1 STARTING YOUR JOURNEY No matter where you are on your journey at Houston Methodist, enrolling in your benefits is an important step. This Benefits Guide can help you evaluate your benefits and make decisions for you and your family. Whether you have been employed at Houston Methodist a while or you are new to Houston Methodist, the 2013 BENEFITS GUIDE can help. Employee Eligibility You are a benefited employee and eligible for Houston Methodist benefits programs if you have weekly Standard Hours of 20 or more and are not classified as temporary or PRN. Defining Standard Hours Standard Hours are the hours you are typically scheduled to work in a week (e.g. 20, 24, 28, 32, 36, 40, etc.) and exclude hours you may work due to overtime. When and Where to Enroll Make your 2013 benefit elections online within 30 days of your benefits eligibility date. To enroll, access the Houston Methodist Administrative Resources System (MARS) at work, home, or other locations. Where to Enroll for Retirement Benefits Please note that you may enroll for all benefits through MARS except for Retirement. Enroll in the 403(b) Plan at mysavingsatwork.com where you can also make your beneficiary elections for the DCP and 403(b) Plan. From a Houston Methodist Facility Go to the MARS link on the Houston Methodist Intranet. Use your Houston Methodist network ID and password to log in to MARS. Click on the Benefits Enrollment link. From Your Home You can access MARS on the internet at home or other non-houston Methodist location by going to mymethodistbenefits.com and clicking on the Enroll button (look for the orange mouse). Just use your ENROLL Houston Methodist network ID and password to log in to the site, click on MARS Portal, and follow the previously stated instructions. Additional Computer Access If you do not have a personal office or home computer, workstations are available in your HR Department and at various office locations. Ask your manager or HR Department where you may enroll. As a new hire or newly benefited employee, you only have 30 days to enroll. Enrollment Requirements Plan Medical, Vision, Dental, Health Care and Dependent Care Flexible Spending Accounts, and Legal Plan Basic Life and Accidental Death and Dismemberment, Paid Time Off, and Defined Contribution Plan Optional Employee Life, Optional Dependent Life, and Voluntary Accidental Death and Dismemberment Short and Long-Term Disability 403(b) Enrollment Requirement Coverage is effective on the date employed in a benefited position if you submit enrollment within 30 days of the benefits effective date. No action is required. You are auto-enrolled upon hire or entering into an eligible employee group. Coverage is effective on the date employed in a benefited position if you submit enrollment within 30 days of the benefits eligible date. You may enroll at any time after employment in a benefited position, but if you enroll after 30 days in Optional Employee or Dependent Life Insurance, you and/or your covered dependents will need to submit and pass Evidence of Insurability. No action is required. You are auto-enrolled upon your 91st day of employment or upon entering into an eligible employee group, whichever occurs later. You are eligible to enroll during the first week of employment in a benefited or non-benefited position. NOTE: You may change your contribution percentage in the 403(b) plan at any time. 2 Lead Your Life
2 How to Enroll Once on the Benefits Enrollment page in MARS, review your enrollment information. This page also displays information about your enrollment event, such as if you are a New Hire or enrolling during Open Enrollment. Click on the Select button to begin your enrollment, follow the instructions and make sure to submit your elections. You know you have successfully submitted when it shows Submitted under the Event Status on the Benefits Enrollment page. Twenty-four hours after you make your elections, you can print and your benefit choices from the Benefits Summary page. If you are a new hire or newly benefits eligible employee you only have 30 days from your eligibility date to make your benefit elections. Contact your HR Department or HR Benefits at or hrbenefits@houstonmethodist.org should you need assistance. Where to Go for More Information Use MARS job aids for step-by-step instructions on how to complete benefits enrollment and adding dependents and beneficiaries. Once logged into MARS, these job aids can be found by clicking on Main Menu, the MARS Training Classes folder, the Employee Self Service folder, and finally the Employee Self Service item. Making Changes to Your Benefits Some enrollments or changes can be made at any time, but most of our plans have strict legally required limitations on when coverage can be elected or changed. You may only make changes to your benefit elections for pre-tax plans (medical, dental, vision, health care flexible spending account, and dependent care flexible spending account) during an open enrollment period or if you have one of the following life events. Change in legal marital status (marriage, divorce, etc.) Change in number of dependents due to birth, adoption, placement for adoption, or death of a dependent Change in employment status of employee, spouse or dependent that results in gain or loss of coverage Change in residence of employee, spouse or dependent to or from the employer s health plan network service area Change in coverage due to and consistent with a court order which requires you to cover a dependent Changes in entitlement to Medicare or loss of Medicare eligibility Medical Plan premiums are higher for benefits eligible employees with weekly Standard Hours of 20-31, so if your Standard Hours change you may be eligible to elect a different medical coverage level or drop coverage. You have 60 days from the date of your Standard Hours change to submit your change. Significant change in cost of coverage* Change in coverage of employee, spouse or dependent under another employer s plan Commencement or return from leave under FMLA, or an unpaid leave that affects eligibility for coverage Change in Medicaid, Children s Health Insurance Program (CHIP) or State Premium Assistance Eligibility/Coverage for an employee or his/her eligible dependent who: Loses Medicaid or CHIP coverage because he/she is no longer eligible, or Becomes eligible for a state premium assistance program under Medicaid or CHIP (such as the Health Insurance Premium Payment Program in the state of Texas). * Includes an increase or decrease in premium due to a change in your weekly Standard Hours (32+ to or vice versa) or from a change in your requirement to pay a spousal surcharge due to your spouse s change in eligibility for his/her employer s plan. If you have a life event, you may only submit changes to your pre-tax plans that are consistent with the event. You must submit changes in MARS that require a change in coverage level and/or a change in your contribution amount within 60 days of the date of the event. If your change is not received within the required timeframe, you will not be able to make the change until the next open enrollment period, with the exception of an ineligible dependent who must be dropped from coverage effective the date the dependent is no longer eligible. It s important to remember that all benefits are effective your first day of eligibility or date of hire. Premiums are retroactive back to the beginning of the pay period of your effective date. Establish Life and AD&D beneficiaries immediately upon initial enrollment so that the rest of your benefit elections will not be delayed. Lead Your Life 3

Heathy Directions Programs Kicking the Tobacco Habit Getting Fit Eating Right Shedding the Pounds Relieving Stress How to Lower Your Medical Plan Deductible If enrolling in the Medical Plan, you and your covered spouse must both do the following to qualify for the lower Medical Plan deductible: Complete the HealthQuotient (HQ) and Biometric Screening. Meet tobacco/nicotine-free status by screening negative for tobacco/nicotine use.* Your in-network deductible for the Choice and Choice Plus Plans will reduce to $250 per individual and $500 for your family. Your out-of-network deductible for the Choice Plus Plan will reduce to $500 per individual or $1,000 for your family. If you are a new hire, newly benefited employee or had a life event, you will have 60 days from your effective date to complete the HQ and Biometric Screening. *If it is unreasonably difficult for you or your spouse to be tobacco/nicotine-free, an alternative lower Medical Plan deductible qualification may be available. Staying Healthy Growing Family Managing Health Conditions Spouses who reside more than 50 miles from a Houston Methodist facility or who will be out of the area more than 30 days during the 60 day time period can visit any physician and have them complete a Healthy Directions Offsite Biometric Form. Qualify for the Lower Medical Plan Deductible Three Simple Steps for You and Your Covered Spouse Step 1: Register with WebMD COMPLETE YOUR 1. Go to mymethodistbenefits.com and click the WebMD icon 2. Select Register Now and follow the registration steps Note: Your Registration ID is your Employee ID from MARS. Your Spouse s Registration ID is composed of the following 13 characters: Last 5 characters of your spouse s Social Security Number, plus Your spouse s date of birth (MMDDYYYY) Example: Jan. 1,1980, entered as Once you have completed the registration steps, click Submit! HQ Healthy Directions Portal, powered by WebMD Registering for the Healthy Directions portal, powered by WebMD, can help you and your spouse take control of your health. Here, you can find the most current, trustworthy, and easy-to-understand health and medical information; develop a personalized health improvement plan; and access the resources both you and your family need to make smarter choices about your health. Step 2: Complete the HealthQuotient Disponible en español Next, complete the HealthQuotient (HQ). It s located in the Quick Links section on the left after you log on to WebMD. Simply answer a few questions about your current health status and lifestyle. Within moments you ll receive a detailed, easyto-understand report. Your personalized HQ report includes: Health summary with prioritized results Customized plans to help reduce or eliminate risk factors Compelling what if scenarios showing the impact of lifestyle changes Important! As you complete the HQ, you ll have the opportunity to enter your most recent Biometric Screening data. Please note that entering this data does not qualify as taking the Biometric Screening. Only Biometric Screening data collected by Wellness Services staff at Houston Methodist will count towards the requirements. Step 3: Complete your Biometric Screening and meet tobacco/nicotinefree* status Complete a Healthy Directions Biometric Screening as part of the requirements to qualify for the lower Medical Plan deductible. Visit mymethodistbenefits.com to find the current Biometric Screening schedule. If you are a new hire, newly benefited employee or had a life event, you will have 60 days from your effective date to complete the HQ and Biometric Screening. Once you complete a Healthy Directions Biometric Screening, your HQ will be updated with your results usually within 10 business days. This will provide you with the most current snapshot of your health. *If it is unreasonably difficult for you or your spouse to be tobacco/nicotine-free, an alternative lower Medical Plan deductible qualification may be available.
