The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. This list does not include prices for certain items you may ask us to buy for you, such as cemetery services, flowers and newspaper notices. The prices for those items will be shown on your bill or the statement of funeral goods and services selected. Professional Services BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF $2795 This charge includes funeral counseling, necessary arrangements, recording vital statistics, securing permits, filing and obtaining death certificates and other forms and claims, preparation of necessary notices, and coordination of service plans with parties involved in the final disposition of the deceased. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select (This fee is already included in our charges for direct cremations, immediate burials and forwarding or receiving remains). EMBALMING $795 Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. In most areas of the country, state or local law does not require that you buy a container to surround the casket or urn in the grave. However, many cemeteries require that you have such a container so that the grave will not sink in. Either a grave liner or a burial vault will satisfy these requirements. 12 FORWARDING REMAINS TO ANOTHER FUNERAL HOME $2295 Our charge for this service includes basic services of funeral director and staff, transfer of remains to funeral home, embalming, supervising and coordinating details; securing, preparing and filing necessary documents; clerical and record keeping, shipping container required by airlines and casket coach (hearse) for transportation to airport. This charge does not include viewing, ceremony or airline shipping charges. RECEIVING REMAINS FROM ANOTHER FUNERAL HOME $2595 Our charge for this service includes basic services of funeral director and staff, supervising and coordinating details; securing, preparing and filing necessary documents; clerical and record keeping, transfer of remains to the funeral home, and casket coach (hearse) for local transportation to local cemetery or crematory. This charge does not include visitation, rites or ceremonies. DIRECT CREMATION ($3720) Our charge for direct cremation without any attendant rites or ceremonies includes basic services of funeral director and staff, local transfer of remains to funeral home, care of remains including sanitary bathing & surface disinfection, transfer to crematory, crematory fee and Coroners Permit to Cremate. If you want to arrange a direct cremation, you can use an alternate container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are made of cardboard, fiberboard, or hardwood. Immediate Burial with Container Provided by Purchaser $3385 Immediate Burial $3385+ Casket Purchased from Funeral Home ($1095-$22395) INFANTS AND CHILDREN UNDER THE AGE OF THREE With respect to infants and children; it is our position that Anderson Funeral & Cremation Services will not charge for any professional staff, facilities or automotive equipment for infants and children under the age of three years old. The only charge will be for merchandise, cash advance items, or other items the family may wish the funeral home to provide. Veterans and First Responders Because of your service to our country and our community, we are pleased to apply a 10% discount on our Professional Staff Services, Facilities, and Automotive Equipment with Anderson Funeral & Cremation Services. Eligibility requirement: DD214 or EMS Credentials. We do not have sufficient financial resources to extend credit over a period of months or years, nor does law allow us to fulfill the functions of a bank or other lending institution. Therefore payment is expected at time of arrangements and due prior to services rendered. We encourage your frank discussion of these matters with your funeral director, and we will attempt to assist you in any way possible. Financing is Available For more information please contact our funeral home Assignment of Proceeds of a Verified Life Insurance Policy. Our acceptance of this mode of payment is dependent on the cooperation of the beneficiary and the insurance company. The insurance policy must be given to the funeral director for verification. We must have the Insurance Company s complete cooperation in providing information to the beneficiary, amount of the death benefit available, outstanding loans information, cooperation to get claim forms in a timely manner, and their acceptance of our written assignment. The insurance company will send the balance of any insurance proceeds directly to the beneficiary after the insurance company pays the funeral bill in full from the proceeds of the insurance policy. The purchaser must obtain a certified death certificate for each insurance policy. * Due to the complexity and time involved, we are unable to accept payment from estates. 13

Patricia has a very diverse background as a counselor that includes over 35 years in therapeutic settings with children, adolescents, adults, families, and couples in both individual and group counseling. She sees clients working on issues like depression, anxiety, life transitions, and grief and loss. Patricia has experience as a Registered Nurse, Teacher, Clinical Social Worker, and she holds a Ph.D. in Psychology. She has also completed two years training in Gestalt Therapy, and three years in Psychosynthesis. She currently maintains a Private Practice as a Psychotherapist and Family Counselor. 14

You have time to make your decisions. Ceremony A ceremony is the time for the community to be with you as you process memories of your loved one. It s the opportunity to celebrate the life that was created and lived. It s a time to share stories and remember, shifting the focus off of you and your loss and refocusing it on honoring the life that was lived. Final Goodbye A final goodbye to your loved one means they are absent in the body, but never absent in your heart or thoughts. Loss of their physical presence is the reality of what has occurred. 21
