Trinity Health has partnered with RedBrick Health to provide state-of-the-art tools to support you in better understanding, maintaining and improving your health. Best of all, these resources are free as a part of our Live Your Whole Life initiative. Wherever you want to focus, Trinity Health and Saint Alphonsus are committed to offering you meaningful choice activities that reward you for working toward your healthy living goals. 3
3 At A Glance Steps to Take To Maintain Your 30% Lower Insurance Premium. Attention Colleagues and spouses/domestic partners on the Saint Alphonsus Aetna Health Plan. Follow these steps to reward yourself by lowering the cost of your medical benefits for focusing on your health. Note: if you need help or have questions about the Live Your Whole Life Health Plan Rewards, visit mybenefits.trinity-health.org/lywl or call (855) Additionally, Trinity employee steps to maintain a reward are different than those for Saint Alphonsus colleagues. If you are a Trinity employee, please refer to the Trinity booklet. Register for your online account With a few clicks, you ll have complete access to Live Your Whole Life. If you have questions, give us a call. mybenefits.trinity-health.org/lywl RedBrick Health Period 1: October 1, 2015 to February 1, 2016 Complete your health assessment and health screening for a snapshot of your health. The health assessment is a 15-minute questionnaire about your health to find your strengths and see where you can improve. Participate in a health screening to understand a few key health measures. Period 2: February 1 to April 30, 2016 Complete a meaningful choice activity. Options include: Redbrick Online Activities (health Journeys, challenges, trackers and telephonic health coaching). Corporate Wellness On-Site Activities (health coaching, weight management, MBSR, Financial Peace, etc). For a full list of Meaningful Choice Activities visit Period 3: May 1 to July 31, 2016 Complete a meaningful choice activity. See options noted above. 4 5
4 3 Opportunities To Maintain Your Lower Health Premium Reward you must complete the action in each period. Dates Action Needed Who Needs To Take Action Timing Period 1: October 1, 2015 to February 1, 2016 Complete your health assessment and health screening Colleagues and their covered spouse / domestic partner If you don t complete these steps, your reward of a lower premium contribution will end beginning with the pay period that includes March 1, 2016 Period 2: February 1 to April 30, 2016 Complete a meaningful choice activity Colleagues and their covered spouse / domestic partner If you don t complete these steps, your reward of a lower premium contribution will end beginning with the pay period that includes June 1, 2016 Online Health Assessments only take about 15 minutes to complete Period 3: May 1 to July 31, 2016 Complete a meaningful choice activity Colleagues and their covered spouse / domestic partner If you don t complete these steps, your reward of a lower premium contribution will end beginning with the pay period that includes September 1, 2016 Note: You will need to complete your health assessment and health screening if did not complete both in Period

6 Periods 2-3: Meaningful Choice Activities We understand that what works best for you might not work best for everyone. That s why we offer a variety of options for you to choose from to reward you for focusing on your health. Periods 2 and 3 Meaningful Choice Activities can only be met by the options listed below. Period 2 (2/1/16 through 4/30/16) Period 3 (5/1/16 through 7/31/16) If it is unreasonably difficult or medically inadvisable for you to attempt to participate in this program, contact RedBrick Health at to discuss your options before the end of each period. Meaningful Choice Activity - Wellness Offerings Provided by SAHS Corporate Health and Wellness. For more information or to sign up visit Personal Health Coaching Results-oriented program that enables employees to meet with a health coach in person to learn how to take charge of their health and make sustainable behavior modifications. Allows participants to address a wide range of topics including; stress, fitness, nutrition, use of tobacco and many more. LYWL Wellness Workshops A variety of workshops will be provided during periods 2 and 3 to support wellness. To count towards rewards, these workshops must be provided by LYWL. Quit with Nancy Tobacco Cessation Nancy provides outstanding, easily accessible and effective tobacco education and cessation program. Lifesteps Weight Management Led by a Dietitian trained in the Lifesteps program. LIFESTEPS provides strategies for successful, long term weight management behavior change. LIFE- STEPS assists adults in building healthy eating and activity habits. Number of Sessions to count as complete Complete 2 sessions per period. Complete 2 workshops per period Complete 2 out of 4 Complete 7 out of 15 Mode (health coach will come to your location). Meaningful Choice Activity Diet Free Healthy Eating and Physical Activity Whether you want to lose weight, improve energy, lower your cholesterol or blood pressure. The eight Diet Free health habits one at a time will change your life. Mindfulness Based Stress Reduction Learn how to replace stress-promoting habits with mindful ones a skill that will last a lifetime. Relieve the symptoms of stress and identify its causes so that you can start living a healthier, happier life. Financial Peace University Dave Ramsey s program on how to set and follow a budget, get rid of debt, and invest for the future. Wellness Offerings Provided by Redbrick For more information or to sign up visit mybenefits.trinity-health.org/lywl Telephonic Health Coaching Coaches will work with you over the phone during scheduled calls to answer your health questions, provide support in overcoming obstacles, and help set small goals. Journeys Journeys allow you to pick the healthy, inspiring steps that appeal to you, unlocking new levels of healthy habits as you go. Challenge Participate to boost your motivation and stretch beyond your usual routine. Look for opportunities in Track Track is a tool designed to help you build and reinforce good habits in just a few minutes each day. 7 Periods 2-3: Who is Eligible to Participate in Meaningful Choice Activities? Your personal information is private RedBrick continually updates our information systems to keep your data safe. Our privacy policy is based on these five basic principles: Those eligible to participate in wellness offerings at no cost include: All colleagues* Spouses on the Aetna health plan Local Trinity Employees are welcome to participate in on-site activities however they will not count towards Meaningful Choice Activities. * Colleagues not on the Aetna Health plan can only participate in wellness activities provided by the Corporate Health and Wellness department. 12 Individuals Not Eligible to Participate in wellness offerings include: Spouses not on the health plan are not eligible to participate. Dependents are not eligible to participate in offerings. Volunteers and contracted employees (ie: employed by STARS, Thomas Cuisine). 1. We will only disclose your health information to organizations that assist us in providing our services to you such as your care managers and physicians as part of a care management plan. Care managers may be provided through RedBrick Health, health plan administrators or clinically integrated networks. Outside of this purpose, your Protected Health Information (PHI) will not be shared with your employer without your consent. 2. Any access to and use and disclosure of PHI will comply with the regulations under the Health Insurance Portability and Accountability Act and any applicable state health information privacy and security laws. 3. Your information will never be sold or given to a third party for marketing purposes. 4. Your personalized programs and information are available to you through a secure, password-protected website. 5. Detailed personal information included in the PHI may not be used by your employer for any employment-related purposes. 8 Q: Why does my wellness participation impact my insurance premium contribution? A: As an organization, we are directed to be stewards of our resources. One of our most important resources is our health. To ensure high quality patient care, hospital staff members must stay healthy, energetic and engaged. We believe that better health, results in better healthcare. Q: Will I maintain my lower premium contribution if my spouse/domestic partner didn t complete the steps and I did? A: If the spouse/domestic partner is covered by the colleague s medical plan, then he/she must also complete the steps. Both the spouse/domestic partner and the colleague must complete the steps each period to maintain the lower premium contribution. Q: When would I receive my lower premium contribution? A: You start the year with it on January 1, 2016 and can maintain it by completing the steps for each period throughout Q: How is this different from last years insurance premium reduction? A: The new initiative will require three steps for all colleagues on the Saint Alphonsus Health Plan to maintain a 30% reduction in premiums: 1) an online Health Assessment (similar to last year but with a new vendor), 2) a Health Screening, and 3) Meaningful Choice Activities (online and in-person wellness offerings as provided by Trinity Health and Saint Alphonsus). 14 COMMON QUESTIONS & ANSWERS Q: What if I am newly enrolled in a the health plan? A: Anyone with a medical benefit effective starting after January 1, 2016 will not be required to participate in the Live Your Whole Life program for this plan year to maintain the lower premium contribution for medical coverage. Q: Can I go directly to a Saint Alphonsus lab to get my screening? A: No, screenings must be completed by one of the following Trinity defined choices: On-site Live Your Whole Life Health Screening Visit your healthcare provider and submit the completed Health Screening Form (you may use results from January 1, 2015 to February 1, 2016). Visit Labcorp and submit completed Health Screening Form Q: If I participated in a previous Live Your Whole Life screening can that count? A: The screening must take place after October 1, 2015 to count. Q: If I am not on the Saint Alphonsus or Trinity Health Plan do I need to do these activities? A: No, the reward design is specific to colleagues on the Health Systems Health Plan. Colleagues who are not on the health plan are still allowed to participate in on-site wellness offerings (excluding the screenings). Q: I participate in wellness activities that are not on the Meaningful Choice List, can these count? A: Trinity is very specific regarding what does or does not count as a Meaningful Choice Activity. The activity must be on the Meaningful Choice Activity List to count. Q: What if I do not want to participate? A: A colleague or spouse can chose not to participate, but they will not maintain their 30% lower premium. If it is unreasonably difficult or medically inadvisable for you to attempt to participate in this program, contact RedBrick Health at to discuss your options before the end of each period. 15
