English Proficiency Plan was established with DHS Aging Services Dept. Goals for 2013: Audit (completed Feb 2/1/13) Needs Assessment for our community Program & Fundraising AssessmentsEmployee/Volunteer Manuals Needs Financing for our transportation program (Care-A-Van). Expenses of this program far outweigh income. There is no charge for the services, but if the client CAN afford to donate, we ask for $1 for transportation inside the city limits & $5 for outside the city limits. This program will cost approximately $36,372 for Volunteers to maintain facilities and yard. Volunteer to oversee recycling (cans). Volunteer to clean office and van. Volunteer Grant Writer. CEO/Executive Director Statement Compassionate Hands is a one of a kind agency in Yukon. We assist people in the zip code area and/or Yukon Public School District. There is no public transportation here in Yukon. So the transportation that we offer is very beneficial to the elderly and disabled. They rely on us to get them to the grocery store, doctors appointments, dialysis & other places as needed. Our services to the elderly and disabled are free, however, we do accept donations from our clients. We never refuse transportation because someone can't pay. Our van runs Monday - Friday, 8am - 4pm. Last year we transported 1,540 elderly and disabled passengers and traveled 28,015 miles. Board Chair Statement From the perspective of the Vice-Chair of Compassionate Hands, I note that Compassionate Hands continues to assist those people in temporary crisis by helping them pay rent, obtain temporary housing, buy groceries or gas and pay for their utilities. Of particular benefit is the crisis management counseling offered by our Executive Director. We are challenging ourselves to review the programs we offer to ensure we meet the needs of the people we serve. We are doing this through a needs assessment survey recently submitted to the first responders in the Yukon community. My interest in this organization derives from personal experiences I had while a single mother in the 1970's and 1980's. Although I have been happily married for 15 years, I still am mindful of the challenges experienced by single parents with limited resources. As an attorney with a bankruptcy practice, I see that a missed paycheck or an unexpected illness can be the first step in a downward spiral that leads to garnishments and bankruptcies. If Compassionate Hands can lend a hand in a crisis, the family can keep the heat on in the winter. Area Served Area Served 2

Our transportation program serves people that live in Yukon/Yukon School District but we will transport them to destination within a 30 mile radius. We transport residents that live in the zip code area and/or Yukon School District. The van will take passengers to desitination in surrounding areas (30 mile radius one way). It is our goal to transport as many clients as possible for as long as possible. Program Long-term Success Program Success Monitoring It is our goal to be able to extend services to 100% of those that request assistance. We would also like to provide transportation services to individuals that are attending the local Tech Center and Community College. There are a lot of people that would like to further their education but arent' able to because they don't have transportation to do so. I would like to hire someone to drive the van during the evenings to offer this program. We receive our van from DHS Aging Services. We report to them monthly regarding how many clients we serve, how many trips we make and how many miles we drive. We also report income & expense for the program. Compassionate Hands paid 17% of the cost of the van and DHS paid the remainder. The maintenance, insurance, payroll & any other expense incurred on the van is the sole responsibility of Compassionate Hands, Inc. 3

4 Examples of Program Success In 2012 we transported 1,540 elderly and disabled passengers and we traveled 28, 015 miles. We would not be able to transport many more clients because the van runs Monday - Friday, 8am - 4pm, with very little down time. Utility Description We provide assistance with Utility Cut-offs. Over the last few years our board has become more engaged in the future planning of Compassionate Hands. We have recently sent out a Needs Assessment to key people in our community that would be able to help us research the needs in our area. We are very anxious to compile the results and set Compassionate Hands on a chart to help more people meet their needs.
