1 UMPHYSICIANS 2018 OPEN ENROLLMENT FREQUENTLY ASKED QUESTIONS Open Enrollment What is Open Enrollment? Open enrollment refers to the specific time each year when you are eligible to enroll in or change your benefit plans without needing to qualify for a special enrollment period. Open enrollment occurs once per year, so you ll want to be aware of open enrollment dates so as to avoid a lapse in coverage. When is Open Enrollment and when do my new elections take effect? The Open Enrollment period begins October 25 and ends on November 8, All changes to benefits become effective January 1, Your new 2018 benefit premiums will be reflected on your first paycheck in January. Do I need to take action during Open Enrollment if I m satisfied with my current elections and do not plan to add new dependents? Yes. You MUST enroll for medical, dental, vision, flexible spending accounts and health savings account. If no action is taken, you will NOT have Benefits (medical, dental, vision, flexible spending accounts or HSAs) in Can I add or remove my eligible dependents during Open Enrollment? Yes, you may add or remove eligible dependents during Open Enrollment. The effective date of any change is January 1, How do I make my Open Enrollment elections?. Follow these simple steps to make your Open Enrollment elections. Access Workday at Go to your Inbox, and click on the Open Enrollment Change action. Please read online instructions carefully (at the top of your screen) before you make your benefit elections When you have completed your elections, please review your summary of elections for accuracy and keep a copy for your records. What if I am unable to submit my elections during the designated Open Enrollment period? Changes to benefits can only be made during the designated Open Enrollment period. Changes will NOT be allowed after Open Enrollment and your 2017 benefits (for medical, dental, vision, HSA, and FSA) will NOT carry over into 2018.

2 Do I need to re-enroll for my 401(k) at this time? No, 401(k) election changes are not part of the annual Open Enrollment process. You can change your 401(k) election at any time. Log onto Do I have to re-enroll for Transportation Plans at this time? No. Do I have to re-enroll for company paid benefits, like Basic Life Insurance, Short-Term Disability and Long-Term Disability? No. Why do I need to enter Social Security numbers for my dependents? The Centers for Medicare and Medicaid require that health plans collect this information. Can I enroll for Supplemental Life Insurance and Accidental Death and Dismemberment? Yes, you can enroll in Supplemental Life Insurance plans (including spouse and children coverage), but you will be asked to submit Evidence of Insurability. After you enroll, you will receive and and letter from UNUM. Once UNUM reviews your Evidence of Insurability information, they will determine if you are able to enroll/increase coverage for Supplemental Life. If you currently have less than $150,000 coverage for yourself, or $20,000 for your spouse, you may be able to increase your coverage without Evidence of Insurability. For you, an increase in coverage of up to $20,000 will not require Evidence of Insurability for amounts up to $150,000. For your spouse, you may increase coverage of up to $10,000 without Evidence of Insurability for amounts up to $20,000. Are premiums increasing in 2018? No, this is the 3 rd consecutive year that UMP has not given a rate increase in biweekly premiums. Do I need to enroll at MyplanbyMedica for my 2018 medical plan and HSA? No, enroll for these benefits in Workday. Who do I contact if I have questions? You may contact UMP Benefits Monday thru Friday from 8am-5pm at extension 3 or benefits@umphysicians.umn.edu. New Hire I am a new hire and my start date falls during the Open Enrollment period. Can I participate in Open Enrollment? Yes, new hires may participate in Open Enrollment. However, it is important that you FIRST, submit your new hire elections (for 2017) as soon as possible before your new hire election deadline. Once your new hire elections are submitted you can proceed and make your Open Enrollment elections. Once you have completed your new hire elections, return to the Workday to make your Open Enrollment elections. If you are unable to complete your 2018 elections by the deadline, please contact benefits@umphysicians.umn.edu to request a special grace period.

3 General Questions Are there changes to the UMP Medical Plans for 2018? Please refer to the 2018 Employee Benefits Information guide for information on the plans being offered. In addition to reducing the number of deductible options, we are happy to announce two new enhancements as covered services: Infertility coverage is enhanced to include $5,000 annual benefit for diagnosis and treatment and $5,000 annual benefit for related drugs. Weight Loss Surgery is now a covered benefit. Why do I have to re-enroll each year? Health care needs may change from year to year. Some plans have been eliminated and a few new ones have been introduced. It is always a good idea to review your options and make your purchasing decision as if you were a consumer making a purchase. Who is eligible for UMP Benefits? UMP offers coverage for regular full-time (work hours per week) and part-time (work hours/week) US employees. You may also enroll your spouse and children in certain plans. Only children to age 26 are eligible for coverage unless your dependent is handicapped. A special request can be made to Medica for exceptions to the age 26 rule. Can I enroll from my computer at home? Yes. Visit to enroll in coverage from any computer. For Workday log in assistance and password resets, contact the UMP IT Service Desk at , or support.ostmgds.com. How Do I Find Out What My Current Benefits Are Today? Go to the Benefits Dashboard in Workday (click on the Benefits Worklet) Navigate to the View/Change Benefit Enrollment Section Click on the Benefit Enrollment As Of Date Enter the Current Date Click OK in the bottom left of the screen You should now see your Current Benefits Can I make changes to my benefits after open enrollment? No, you cannot make changes to your medical plans unless you experience a Qualified Life Event, Qualifying Life Events include: Marriage/Divorce/Legal Separation Birth/Adoption/Death of a dependent You or your eligible dependent loses or gains other coverage Change in FTE status from Full time to Part time (or vice versa) Will I get new medical, dental and vision insurance cards? If you enrolled in medical insurance you will receive a new insurance card. If you do not make any changes to your current dental plan enrollment, you can continue to use your current Delta Dental insurance card. VSP, UMPs Vision Provider, does not issue insurance cards. As a result, be sure to inform the provider you have VSP coverage and they can look you up in their system.

4 Where can I find out more information about the Vision Plan through VSP, Identity Theft Protection coverage through ID Watchdog, and Bright Horizons Back-Up Care? Both resource and Workday have additional information about all of these programs. Navigate to Human Resources, Benefits, and Open Enrollment on resource to find additional information. Or click on the Employee Benefits worklet in Workday to see links and useful tools. Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA) You must meet the following eligibility requirements to qualify for an HSA: You must be enrolled in a High Deductible Health Plan (HDHP) You cannot be covered by another healthcare plan that is not an HDHP You cannot be claimed as a tax dependent on someone else s tax return You cannot be enrolled for Medicare What expenses are eligible for reimbursement from my FSA or HSA? You are eligible to receive tax-free reimbursement for qualified healthcare expenses, including dental and vision, not covered by your insurance. A list of these expenses is available on the IRS website, Can I change my Health Savings Account (HSA) contribution during at any time during the plan year? Yes. You can prospectively start, stop or change your contribution amount at any time. You may change your HSA election in Workday. What about healthcare expenses for my dependents who are not covered by my UMP HDHP Plan? You can use your HSA to pay for qualified healthcare expenses for your qualified dependents even if they are not covered by your UMP HDHP. What are the HSA Plan limits for 2018? $3,450 for Employee Only coverage and $6,900 for Family coverage (including Employee + 1). If you are 55 or older, you can add $1,000 to the limits. Can I open an HSA if I am over 65 and NOT enrolled in Medicare? Yes Can I contribute to a HSA if I am enrolled in Medicare Part A, B, or D? No, if you are enrolled in any portion of Medicare, you are not eligible for a HSA. Can I use the money in my HSA to pay for healthcare expenses which incurred prior to opening my HSA? No. Qualified expenses must be incurred after the HSA is established.

5 How much does UMP contribute toward my HSA? For single coverage - $750/annually ($28.85 each pay period) For employee + 1 or family coverage - $1,250/annually ($48.08 each pay period) What is a Flexible Spending account? A flexible spending account allows you to reimburse yourself (with your own money) for eligible Health Care and/or Dependent Care expenses - tax free. By participating in these accounts, you do not pay federal, state, or city taxes on the money you contribute. Participation is voluntary and employees must sign up during open enrollment or within 31 days of hire. There are two types of Flexible Spending Accounts you may elect: FSA Health Care used to reimburse you for out-of-pocket health care expenses not covered under your health insurance plan, including prescription medications for you and your eligible dependents. Dependent Care FSA Dependent Care used to reimburse you for out-of pocket expenses for dependent care expenses, whether for a child, a spouse or dependent who is not physically or mentally able to care for themselves, or an elder. This includes expenses for someone else to care for your dependent (under the age of 13 for dependent children) so you may work. What happens if I have money left in my FSA at the end of the plan year? Any unused funds will be forfeited per Internal Revenue Service (IRS) guidelines. That is why it is very important to plan conservatively. If you do not apply for reimbursement of your FSA account by March 30 for your previous plan year's dollars, you lose the total amount left in your account. When are my funds available for reimbursement for my eligible expenses? FSA - Health Care: Your total annual election is available to you for reimbursement of eligible health care expenses, including prescription medication incurred after the effective date of coverage. You do not have to wait for the contributions to accumulate in your account. You can use it to pay for your eligible health care expenses from your first date enrolled in the account. FSA - Dependent Care: Funds are available as they are withdrawn from your paycheck. Unlike the FSA Health Care election, the entire maximum annual amount is not available during the plan year, but rather after your payroll deductions are made. You must incur services before submitting for reimbursement. What is are the FSA limits for 2018? $2,600 for Healthcare FSA, and $5,000 for Dependent Care FSA.
