2 2019 Wellness Program Enrollment and participation in the SWS-USA Wellness Program is an important part of achieving and keeping good health, but is also fun to do together with coworkers! We hope you will join us in the Wellness Program in 2019! We have some exciting events programmed, with benefits and awards for certain achievements and for meeting program eligibility requirements! The biggest award for participation, upon completion of eligibility requirements, is a 25% discount to your individual pre-tax contribution out of your paycheck towards your medical coverage! To be eligible for this discount and other awards, certain requirements must be completed. Please continue reading to see how you can receive a 25% discount on your medical contributions! 2

3 Eligibility Requirements Wellness Program Enrollment Form (Deadline 1/26/2018) NEW HIRES: Hired before Oct 1 st 2018: must complete ALL eligibility requirements Hired after Oct 1 st 2018: will be expected to do as much as possible in good faith, if signed up for the program, in order to receive the 2019 discount. COMPLETION OF THE 2018 ELIGIBILITY REQUIREMENTS: Will qualify you for the 2019 benefits discount. Future benefits discounts are contingent upon completion of the prior year s eligibility requirements. Those who sign up for the 2018 program but do not complete the eligibility requirements listed below will not receive the premium discount for Premium changes go into effect in January of each year. 2. Year-End Health Assessment (Must be completed Nov 1 st Dec 1 st ) -Health Assessments will be accepted NO LATER than Dec 1st -On-line instructions for completing: - -Click on SWS main tab -Click on Health Assessment -Password is swswiredup 3. Year-End Blood Work (Deadline Dec 1 st ) -Done on site Nov 1 st Dec 1 st -(Any blood work completed and submitted to One Stop Wellness January 1 st Dec 1 st, 2018 will be accepted for the Wellness Program). -Blood work can be submitted by or fax (onestopwellness@epbnet.com or fax to (888) ) 4. Two Qualifying Year-Long Events (Deadline November 1 st 2018) -Independent Programming (listed on page 3) -Group Programming (listed on page 3) 3

4 Qualifying Events & Guidelines 2 Qualifying Events Must come from the list below or be approved for independent SWS Wellness Programming. 1) Independent -Provide proof from your fitness center that you have worked out at least 24 times in an 8 week period. Most fitness centers will provide a print-out of your attendance upon request. -5K, 10K, ½ Marathon, Full Marathon, Triathlon/Iron Man, or Cycling Event Provide proof that you participated. -A physical team activity, such as hockey or softball, that is sustained for at least an 8-week period. Provide proof that you participated. See a Wellness Program Champion for any questions on what types of team sports qualify. -Tobacco Cessation Quit tobacco for a minimum of 6 months to receive credit. Simply communicate with your local champion that you have decided to quit and the method you are using. At the end report back once you have quit. You may be required to provide some form of proof showing the method of quitting. Each person s journey is very different. There is a 12-week self-directed program available on One Stop Wellness, 2) Group Any of the programming on the agenda for your location in 2018 *** A minimum of one group qualifying event will be required to receive your benefits discount in ***If in any way you feel you are unable to complete any of the qualifying events due to health issues or any other restrictions please contact One Stop Wellness and we are happy to provide you with replacement programming that meets your restrictions (onestopwellness@epbnet.com /fax: /phone: ). 4

5 Incentives Medical Benefits Co-Pay: 25% discount in 2019! FREE Blood Work! Health Assessment! It establishes a baseline of health that you can track from year- to-year! Health Coaching! An opportunity to privately discuss blood work or other health concerns/goals! Programming: there are weight-loss programs, tobacco cessation programs and various educational programs! Informative newsletters full of current health maintenance information! Targeting: take advantage of targeting your numbers. Maintain or improve them and you will be rewarded! Great supporting resources: local Wellness Champions are available to hear ideas and answer questions and One Stop Wellness is always available for coaching, personal programming and other important health information you may need! 5
