1 Access Ultipro, choose Myself, choose Open Enrollment ULTIPRO 2018 OPEN ENROLLMENT GUIDE TO REVIEW AND CONTINUE BENEFITS FOR 2018 To continue the same benefits into 2018, click on the Stay enrolled in current benefits plan button. Then click OK. Review 2017 benefits then print and/or close.
2 TO MAKE BENEFIT CHANGES FOR 2018 To make changes to your benefits enrollment for 2018 click on the Make new elections button. Then click OK To navigate through the session use the next button or using your mouse click on an option from the menu on the left hand side of the screen. Open enrollment is the time to update your dependent and beneficiary information. Be sure to review the data for each dependent listed by clicking on their name.
3 To edit dependent information click on the edit button. If you need to delete the dependent then click on the delete button. If the dependent is a current dependent update the Social Security Number, Date of Birth and Address (if different than your address) Review and update the designation as Dependent, Beneficiary or Emergency Contact. Note: It is important to designate or add beneficiaries at this point so you can assign them to benefit plans during the enrollment session. Designations cannot be removed in this screen unless the person is not assigned to any current enrollments. Dependents will not be eligible for enrollment without a social security number and date of birth. When finished, click on the save button. Repeat the process to edit the information for all dependents listed.
4 ADDING A DEPENDENT/BENEFICIARY To add a dependent or beneficiary click on the add button Complete all the required information for the new dependent. Click on the save button. REMOVING A DEPENDENT/BENEFICIARY Dependent or beneficiary designations cannot be changed in the Verify Beneficiary and Dependent Information screen unless the person is not attached to any current benefit plans. Removing these designations must be done within each plan during the enrollment process.
5 Each plan that requires dependents or beneficiaries will have a listing within the plan. Dependents and beneficiaries that need to be removed going forward should not have a check mark in the box next to their name. These changes will take effect on 1/1/2018 MEDICAL Continue the enrollment session by choosing the Medical option from the left hand menu. Review the plans available to you.
6 Choose to decline the plans if you do not want to have medical coverage. If you want to enroll in medical coverage click on the button next to the plan you want to enroll in and then click on the button next to the coverage level from the list of options. Check the dependents you would like covered on your medical plan. Complete the same process for DENTAL and VISION plans.
7 HEALTH SAVINGS ACCOUNT (HSA) This section is to enroll in the option to have deductions set up to be deposited into your Health Savings Account (HSA). Decline this coverage if you: w Did not enroll in a medical plan with an HSA Do not want to set up HSA deductions If you did enroll in a medical plan with an HSA option and do want to set up deductions Choose the option that matches your medical plan coverage Single for employee only, Family for coverage of you plus any dependents For employees 55 and over who want to be able to contribute the catch up contribution choose the Catch Up option Choose the option first and then choose contribution per pay check or annual contribution. Enter the amount you want to contribute.
8 FLEXIBLE SPENDING ACCOUNT (FSA) MEDICAL This section is to set up a Flexible Spending Account (FSA) to pay for eligible medical expenses Decline this coverage if you: w To Enroll: Enrolled in a high deductible health plan with HSA Do not want to set up a Flexible Spending Account for 2018 w Choose the FSA Medical option first and then choose contribution per pay check or annual contribution. Enter the amount you want to contribute. FLEXIBLE SPENDING ACCOUNT (FSA) DEPENDENT CARE This section is to set up a Flexible Spending Account (FSA) to pay for eligible dependent care expenses Decline this coverage if you: w Do not have dependents Do not want to set up a Flexible Spending Account to pay for dependent care expenses for 2018 To Enroll: Choose the FSA Dependent Care option first and then choose contribution per pay check or annual contribution. Enter the amount you want to contribute.
9 COMPANY PAID LIFE/AD&D This is a company paid plan and can t be declined. It is included in the enrollment session to have beneficiary information designated to the plan. Choose the button next to the plan name Put a check mark next to the name of the beneficiary that you want to designate as a beneficiary on this plan. If you need to add a beneficiary to this plan use the menu on the left of the screen to choose Verify Beneficiary and Dependent Information. Click on the Add button. Add/Change Contact: Enter the first name and last name for the new beneficiary. Choose the relationship from the drop down menu. Put a check mark in the box to designate the addition as a beneficiary. Return to the COMPANY PAID LIFE/AD&D section. The new beneficiary will be listed in the enroll beneficiaries section. Designate the primary or secondary percentage for each of your beneficiaries.
10 COMPANY PAID DISABILITY LONG TERM (LTD) SHORT TERM (STD) These options are company paid options. In each of these sections click on the button next to the plan option. EMPLOYEE VOLUNTARY LIFE 100% Employee Paid Either Decline Employee Voluntary Life or choose the plan and enter the benefit amount you want to submit for approval. Amounts applied for during open enrollment are subject to underwriting approval and will require an Evidence of Insurability form to be completed.
11 EMPLOYEE VOLUNTARY AD&D 100% Employee Paid Either Decline Employee Voluntary AD&D or choose the plan and enter the benefit amount. SPOUSE VOLUNTARY LIFE 100% Employee Paid Either Decline Spouse Optional Life or choose the plan and enter the benefit amount you want to submit for approval. You must be enrolled in Employee Voluntary Life to request coverage for your spouse. Spouse coverage can be up to 100% of the Employee Voluntary Life Coverage. Amounts applied for during open enrollment are subject to underwriting approval and will require an Evidence of Insurability form to be completed.
12 SPOUSE VOLUNTARY AD&D 100% Employee Paid Either Decline Spouse Voluntary AD&D or choose the plan and enter the benefit amount. You must be enrolled in Employee Voluntary AD&D in order to enroll in Spouse Voluntary AD&D. CHILD VOLUNTARY LIFE 100% Employee Paid Either Decline Child Voluntary Life or choose the plan and enter the benefit amount you want to submit for approval. You must be enrolled in Employee Voluntary Life to request coverage for your child or children. Amounts applied for during open enrollment are subject to underwriting approval and will require an Evidence of Insurability form to be completed.
13 CHILD VOLUNTARY AD&D 100% Employee Paid Either Decline Child Voluntary AD&D or choose the plan and enter the benefit amount. You must be enrolled in Employee Voluntary AD&D in order to enroll in Child Optional AD&D. ALLSTATE GROUP ACCIDENT If you are not going to enroll in Allstate coverage choose to Decline the Allstate Group Accident plan option. To Enroll: Click on the Allstate Group Accident Plan Click on the Option for the coverage you are electing If you are choosing the employee + children, + spouse or + family option the enroll dependents box will open and allow you to choose the dependents to be covered on this plan.
14 ALLSTATE GROUP CRITICAL ILLNESS If you are not going to enroll in Allstate coverage choose to Decline the Critical Illness coverage. Note: You will need to decline on each of the 4 levels of coverage. To Enroll: Click on the Allstate Group Critical Illness Plan for the option of coverage that you are electing. If you are choosing employee + children, + spouse or + family coverage a box will open to choose the dependents to be covered on this plan. ALLSTATE GROUP HOSPITAL If you are not going to enroll in Allstate coverage choose to Decline the Allstate Group Accident plan option. To Enroll: Click on the Allstate Group Hospital Plan Click on the Option for the coverage you are electing If you are choosing employee + children, + spouse or + family coverage a box will open to choose the dependents to be covered on this plan.
15 PREPAID LEGAL ARAG If you are not going to enroll in ARAG coverage choose to Decline the Prepaid Legal ARAG coverage. To Enroll: Click on the Prepaid Legal ARAG Plan. ALLSTATE CERTIFICATION To enroll in any of the Allstate Voluntary Plans (Critical Illness, Hospital Indemnity or Accident Insurance) you must choose the Allstate Certification option. Click on Read More to read the statement. Choose Yes If you are not going to enroll in Allstate coverage choose to Decline the certification option.
16 ALLSTATE HEALTH COVERAGE QUESTION To enroll in any of the Allstate Voluntary Plans (Critical Illness, Hospital Indemnity or Accident Insurance) you must choose the Allstate Health Coverage Question. Click on Read More to read the statement. Answer the question by choosing the Yes or No option. If you are not going to enroll in Allstate coverage choose to Decline the Health Coverage Question option. ALLSTATE TOBACCO CERTIFICATION To enroll in any of the Allstate Voluntary Plans (Critical Illness, Hospital Indemnity or Accident Insurance) you must choose the Allstate Tobacco Certification Question. Click on Read More to read the statement. Answer the question by choosing the Yes or No option. If you are not going to enroll in Allstate coverage choose to Decline the Tobacco Certification plan option.
17 PAY TAX ON LONG TERM & SHORT TERM DISABILITY You have the option to pay tax on this company paid benefit now or when you file a claim for Long Term and Short Term Disability Benefits. If the answer is No, choose to decline this benefit If the answer is Yes, click on the button next to the question Click on Yes in the Options section
18 Click on Confirm Your Elections or Changes In the example you can see highlighted in blue is action that needs to be taken to submit your benefit elections. The area will show information that needs to be updated until all entries have been made. Return to each item that needs action by choosing it from the menu on the left hand side of the screen. After completing all the actions required in the information section you will see the confirmation of your benefit elections. Click on Confirm Your Elections or Changes In the example you can see highlighted in blue is action that needs to be taken to submit your benefit elections. The area will show information that needs to be updated until all entries have been made. After completing all the actions required in the information section you will see the confirmation of your benefit elections. When done confirming your elections click on the submit button. Note: The Estimated Total Cost only includes elections made during this session.
